Efficacy and Safety of Prothrombin Complex Concentrate (PCC) for Direct Oral Anticoagulant Reversal: A Single Institutional Experience
ConclusionIn this cohort, 80% of pts demonstrated clinical stability/improvement of hemorrhage. All pts were able to undergo surgical procedures; however 17% required additional FFP to achieve hemostasis. Thromboembolism occurred in 12% of patients. No deaths could definitely be attributed to 4PCC; however, 30% of deaths could be attributable to the underlying hemorrhagic episode.DisclosuresNo relevant conflicts of interest to declare.

Reassessing the Need for Pre-Operative Transfusions in Sickle Cell Disease Patients with a High Baseline Hemoglobin
Conclusion: Patients with SCD and a baseline hemoglobin ≥90 g/L who receive a pre-operative top-up transfusion have an increased risk of post-operative complications compared to those who are not transfused. In low and medium risk surgeries, a policy of withholding transfusions for such patients may be considered. Red blood cell transfusion to treat or prevent complications in sickle cell disease: an overview of Cochrane reviews.
CONCLUSIONS: This overview provides support from two high-quality Cochrane Reviews for the use of RBC transfusions in preventing stroke in children and adolescents at high risk of stroke (abnormal TCDs or SCI) and evidence that it may decrease the risk of SCI in children with abnormal TCD velocities. In addition RBC transfusions may reduce the risk of ACS and painful crisis in this population.This overview highlights the lack of high-quality evidence in adults with SCD and the number of reviews that have no evidence for the use of RBC transfusions across a spectrum of SCD complications. Conclusion Early onset of MoyaMoya syndrome related stroke postoperatively after laparoscopic cholecystectomy is difficult to be assessed immediately. Laparoscopic cholecystectomy in an adult moyamoya disease case
Moyamoya disease (MMD) is a progressive disorder caused by blocked arteries or spontaneous occlusion of the blood vessels of the circle of Willis with compensatory collateral vasculature; it is an increasingly recognized cause of stroke [1,2]. Anesthetic management of patients with MMD is challenging because appropriate control of hemodynamic state and ventilation is required to prevent neurologic adverse events. We previously reported the perioperative management of video-assisted thoracoscopic surgery in an adult patient with MMD [3].

Reversal of cardiac remodeling after treatment of IgG4 related cholangitis
A 38-year-old woman visited the doctor in our hospital for complete left bundle branch block (CLBBB) on electrocardiography(ECG) during physical examination in May 2014. She did not have any symptoms. The patient's past history included cholecystectomy due to cholecystitis in 2012 and resection of extrahepatic bile duct due to IgG4 related cholangitis in May 2013. She took orally 250mg of ursodeoxycholic acid twice daily for long-term after operation. The patient denied history of hyptension, diabetes, stroke, dyslipidemia, smoking and drinking habit.

Hemodynamic changes and stress response during BIS-guided TCI anesthesia with propofol-fentanyl in laparoscopic versus open cholecystectomy
Conclusion BIS-guided TCI anesthesia with propofol-fentanyl offers a good and safe anesthesia technique for patients undergoing either laparoscopic or open cholecystectomy. The hemodynamic stability guided by esophageal Doppler monitor makes it a very appealing choice.

Presence of gallstones and the risk of cardiovascular diseases: The EPIC-Germany cohort study
Conclusions Our results indicate an increased cardiovascular risk for gallstone formers, which cannot be counteracted by gallbladder removal and opens up perspectives for individualized prevention strategies.

Pneumoperitoneum affects stroke volume variation in humans
Conclusion A pneumoperitoneum increased SVV, which is similar to the findings of previous animal studies but is different from a previous clinical study. Upon release of the pneumoperitoneum, SVV decreased significantly, which is new information. SVV values must be estimated cautiously during a pneumoperitoneum.

Surgical Procedures and Outcomes Among Children with Sickle Cell Disease.
CONCLUSION:: Surgical procedures such as cholecystectomy, tonsillectomy, splenectomy, hernia repair, and appendectomy account for a small but significant proportion of hospital admissions in children with sickle cell disease.
