Association of health insurance status and receipt of health care among US adults with self-reported hypertension
Hypertension is a major risk factor for heart disease and stroke; yet only half of the 70 million US adults with hypertension have it under control. Previous studies have shown that adults with hypertension and health insurance were more likely to have controlled blood pressure than those without health insurance. However, less is known about hypertension management among those classified as underinsured (i.e., those with health insurance, but with financial difficulty in paying for health care).

Utilization and Predictors of Electrical Cardioversion in Patients Hospitalized for Atrial Fibrillation.
Abstract Atrial fibrillation (AF) is a common arrhythmia in adults associated with thromboembolic complications. External electrical cardioversion (DCCV) is a safe procedure used to convert AF to normal sinus rhythm. We sought to study factors that affect utilization of DCCV in hospitalized patients with AF. The study sample was drawn from the Nationwide Inpatient Sample (NIS) of the Healthcare Cost and Utilization Project in the United States. Patients with a primary discharge diagnosis of AF that received DCCV during hospitalization in the years 2000-2010 were included. An estimated 2,810,530 patients with a pri...

The Use of Oral Anticoagulants for Stroke Prevention in Atrial Fibrillation Patients with History of Intra-Cranial Hemorrhage.
CONCLUSIONS: -Warfarin use may be beneficial for AF patients with prior ICH having a CHA2DS2-VASc score ≥6. Whether the use of non-vitamin K antagonist oral anticoagulants (NOACs) could lower the threshold for treatment deserves further study. PMID: 26969761 [PubMed - as supplied by publisher]

Abstract 138: Contemporary Patterns of Use of Antiplatelet Agents in Patients With Acute Myocardial Infarction: Insight From the National Cardiovascular Data Registry (NCDR) Session Title: Abstract Poster Session I and Reception
Conclusions: Our contemporary dataset shows a modest but significant increase in use of ticagrelor early and at discharge, with simultaneous decline in the use of clopidogrel and prasugrel in patients presenting with AMI.

Effect of Adherence to Oral Anticoagulants on Risk of Stroke and Major Bleeding Among Patients With Atrial Fibrillation Arrhythmia and Electrophysiology
Conclusions Adherence to anticoagulation is poor in practice and may be modestly improved with NOACs. Adherence to therapy appears to be most important in patients with CHA2DS2-VASc score ≥2, whereas the benefits of anticoagulation may not outweigh the harms in patients with CHA2DS2-VASc score 0 or 1.

Clinical Characteristics and Lifestyle Behaviors in a Population-Based Sample of Chinese and South Asian Immigrants With Hypertension.
CONCLUSIONS: Findings from this study highlight important clinical distinctions in hypertensive Chinese and South Asian immigrant communities with respect to age and body size. Whether targeted and culturally appropriate approaches would reduce cardiovascular disease-related mortality in these groups needs further study. PMID: 26888778 [PubMed - as supplied by publisher]

Medication Adherence and the Risk of Cardiovascular Mortality and Hospitalization Among Patients With Newly Prescribed Antihypertensive Medications Epidemiology/Population
This study evaluated the effect of antihypertensive medication adherence on specific cardiovascular disease mortality (ischemic heart disease [IHD], cerebral hemorrhage, and cerebral infarction). Our study used data from a 3% sample cohort that was randomly extracted from enrollees of Korean National Health Insurance. Study subjects were aged ≥20 years, were diagnosed with hypertension, and started newly prescribed antihypertensive medication in 2003 to 2004. Adherence to antihypertensive medication was estimated as the cumulative medication adherence. Subjects were divided into good (cumulative medication adherence, &g...

Angiotensin-converting enzyme inhibitors, angiotensin II receptor blockers and the risk of major adverse cardiac events in patients with diabetes and prior stroke: a nationwide study
Conclusion: Our study supports the hypothesis that the risks of MACEs and two additional secondary outcomes in patients with diabetes who survived ischemic stroke did not differ according to ACEI versus ARB use.

Common carotid artery end-diastolic velocity is independently associated with future cardiovascular events
Conclusions Low common carotid EDV and PSV were independently associated with future CVD, and EDV improved the prediction of future CVD. More prospective studies are required in different ethnic groups to understand the significance and implication of these findings.

Risks of Death and Stroke in Patients Undergoing Hemodialysis With New-Onset Atrial Fibrillation: A Competing-Risk Analysis of a Nationwide Cohort.
CONCLUSIONS: -The risk of stroke was only modestly higher in patients undergoing hemodialysis with new-onset AF than in those without AF, and it became insignificant when accounting for the competing risk of in-hospital death. PMID: 26680239 [PubMed - as supplied by publisher]

Trends in Ideal cardiovascular Health Metrics Among Employees of a Large Healthcare Organization (From the Baptist Health South Florida Employee Study)
The American Heart Association’s (AHA) 2020 goal is to improve the cardiovascular health (CVH) of people living in the United States by 20% and reduce mortality from cardiovascular diseases (CVD) and stroke by 20%. Given that 155 million adults are in the U.S workforce, and over 60% have employee-based insurance, workplace studies provide an important opportunity to assess, and potentially advance CVH through the use of comprehensive workplace wellness programs. Among a cohort of employees of the Baptist Health System, CVH was assessed annually during voluntary health fairs and health risk assessments (HRA) from 2011-201...

Changing Cholesterol Levels and Coronary Heart Disease Risk.
Abstract A report by Colantonio and colleagues in this issue of Circulation describes the relations between serum lipoprotein cholesterol levels and triglycerides with coronary heart disease (CHD) incidence.(1) Their analyses draw upon the experience of three American population samples of adults with 8.9 years of follow up. The first study group is the Reasons for Geographic and Racial Differences in Stroke (REGARDS, National Institutes of Health observational cohort, baseline 2003-2007), the second investigation is the Atherosclerosis Risk in Communities (ARIC, National Heart, Lung and Blood observational cohort...
