Functional gain following rehabilitation of recurrent ischemic stroke in the elderly: Experience of a post-acute care rehabilitation setting
Stroke remains a major cause of mortality all over the world (Murray & Lopez, 1997). The fastest growing segment of the elderly population is at highest risk of sustaining strokes (World Population Prospects, 2000). As a result, stroke will remain a major healthcare problem unless the incidence and disabling consequences of first-ever and recurrent strokes are reduced. Approximately 25% of patients with first-ever stroke will suffer a recurrent stroke over a period of five years, with one quarter of them being fatal within 28 days (Hardie, Jamrozik, Hankey, Broadhurst, & Anderson, 2005).

Risk factors for incident dementia after stroke and transient ischemic attack
Conclusions Chronic brain changes including WMCs, MTLA, and AD pathology are associated with incident dementia after stroke/TIA. Interventions targeting these chronic brain changes may reduce burden of vascular cognitive impairment.

Cerebral Microbleeds and Cognitive Function in Ischemic Stroke or Transient Ischemic Attack Patients
Conclusion: CMBs were associated with cognitive dysfunction in stroke/TIA patients, especially in the attention domain. Longitudinal functional changes, depression, and health‐related quality of life among stroke survivors living at home after inpatient rehabilitation
ConclusionSocial participation was associated with long‐term improvement in activities of daily living, indicating that interventions targeting social function are important for maintaining and further improving activities of daily living in stroke survivors. Health‐related quality of life was correlated with activities of daily living and depression; therefore, interventions targeting these parameters are important in improving the quality of life of stroke survivors.

Quality of Life After Off-Label Thrombolysis for Ischemic Stroke in Elderly Patients
Conclusion QOL after intravenous thrombolysis in the elderly population was comparable to that of younger subjects. Despite its small sample size, this study showed promising results in favor of intravenous thrombolysis in the elderly population and highlighted the importance of systematic screening for post-stroke cognitive impairment, particularly in this population.

Use of Oral Anticoagulant Therapy in Older Adults with Atrial Fibrillation After Acute Ischemic Stroke
ConclusionDespite very high stroke risk, more than 40% of participants were not discharged with an OAC. Dominant reasons included fall risk, poor prognosis, older age, and dementia. These individuals’ high 1‐year mortality rate confirmed their high level of comorbidity. To improve anticoagulation decisions and outcomes in this population, future research should focus on strategies to mitigate fall risk, improve assessment of risks and benefits of anticoagulation in individuals with AF, and determine whether newer anticoagulants are safer in complex elderly and frail individuals.
