Paracetamol (acetaminophen) or non-steroidal anti-inflammatory drugs, alone or combined, for pain relief in acute otitis media in children.
CONCLUSIONS: Despite explicit guideline recommendations on its use, current evidence on the effectiveness of paracetamol or NSAIDs, alone or combined, in relieving pain in children with AOM is limited. Low quality evidence indicates that both paracetamol and ibuprofen as monotherapies are more effective than placebo in relieving short-term ear pain in children with AOM. There is insufficient evidence of a difference between ibuprofen and paracetamol in relieving short-term ear pain in children with AOM, whereas data on the effectiveness of ibuprofen plus paracetamol versus paracetamol alone were insufficient to draw any fi...

Prophylactic drug management for febrile seizures in children.
CONCLUSIONS: We found reduced recurrence rates for children with febrile seizures for intermittent diazepam and continuous phenobarbitone, with adverse effects in up to 30%. Apparent benefit for clobazam treatment in one trial needs to be replicated to be judged reliable. Given the benign nature of recurrent febrile seizures, and the high prevalence of adverse effects of these drugs, parents and families should be supported with adequate contact details of medical services and information on recurrence, first aid management and, most importantly, the benign nature of the phenomenon. Unsealed Documents Reveal Prince's Doctor Prescribed Meds For Singer Under Another Name
Opioid painkiller were found in pop star Prince’s home after his sudden death a year ago, and investigators were told that the musician had history of withdrawals “believed to be the result of abuse of prescription medication,” according to court documents unsealed on Monday. According to a search warrant of Prince’s Paisley Park complex outside Minneapolis, investigators found several pills labeled Watson 853 - the identifier for the generic hydrocodone-acetaminophen - in various rooms. A search also turned up other “numerous narcotic controlled substance pills” in various containers, s...

Oral paracetamol (acetaminophen) for cancer pain.
CONCLUSIONS: There is no high-quality evidence to support or refute the use of paracetamol alone or in combination with opioids for the first two steps of the three-step WHO cancer pain ladder. It is not clear whether any additional analgesic benefit of paracetamol could be detected in the available studies, in view of the doses of opioids used. Osmotic therapies added to antibiotics for acute bacterial meningitis.
CONCLUSIONS: Glycerol was the only osmotic therapy evaluated, and data from trials to date have not demonstrated an effect on death. Glycerol may reduce neurological deficiency and deafness. PMID: 29405037 [PubMed - as supplied by publisher]

Interventions for paracetamol (acetaminophen) overdose.
CONCLUSIONS: These results highlight the paucity of randomised clinical trials comparing different interventions for paracetamol overdose and their routes of administration and the low or very low level quality of the evidence that is available. Evidence from a single trial found activated charcoal seemed the best choice to reduce absorption of paracetamol. Acetylcysteine should be given to people at risk of toxicity including people presenting with liver failure. Further randomised clinical trials with low risk of bias and adequate number of participants are required to determine which regimen results in the fewest advers...

Mixed Treatment Comparisons for Nonsurgical Treatment of Knee Osteoarthritis: A Network Meta-analysis.
DISCUSSION: Naproxen ranked most effective among conservative treatments of KOA and should be considered when treating pain and function because of its relative safety and low cost. The best available evidence was analyzed, but there were instances of inconsistency in the design and duration among articles, potentially affecting uniform data inclusion. Piriformis muscle syndrome remains controversial and diagnosis is difficult
Piriformis syndrome is a controversial entrapment neuropathyWhat is  piriformis muscle? The piriformis muscle is a small but important external rotator of the hip that crosses the sciatic nerve and is believed by some to cause sciatica-type pain when it compresses the nerve. However, the existence of this so-called "piriformis syndrome" remains controversial and diagnosis is difficult.Controversy is due to the limited research about the condition and the difficulty of making the diagnosis, particularly as symptoms mimic many other more common diagnoses. How common is it?Piriformis syndrome may account for 0.3-6% of scia...

Does long-term use of pain relievers have an impact on the rate of orthodontic tooth movement? A systematic review of animal studies.
Conclusions: Long-term consumption of pain relievers may affect the rate of orthodontic tooth movement. The orthodontist should be capable of identifying patients taking pain relievers independently of orthodontic treatment and consider the possible implications. Trial registration: PROSPERO (CRD42017078208). PMID: 30590549 [PubMed - as supplied by publisher]

Safety and Efficacy of Anti-PD-1 Monoclonal Antibodies in Patients With Relapsed or Refractory Lymphoma: A Meta-Analysis of Prospective Clinic Trails
Conclusions: Nivolumab and pembrolizumab have promising outcomes with tolerable AEs and drug-related deaths in patients with relapsed or refractory lymphoma. Pembrolizumab caused less any grade AEs like fatigue and rash than nivolumab. Patients with HL got better response than NHL. Introduction Programmed cell death protein 1 (PD-1) is an immune checkpoint receptor mainly expressed on activated T cells, natural killer cells, and B cells (Ishida et al., 1992). The PD-L1 and PD-L2 are its known ligands, which interact with PD-1 on T cells and prevent T-cell activation and proliferation. PD-L1 is expressed on macropha...

A Systematic Review of the Evidence Behind Use of Reduced Doses of Acetaminophen in Chronic Liver Disease.
Authors: Schweighardt AE, Juba KM Abstract Acetaminophen is among the most commonly used nonopioid analgesics, but significant variation exists in its prescribing practices for cirrhosis patients. Our primary objective was to describe the quality of evidence supporting or refuting the use of acetaminophen in patients with hepatic dysfunction. A comprehensive literature review of PubMed, Cochrane Library, Web of Science, and International Pharmaceutical Abstracts using the search terms "acetaminophen," "paracetamol," "chronic liver disease," "cirrhosis," and "hepatic disease" for studies describing changes in acetam...

Pain Management Strategies To Reduce Opioid Use Following Total Knee Arthroplasty.
CONCLUSION: The optimal solution for postoperative TKA pain management has yet to be determined. Although several options exist, many of them have been associated with adverse effects limiting their generalizability. Knee braces, however, have been identified as one potentially successful option. Importantly, knee braces are safe for the majority of patients and should be widely recommended for patient use. Effects of systemic medication on root resorption associated with orthodontic tooth movement: a systematic review of animal studies
ConclusionsThe pharmaceutical substances investigated were shown to exhibit variable effects on root resorption. Although the overall quality of evidence provides the clinician with a cautious perspective on the strength of the relevant recommendations, good practice would suggest that it is important to identify patients consuming medications and consider the possible implications.RegistrationPROSPERO (CRD42017078208)
