Effect of Posterior Pericardiotomy on the Incidence of Postoperative Atrial Fibrillation in 1000 Consecutive Isolated CABG
Purpose: Postoperative atrial fibrillation (POAF) is the most common arrhythmia and morbidity after cardiac surgery and is associated with prolonged hospital stay, stroke and increased costs. At the Royal Hobart Hospital, one of the two surgeons routinely performed posterior pericardiotomy for patients undergoing coronary artery bypass grafting (CABG). We took this opportunity to review risk factors associated with postoperative atrial fibrillation and in particular if posterior pericardiotomy reduces the incidence of postoperative atrial fibrillation.

Association of the magnitude of weight loss and changes in physical fitness with long-term cardiovascular disease outcomes in overweight or obese people with type 2 diabetes: a post-hoc analysis of the Look AHEAD randomised clinical trial
We examined whether the incidence of cardiovascular disease in Look AHEAD varied by changes in weight or fitness. Methods Look AHEAD was a randomised clinical trial done at 16 clinical sites in the USA, recruiting patients from Aug 22, 2001, to April 30, 2004. In the trial, 5145 overweight or obese adults aged 45–76 years with type 2 diabetes were assigned (1:1) to an intensive lifestyle intervention or diabetes support and education. In this observational, post-hoc analysis, we examined the association of magnitude of weight loss and fitness change over the first year with incidence of cardiovascular disease. The primar...

Comparison of Percutaneous Coronary Intervention Versus Coronary Artery Bypass Graft in Aged Patients With Unprotected Left Main Artery Lesions.
This study compared the outcomes of percutaneous coronary intervention (PCI) versus coronary artery bypass graft (CABG) in this population.A total of 126 patients older than 60 years of age with LM lesions who underwent revascularization in our hospital from January 2012 to December 2013 were followed up for an average of 15.2 months. The cumulative incidence of major adverse cardiac and cerebral events (MACCE) was estimated by Kaplan-Meier plots. During follow-up, the CABG group had higher proportions of cardiac death, stroke, and worsening of heart failure while the PCI group had a higher proportion of recurrence of angi...

Transaortic Transcatheter Aortic Valve Implantation and Concomitant Off Pump Revascularization
We present 4 successfully combined off-pump procedures consisting of a transcatheter aortic valve implantation (Edwards SAPIEN XT) via the transaortic approach and an off-pump coronary artery bypass grafting. All patients were discharged free from stroke, myocardial infarction, or access site complications either to rehabilitation facility or to the referring hospital with none or trace aortic regurgitation and patent grafts. These cases confirm the feasibility of those combined operations and should be considered as realistic alternative for surgical treatment in high-risk patients who are clearly identified to benefit fr...

Carrie Fisher's Death Highlights The Reality Of Heart Disease In Women
Carrie Fisher died early Tuesday morning, four days after suffering a heart attack on a flight from London to Los Angeles. The actress and author, best known for her iconic role as Princess Leia in the “Star Wars” franchise, was 60 years old.  Experts say that Fisher’s death highlights an important reality about heart disease: It is the leading cause of death among men and women alike in the U.S. While heart disease encompasses many different conditions, a heart attack occurs when coronary arteries become blocked and oxygenated blood can’t reach the heart. About 735,000 Americans have hea...

Comparison of outcomes between off-pump versus on-pump coronary artery bypass surgery in elderly patients: a meta-analysis.
Abstract The aim of this study was to analyze if off-pump coronary artery bypass surgery (CABG) is associated with better treatment outcomes in elderly patients (>70 years of age) than on-pump CABG, using meta-analysis. Medline, PubMed, Cochrane and Google Scholar databases were searched until September 13, 2016. Sensitivity and quality assessment were performed. Twenty-two studies, three randomized control trials (RCTs) and 20 non-RCTs were included with 24,127 patients. The risk of death associated with on-pump or off-pump CABG in the RCTs were similar (pooled OR=0.945, 95%CI=0.652 to 1.371, P=0.766). However...

Long-Term Outcomes of Patients With Mediastinal Radiation-Associated Severe Aortic Stenosis and Subsequent Surgical Aortic Valve Replacement: A Matched Cohort Study Valvular Heart Disease
BackgroundCardiac disease after mediastinal radiotherapy for thoracic malignancy (chest radiotherapy [XRT]) often manifests as progressive aortic stenosis. In patients with XRT‐induced severe aortic stenosis undergoing surgical aortic valve replacement (SAVR), we sought to: (1) study long‐term survival and compare these patients with a matched cohort undergoing SAVR during the same time frame; and (2) identify potential predictors of long‐term mortality. Methods and ResultsWe studied patients with symptomatic severe aortic stenosis undergoing SAVR at our institution, of which there were 172 mediastinal XRT patients (6...

Effect of Preoperative β-Blocker Use on Outcomes Following Cardiac Surgery
Recent studies suggest that the use of preoperative β-blockers in cardiac surgery may not provide improved mortality rates, and may even contribute to negative clinical outcomes. We therefore assessed the role of β-blockers on several outcomes following cardiac surgery (delirium, acute kidney injury (AKI), stroke, atrial fibrillation, mortality, an d hospital length of stay (LOS)) in 4,076 patients who underwent elective coronary artery bypass grafting, coronary artery bypass grafting + valve, or valve cardiac surgery between November 1, 2009 to September 30, 2015 at Vanderbilt Medical Center.

Remote Ischemic Preconditioning and Clinical Outcomes in On ‐Pump Coronary Artery Bypass Grafting: A Meta‐Analysis of 14 Randomized Controlled Trials
Abstract The purpose of this article is to perform the first pooled analysis on remote ischemic preconditioning (RIPC) used for the improvement of clinical outcomes of patients only undergoing on‐pump coronary artery bypass grafting (CABG) in randomized controlled trials (RCTs). A systematic search was performed using PubMed, the Cochrane Library, and the Web of Science to identify studies that described the effect of RIPC on postoperative mortality in patients only undergoing on‐pump CABG. The outcomes included postoperative mortality, postoperative morbidity (including incidence of myocardial infarction, atrial fibri...

My Approach to the Patient With Left Main Disease
Coronary artery bypass graft surgery (CABG) has historically been recommended over percutaneous coronary artery intervention (PCI) as the preferred revascularization strategy for patients with significant unprotected left main coronary artery disease (CAD). However, PCI has more recently become a treatment option in selected patients with left main CAD who do not have extensive or complex CAD in the other coronary arteries. When equipoise is present, the 30-day rates for death, myocardial infarction (MI), and stroke will be lower with PCI, and CABG will have a higher risk for bleeding and infection and longer hospital and recovery times.
