Undoctored: The code on health has been cracked
Here’s another excerpt from my new book, Undoctored: Why the Health Care System Has Failed You and How You Can Become Smarter Than Your Doctor. Undoctored seeks to set you free from the predatory practices of the healthcare industry by helping you achieve ideal health, weight, and day-to-day functioning. Undoctored: The code on health has been cracked Many modern doctors hold themselves up as all-knowing, capable of managing every aspect of health, from delivery to death, from vaccination to senility. I know because I was guilty of this. The “I’m-the-doctor, you’re-the- patient relationship” has been frozen ...

Cost-effectiveness analysis of vaccinations and decision makings on vaccination programmes in Hong Kong: A systematic review.
CONCLUSIONS: There was a paucity of cost-effectiveness models evaluating vaccination targeted to the Hong Kong population. All evaluated vaccinations and immunisation interventions in Hong Kong, except for Haemophilus influenzae b, hepatitis A and HPV vaccinations, were considered either cost-saving or very cost-effective when compared to status quo. Helping Patients Make More Informed Postacute Care Choices
Conclusion Selecting the right postacute provider is a complex choice, frequently made under difficult circumstances, with meaningful consequences for the patient and the broader health care system. This complexity will likely increase for patients as reform efforts continue to push integration between acute and postacute providers, possibly creating financial incentives to influence patient choice in ways that may or may not be consistent with their preferences. If consumers are to play an active role in the evolving postacute care choice environment, it is essential that they be well-positioned to make good decisions. Un...

Pharmacists' Scope of Practice: Supports for Canadians with Diabetes
Conclusions Canadians with diabetes can expect to receive influenza vaccines and have medications renewed at their pharmacies. Many patients with diabetes qualify for a publicly funded medication review, and some provinces allow pharmacists to order and interpret laboratory results. This expanded scope provides greater opportunities for pharmacists to help support patients with diabetes in conjunction with other health-care team members.

Inclusion of the value of herd immunity in economic evaluations of vaccines. A systematic review of methods used.
CONCLUSION: The proportion of economic evaluations using a dynamic model has increased in recent years. However, 55% of the included studies used a static model for estimating herd immunity. Values from a static model can only be considered reliable if high quality surveillance data are incorporated into the analysis. Without this, the results are questionable and they should only be included in sensitivity analysis. PMID: 29146380 [PubMed - as supplied by publisher]

Bundling' COPD Care
Conclusions: Our study shows there is indeed a benefit of administering the bundle in COPD patients in terms of lung function, symptom improvement, exercise tolerance and excerbations. Hence the concept of a care bundle should be explored.

Genetic Analyses of Multidrug-Resistant Streptococcus pneumoniae Serogroup 19 CC320/271 Clone in China
Conclusions: There is a high prevalence of macrolide and tetracycline resistance S. pneumonia with a low PCV7 coverage. The serogroup 19 MDR CC320/271 clone warrants close monitoring to evaluate its effect after the switch to expanded conjugate vaccines.

Community-Acquired Pneumonia in Adults.
CONCLUSION: Major considerations include appropriate risk stratification and the implementation of a management strategy adapted to the degree of severity of the disease, along with the establishment of structured follow-up care and secondary prevention, especially for patients with comorbidities. Influenza vaccines in immunosuppressed adults with cancer.
CONCLUSIONS: Observational data suggest lower mortality and infection-related outcomes with influenza vaccination. The strength of evidence is limited by the small number of studies and low grade of evidence. It seems that the evidence, although weak, shows that the benefits overweigh the potential risks when vaccinating adults with cancer against influenza. However, additional placebo or no-treatment controlled RCTs of influenza vaccination among adults with cancer is ethically questionable. There is no conclusive evidence regarding the use of adjuvanted versus non-adjuvanted influenza vaccine in this population. Vaccines for preventing influenza in the elderly.
CONCLUSIONS: Older adults receiving the influenza vaccine may have a lower risk of influenza (from 6% to 2.4%), and probably have a lower risk of ILI compared with those who do not receive a vaccination over the course of a single influenza season (from 6% to 3.5%). We are uncertain how big a difference these vaccines will make across different seasons. Very few deaths occurred, and no data on hospitalisation were reported. No cases of pneumonia occurred in one study that reported this outcome. We do not have enough information to assess harms relating to fever and nausea in this population. The evidence for a lower risk of...

A bibliometric analysis of systematic reviews on vaccines and immunisation.
CONCLUSIONS: SYSVAC provides a freely searchable platform to identify existing vaccine-policy-relevant systematic reviews. Systematic reviews will need to be assessed adequately for each specific question and quality. PMID: 29576305 [PubMed - as supplied by publisher]

Efficacy of whole-cell pneumococcal vaccine in mice: A systematic review and meta-analysis
Conclusions The best protocol for use of a WCV is its application with CT adjuvant administered intranasally in three inoculations at doses of 10⁸ CFU. Further studies performed under similar conditions to obtain accurate results on the effectiveness of this vaccine are recommended.
