Transient left bundle branch block associated with very high coronary artery calcium: a case report
We present a case of very high CAC in a 64-year-old woman with hypertension, type 2 diabetes, and hyperlipidemia presenting with dyspnea. Initial electrocardiogram (ECG) demonstrated normal conduction with a heart rate of 76 beats/min, but new T-wave inversions in V1-V4 and a high-sensitivity troponin-I (hsTnI) value of 6 ng/L (normal < 6 ng/L). Obstructive jaundice as  a rare complication of multiple pancreaticoduodenal artery aneurysms due to median arcuate ligament syndrome: a case report and review of the literature
ConclusionIn unexpected obstructive jaundice due to a mass with vascular origin in the head of the pancreas, PDAA should be considered, and celiac trunk should be evaluated because the main reason for PDAA is celiac trunk stenosis or occlusion by atherosclerosis or MALS. The treatment method chosen (including transarterial embolization, open surgery, or combined method) depends on the patient's clinical status and radiological findings, but transarterial embolization would be safer and should be used as a first-line method.

DISCUSSION:The administration of opioid analgesia prior to cath in patients with concern for ACS is associated with longer door-to-balloon times, and greater peak troponin levels. The rate of Occlusion MI in those who received pre-cath opioids was double the rate of those without pre-cath opioids, and STEMI(-) OMI patients who received pre-cath opioids waited 10 hours on average longer to get cath than those who did not received opioids.
