Ludwig's angina and acute myocardial infarction: A case report
Visible anterior neck swelling. Ultrasound scan shows extensive submandibular abscisic mass. AbstractLudwig's angina was first described in 1839 by German physician, Wilhelm Frederick Von Ludwig as a rapidly and fatal progressive gangrenous cellulitis and edema of the soft tissues of the neck and floor of the mouth with rapid spread to other places like anterior mediastinum. However, Type 2 acute myocardial infarction (MI) due to Ludwig's angina has not been documented. A 62-year-old male presented to the emergency department with visible anterior neck swelling for 1  week, which was preceded by a tooth arch 1 week pri...

Role of computed tomography cardiac angiography in acute chest pain syndromes
Use of CT coronary angiography (CTCA) to evaluate chest pain has rapidly increased over the recent years. While its utility in the diagnosis of coronary artery disease in stable chest pain syndromes is clear and is strongly endorsed by international guidelines, the role of CTCA in the acute setting is less certain. In the low-risk setting, CTCA has been shown to be accurate, safe and efficient but inherent low rates of adverse events in this population and the advent of high-sensitivity troponin testing have left little room for CTCA to show any short-term clinical benefit. In higher-risk populations, CTCA has potential to...

Impact on stable chest pain pathways of CT fractional flow reserve
Conclusions FFRCT adoption was associated with greater compliance with NICE guidelines, reduced invasive diagnostic angiography, planned admissions and needing more than one test to reach a pathway endpoint.

Spontaneous rupture of a gastroduodenal artery side branch immediately following endovascular treatment of gastroepiploic artery aneurysm or pseudoaneurysm
Selective DSA of spontaneous rupture of a gastroduodenal artery side branch. Key clinical massagePseudoaneurysms and aneurysms of the visceral arteries are rare entities. To the best of our knowledge, rupture of a proximal parental artery during endovascular treatment of a visceral aneurism/pseudoaneurysm has not been reported and should be kept in mind as a tragic possibility immediately following an apparently successful management of them.AbstractA 55-year-old woman with a history of coronary artery disease was referred to our hospital with abdominal pain as her primary complaint. Early works revealed anemia, a small am...

Unexpected form of ischemic colitis: A case report
Exp Ther Med. 2023 Jul 13;26(3):419. doi: 10.3892/etm.2023.12118. eCollection 2023 Sep.ABSTRACTIschemic colitis (IC) is an ischemic injury to the colon caused by an occlusive arterial blood supply or blocked venous return. Colonoscopic manifestations most frequently include colonic mucosal edema and erosion, while tubular channels are rare. It is also known as colon cast, and doctors often misdiagnose it when they encounter it. The present study presents a rare form of IC. An 80-year-old man visited The Affiliated People's Hospital of Ningbo University (Ningbo, China) due to abdominal pain and bloody stools. He had no sign...

Aortoesophageal Fistula Occurring Due to Aortic Aneurysm
We present the case of a 61-year-old male who presented with chest pain and hematemesis. A chest X-ray showed a widened mediastinum. The patient underwent computed tomography angiography (CTA), which showed the presence of a large aneurysm in the aorta, which caused compression of the trachea, esophagus, and left pulmonary artery. Additionally, there was evidence of an AEF. It was decided to perform an emergency surgical intervention on the patient. However, the patient had multiple episodes of hematemesis and expired.PMID:37602018 | PMC:PMC10438792 | DOI:10.7759/cureus.42148

75 year old dialysis patient with nausea, vomiting and lightheadedness
Written by Jesse McLaren A 75 year-old patient with diabetes and end stage renal disease was sent to the ED after dialysis for three days of nausea, vomiting, loose stool, lightheadedness and fatigue. RR18 sat 99% HR 90 BP 90/60, afebrile. Below is the 15 lead ECG. What do you think? There ’s normal sinus rhythm, normal conduction, normal axis, normal R wave progression and normal voltages. There’s subtle inferior ST elevation with straightening of the ST segment, reciprocal ST depression and T wave inversion in aVL, and ST depression in V2. This is diagnostic of infero-posterior OMI, but it is falsely n...

Application of postmortem computed tomography angiography to settle a medical dispute after aortic dissection surgery: a forensic case report
CONCLUSIONS: While many previous postmortem imaging case reports have shown positive results that provided evidence of medical malpractice, the current case excludes the possibility of physician negligence and reasonably settles the medical dispute from another perspective. In short, the PMCTA approach we describe here was an effective tool that can be applied to certain medical-related forensic cases.PMID:37596679 | PMC:PMC10439614 | DOI:10.1186/s13019-023-02353-8
