Improving outcomes in patients with atrial fibrillation: Rationale and design of the Early treatment of Atrial fibrillation for Stroke prevention Trial
Conclusion: EAST will determine whether rhythm control therapy, when applied early after the initial diagnosis of AF, can prevent cardiovascular complications associated with AF.

Stroke and ventricular arrhythmias
Abstract: Electrocardiographic abnormalities and cardiac arrhythmias are commonly noted after acute stroke. Risk of malignant ventricular arrhythmias is increased after a stroke and is associated with sudden cardiac death. Autonomic imbalance modulated by direct injury to neurogenic structures and enhanced by catecholamine storm can lead to myocardial damage and arrhythmogenesis. Experimental and clinical evidence suggests that insular cortex infarcts play a key role in autonomic dysregulation that lead to arrhythmias in the acute setting. Management of ventricular arrhythmias associated with acute stroke should focus on c...

Relationships between the QTc interval and cardiovascular, stroke, or sudden cardiac mortality in the general Japanese population.
CONCLUSION: In the general Japanese population, cardiovascular and stroke mortality increase progressively with the Bazett QTc interval quartile, while the risk of sudden cardiac death exhibits a J-shaped relationship with the latter variable. PMID: 25066337 [PubMed - as supplied by publisher]

Atrial fibrillation in patients with ischaemic stroke in the Swedish national patient registers: how much do we miss?
Conclusion Despite the high specificity, AF diagnosis in the Swedish Patient Register assessed in the population of ischaemic stroke patients and age- and gender-matched control subjects has modest sensitivity, which may result in underestimating prevalent and incident AF cases if only register data are used for identification of subjects with AF in epidemiology studies.

Prothrombotic risk factors and antithrombotic therapy in children with ischemic stroke
Conclusion: Ischemic CVS in children is multifactorial. Thrombophilia testing should be performed in any child with CVS. Early use of aspirin improves the prognosis and has less effect on cognitive function.

New-Onset Paroxysmal Atrial Fibrillation Diagnosis in Ischemic Stroke Patients
Conclusions: This risk prediction model can be helpful to estimate the risk of an underlying pAF within 3 months after suffering an IS/TIA, contributing to increased AF detection efforts, thereby starting the correct secondary prevention treatment.Eur Neurol 2015;74:211-217

Continuous Rapid Quantification of Stroke Volume Using Magnetohydrodynamic Voltages in 3T Magnetic Resonance Imaging Magnetic Resonance Imaging
Conclusions— Accurate beat-to-beat stroke volume and BF were estimated using VMHDVCG extracted from intra-MRI 12-lead ECGs, providing a means to enhance patient monitoring during MR imaging and MR-guided interventions.

Detection of occult paroxysmal atrial fibrilation by implantable long-term electrocardiographic monitoring in cryptogenic stroke and transient ischemic attack population: a study protocol for prospective matched cohort study
DiscussionIn our study we are focusing on a possible correlation between detection of atrial fibrillation by an implantable ECG recorder, and PITX2 and/or ZFHX3 gene mutations in cryptogenic stroke/TIA patients. A correlation could lead to implementation of this genomic approach to cryptogenic stroke/TIA diagnostics and management. The results will be published in 2018.Trial registrationClinicalTrials.gov: NCT02216370.

Cryptogenic stroke in a young patient with heart disease and kidney failure.
CONCLUSIONS: Fabry's disease must be suspected in young males with heart disease, stroke or peripheral neuropathy, skin lesions, kidney failure and a history of cases in the family. Hormone replacement therapy must be established at an early stage, as it can improve the prognosis. PMID: 28497441 [PubMed - in process]

Refinement of detecting atrial fibrillation in stroke patients: results from the TRACK ‐AF Study
ConclusionsA combination of SRA and ICM is a promising strategy to detect occult AF. SRA is reliable in predicting incident AF with a high negative predictive value. Thus, SRA may serve as a cost‐effective pre‐selection tool identifying patients at risk for AF who may benefit from further cardiac monitoring by ICM.

Atrial Cardiopathy and Stroke Prevention
AbstractPurpose of ReviewMany patients with ischemic stroke have a stroke syndrome and neuroimaging findings consistent with embolism, but no evidence of atrial fibrillation. Recent data suggest that the pathological atrial substrate, oratrial cardiopathy, that underlies atrial fibrillation, rather than atrial fibrillation itself, may be an important cause of unexplained strokes. This paper will review the literature on the rationale and data behind the concept of atrial cardiopathy, its pathophysiology, suggested biomarkers of atrial cardiopathy, and therapeutic implications.Recent FindingsSeveral lines of evidence sugges...
