Design and rationale of a pragmatic trial integrating routine screening for atrial fibrillation at primary care visits: The VITAL-AF trial
Publication date: Available online 22 June 2019Source: American Heart JournalAuthor(s): Jeffrey M. Ashburner, Steven J. Atlas, David D. McManus, Yuchiao Chang, Ana T. Trisini Lipsanopoulos, Leila H. Borowsky, Wyliena Guan, Wei He, Patrick T. Ellinor, Daniel E. Singer, Steven A. LubitzAbstractGiven the preventable morbidity and mortality associated with atrial fibrillation (AF), increased awareness of undiagnosed AF, and advances in mobile ECG technology, there is a critical need to assess the effectiveness of using such technology to routinely screen for AF in clinical practice. VITAL-AF is a pragmatic trial that will test...

Effects of heat stroke on surface ECG: a study on clinical outcomes
Conclusion There was high incidence of atrial arrhythmias in patients with classic heat stroke. A low P-wave voltage (<0.01 mV) in lead II was predictive of adverse in-hospital outcome in this cohort of patients.

Prevalence of atrial fibrillation in the Italian elderly population and projections from 2020 to 2060 for Italy and the European Union: the FAI Project
ConclusionsOur findings indicate a high burden of AF in coming decades, especially among the oldest-old, who carry the higher AF-related risk of stroke and medical complications.

13 The shock team approach: the rationale and evidence
Cardiogenic shock (CS) is defined as a state of ineffective cardiac output caused by a cardiac disorder that results in both clinical and biochemical manifestations of inadequate tissue perfusion.1 Among patients presenting with CS, there is a spectrum of disease whereby some patients can be stabilised with pharmacologic interventions alone, while others require escalation to mechanical circulatory support (MCS).2 As patients and treatment options both become increasingly complex, comprehensive critical care may be best delivered in disease-specific service line ICUs.2 The model of the cardiac ICU has transitioned over tim...

Screening for Atrial Fibrillation using a Mobile, Single-Lead Electrocardiogram in Canadian Primary Care Clinics
ConclusionPreviously undiagnosed AF is common in older individuals attending primary care clinics. The Kardia Mobile ECG device appears to be an effective screening tool for AF with high physician acceptability. More research on the feasibility of such novel technologies is warranted for future consideration of integration in population-based screening programs.

Device Therapy and Arrhythmia Management in Left Ventricular Assist Device Recipients: A Scientific Statement From the American Heart Association.
linical Cardiology; and Council on Cardiovascular and Stroke Nursing Abstract Left ventricular assist devices (LVADs) are an increasingly used strategy for the management of patients with advanced heart failure with reduced ejection fraction. Although these devices effectively improve survival, atrial and ventricular arrhythmias are common, predispose these patients to additional risk, and complicate patient management. However, there is no consensus on best practices for the medical management of these arrhythmias or on the optimal timing for procedural interventions in patients with refractory arrhythmias. Altho...

Characteristics and 12-month outcome of patients with atrial fibrillation at a tertiary hospital in Botswana.
CONCLUSIONS: In this study, hypertension, rheumatic heart disease and heart failure were the most prevalent co-morbidities. AF presented in young people and conferred high mortality rates in both valvular and non-valvular AF patients. Prevention and optimal management of AF and associated co-morbidities are of critical importance. PMID: 31140547 [PubMed - as supplied by publisher]

Device-Detected Atrial Fibrillation: Evidencing the Knowledge Gap.
Abstract Stroke is a devastating complication of atrial fibrillation (AF) the odds of which can be reduced by use of oral anticoagulation based on the stroke risk score.1,2 Patients with asymptomatic clinical AF have a similar stroke risk as patients with symptomatic clinical AF.3 Thus far, electrocardiographic- (ECG) or Holter monitor-detected AF is a prerequisite before use of oral anticoagulation,1,2 because the guideline recommendations are based on studies that included only ECG- or Holter-detected AF ('clinical' AF).1,2 In the past decades increasing number of cardiac implantable electronic devices (CIED) ha...

Rationale and design of the randomized, controlled early valve replacement guided by biomarkers of left ventricular decompensation in asymptomatic patients with severe aortic stenosis (EVOLVED) trial
ConclusionThe EVOLVED trial is the first multicenter randomized controlled trial to compare early aortic valve replacement to routine care in asymptomatic patients with severe aortic stenosis and mid-wall LGE.

Effect of Extensive Ablation on Recurrence in Patients with Persistent Atrial Fibrillation Treated with Pulmonary Vein Isolation (EARNEST-PVI) trial: Design and rationale.
CONCLUSIONS: The EARNEST-PVI trial is a randomized controlled trial designed to assess whether PVI alone is non-inferior to extended substrate ablation for patients with persistent AF undergoing a first catheter ablation. PMID: 30853354 [PubMed - as supplied by publisher]

Clinical Performance of Apixaban vs. Vitamin K Antagonists in Patients with Atrial Fibrillation Undergoing Direct Electrical Current Cardioversion: A Prospective Propensity Score-Matched Cohort Study
ConclusionsNewly initiated anticoagulation with apixaban in patients with nonvalvular AF scheduled for TEE-guided DCC seems to be as effective and safe as uninterrupted VKA therapy during 12  months of follow-up.

Prevalence and risk factors for atrial fibrillation and its anticoagulant requirement in adults aged ≥40 in Jimma Town, Southwest Ethiopia: A community based cross-sectional study
ConclusionThe prevalence of AF is high and common risk factors were sex, current smoking, hypertension, and higher BMI in this cohort. More than two-third of study participants with AF were at higher need of oral anticoagulants.

Sex Differences in Risk Factors for Incident Atrial Fibrillation (From the Reasons for Geographic And Racial Differences in Stroke (REGARDS) Study)
We examined whether the sex differences in atrial fibrillation (AF) is related to difference in risk factors leading to AF or due to a differential impact of the same risk factors in 11,806 participants (55.2 % women) from the REGARDS study. Incident AF was ascertained by electrocardiograms and medical history at a follow-up examination. Backwards elimination logistic regression was used to identify AF risk factors in men and women, separately. Over a median follow-up of 9.0 years, 588 (11.1%) men and 428 (6.6%) women (p-value

Community screening for atrial fibrillation in the era of smart devices
Atrial fibrillation (AF) associated strokes can be effectively prevented by oral anticoagulant therapy, but patients often miss opportunities for adequate thromboprophylaxis due to lack of symptoms or electrocardiographic (ECG) documentation of AF. Early identification of AF has become an important issue not only from the healthcare perspective but also in the social aspect, given the burden of disabling stroke related to this common arrhythmia. The latest guidelines have therefore recommended the opportunistic screening for AF by pulse palpation or single ‑lead ECG in individuals aged>65  years or in individuals with ...
