DiscussionThis exploratory study will be the first to investigate the effects of physical fitness training in patients with ischemic stroke on MRI-based cerebral perfusion, pertinent blood biomarker levels, and functional outcome. The study may have an impact on current patient rehabilitation strategies and reveal important information about the roles of MRI and blood-derived biomarkers in ischemic stroke.Trial registration: NCT01954797.

Acute Ischemic Stroke as a Presenting Feature of Multiple Myeloma (P3.279)
Conclusions:Stroke is a leading and preventable cause of disability in the US adult population. The majority of strokes are due to atherosclerotic vascular or cardioembolic disease. Understanding the diagnostic work up of stroke and recognizing rare etiologies like MM is important for stroke prevention. This brings to light the importance of recognizing stroke as an initial presenting feature of multiple myeloma and of multiple myeloma as an important treatable etiology of stroke.Disclosure: Dr. Chung has nothing to disclose. P-016 Relationship between stroke recurrence, infarct pattern, and vascular distribution in patients with symptomatic intracranial stenosis
Conclusion We determined that basilar artery stenosis was most likely to present as a perforator stroke. As expected, patients discharged with suboptimal medical therapy were twice as likely to have a recurrent stroke (29% versus 13%). Among patients with optimal medical therapy, no recurrent strokes were seen in patients with embolic infarct pattern, while 57% recurrence rate was seen in patients with a watershed infarct pattern, suggesting that an embolic infarct pattern on initial presentation is possibly related to underlying plaque instability that is modifiable with optimal medical therapy. Recanalisation therapies for wake-up stroke.
CONCLUSIONS: There is insufficient evidence from randomised controlled trials for recommendations concerning recanalisation therapies for wake-up stroke. Automated CT Perfusion Imaging to Aid in the Selection of Patients With Acute Ischemic Stroke for Mechanical Thrombectomy: A Health Technology Assessment.
Conclusions: Automated CT perfusion imaging has an acceptable sensitivity and specificity for detecting brain areas that have been affected by stroke. In patients selected for mechanical thrombectomy using automated CT perfusion imaging, there was significant improvement in functional independence. Mechanical thrombectomy informed by automated CT perfusion imaging is likely to be cost-effective. We estimate that publicly funding automated CT perfusion imaging in Ontario would result in additional costs of $1.3 million in year 1 and $0.9 million per year thereafter. Characteristics of cerebral ischemic stroke based on moyamoya disease and atherosclerosis-associated intracranial arterial stenosis
ConclusionsDiffering from LAS stroke, MMD stroke mainly presents with WSI and does not feature with platelet hyper-aggregation and fragmentation of ulcer plaque. Whereby, focusing on perfusion improvement rather than antiplatelets and statins may be the predominant step in MMD-stroke correction.

Reduction in Acute Stroke Admissions during the COVID-19 Pandemic: Data from a National Stroke Registry
Conclusion: The marked decrease in admissions for acute stroke and TIA, occurring at a time of a relatively low burden of COVID-19, is of great concern. Effect of Intraoperative Arterial Hypotension on the Risk of Perioperative Stroke After Noncardiac Surgery: A Retrospective Multicenter Cohort Study
CONCLUSIONS: This study found no evidence to conclude that intraoperative hypotension within the range studied was associated with early perioperative stroke within 7 days after surgery. Large Culprit Plaque and More Intracranial Plaques Are Associated with Recurrent Stroke: A Case-Control Study Using Vessel Wall Imaging ADULT BRAIN
CONCLUSIONS: Large culprit plaque and more intracranial plaques were independently associated with recurrent stroke. Performing whole-brain vessel wall imaging may help identify patients with a higher risk of recurrent stroke.
