Perioperative Stroke
Abstract Purpose of Review The significance of stroke in the noncardiac, nonneurologic, and nonvascular perioperative setting has been routinely underestimated. A complex interplay of multiple perioperative, patient, and surgical factors is involved in the pathogenesis of perioperative stroke. Multiple risk reduction strategies are required to decrease the incidence of this devastating complication. Recent Findings Recent evidence demonstrates that one in every 1000 patients undergoing low-risk surgery may develop perioperative stroke which can result in...

Neuroimaging as a Selection Tool and Endpoint in Clinical and Pre-clinical Trials
AbstractStandard imaging in acute stroke enables the exclusion of non-stroke structural CNS lesions and cerebral haemorrhage from clinical and pre-clinical ischaemic stroke trials. In this review, the potential benefit of imaging (e.g., angiography and penumbral imaging) as a translational tool for trial recruitment and the use of imaging endpoints are discussed for both clinical and pre-clinical stroke research. The addition of advanced imaging to identify a “responder” population leads to reduced sample size for any given effect size in phase 2 trials and is a potentially cost-efficient means of testing interventions...

Isoflurane preconditioning inhibits the effects of tissue-type plasminogen activator on brain endothelial cell in an in vitro model of ischemic stroke.
In this study, we used oxygen/glucose deprivation and reperfusion (OGD/R) condition to mimic an ischemic stroke in vitro. Matrix metalloproteinases (MMP) activity was measured in endothelial cell media. Also, neuronal cell culture was performed to investigate the effect of pretreated isoflurane on the neuronal cell survival after tPA-induced injury during OGD/R. Isoflurane pretreatment prevented tPA-induced MMP-2 and MMP-9 activity and suppressed tPA-triggered LRP/NF-κB/Cox-2 signaling after OGD/R. Neuronal cells, incubated with endothelial cell conditioned medium (EC-CM) after tPA + OGD/R, showed upregulation of pro-apop...

Interpretation of Brain CT Scans in the Field by Critical Care Physicians in a Mobile Stroke Unit
CONCLUSIONThis pilot study shows that anesthesiologists trained in prehospital critical care may effectively assess cerebral CT scans in an MSU, and determine if there are radiological contraindications for thrombolysis.

The Impact of Conscious Sedation versus General Anesthesia for Stroke Thrombectomy on the Predictive Value of Collateral Status: A Post Hoc Analysis of the SIESTA Trial INTERVENTIONAL
CONCLUSIONS: The sedation mode, conscious sedation or general anesthesia, did not influence the predictive value of collaterals in patients with large-vessel occlusion anterior circulation stroke undergoing thrombectomy in the SIESTA trial.

Improving mechanical thrombectomy time metrics in the angiography suite: Stroke cart, parallel workflows, and conscious sedation.
Conclusions Use of a stroke cart, parallel workflows by neurointerventionalists, technologists, and nursing staff, and use of conscious sedation may be useful to other institutions in efforts to improve procedural times. PMID: 29145742 [PubMed - as supplied by publisher]

Anaesthesia for Endovascular Treatment of Acute Ischemic Stroke: Still Controversial?
This article reviews recent research on anaesthetic management of acute ischemic stroke revascularization. With emphasis on the effect of anaesthesia type on outcome, we will discuss controversial discoveries and future perspectives.Recent FindingsA substantial number of retrospective observational studies have suggested that general anaesthesia (GA) compared to conscious sedation (CS) is associated with worse outcomes after endovascular therapy for acute ischemic stroke. In contrast, recent randomized trials reported no difference in outcomes in ischemic stroke patients receiving either GA or CS during revascularization w...

Clinical effects and safety evaluation of dexmedetomidine hydrochloride combined with etomidate fat emulsion in patients undergoing interventional treatment of stroke during anesthesia.
CONCLUSIONS: Dexmedetomidine hydrochloride combined with etomidate fat emulsion in patients undergoing interventional treatment of stroke during anesthesia has potential value in clinical applications, through the maintenance of ideal hemodynamics and a reduction in complications. Behavior of stroke volume variation in hemodynamic stable patients during thoracic surgery with one-lung ventilation periods
ConclusionThe decrease of stroke volume variation values during one-lung ventilation with protective ventilatory strategies advices not to use the same threshold values to determine fluid responsiveness.

Calycosin-7-O-β-D-glucoside reduces myocardial injury in heat stroke rats
ConclusionWe thus conclude that myocardial injury can be a pressing need for the design of diagnostic and therapeutic modalities for heat stroke. In particular, our data indicate that CG protects against heat stroke in rats by mitigating myocardial injury.

Dose reference levels and clinical determinants in stroke neuroradiology interventions
ConclusionsStroke dose RLs introduced in this work promote the optimisation of patient doses. Male gender, number of passages and success of recanalisation are independent key parameters affecting patient dose.Key Points• Stroke dose RLs derived in terms of total DAP (162 Gy cm2), Ka,r(854 mGy), FT (42 min) and NI (559) will help optimise the radiation safety of patients treated with mechanical thrombectomy.• Male gender (relative dose multiplier 1.31; 95% CI 1.12–1.45), number of passages (RDM 1.22 per passage; 95% CI 1.10–1.22) and success of recanalisation TICI score> 2b (RDM 0.52, 95% CI 0.55 –0.80) are in...
