Rhinosinusitis in Solid Organ Transplant Recipients: Analysis of 4562 Transplanted Patients.
CONCLUSIONS: Interestingly, acute fulminant infection or sinusitis complications in solid organ transplant patients were much lower than expected. CRS in this patient group was less frequent than expected as well. Whether chronic immunosuppression minimizes the likelihood for CRS deserves further investigation. A more surgically oriented approach in CRS patients may be favored early in the management course of medically refractory patients in light of patients excellent outcomes. Resection of Inverted Papilloma of the Maxillary Sinus via a Prelacrimal Recess Approach: A Multicenter Retrospective Analysis of Surgical Efficacy.
Conclusions These multicenter follow-up results demonstrated that the PLRA is a safe and effective method for the excision of primary or recurrent IPMS with lower postoperative complications and recurrent rate. PMID: 30251558 [PubMed - as supplied by publisher]

Understanding the Propensity for Chronic Sinusitis in Patients on Immunosuppressive Therapy.
Conclusion CRSi patients exhibit histopathology and disease severity more similar to CRSsNP with trends toward increased neutrophilia and reduced fibrosis. In the appropriate clinical context, discontinuing or changing a patient's immunosuppressive regimen may be a valid treatment option in patients with CRSi. This study provides initial insight into understanding the propensity for chronic sinusitis in patients undergoing immunosuppressive treatment which may have implications on disease management. Evolution in the Surgical Management of Chronic Rhinosinusitis: Current Indications and Pitfalls
Chronic rhinosinusitis (CRS) consists of a range of inflammatory conditions in the sinuses that may result in clinical symptoms and the underlying pathophysiology and its relationship to lower airway disease is complex. Current definitions of CRS may serve more as an indication for potential surgical intervention rather than a marker of disease state. CRS can be asymptomatic and may require medical management to avoid disease progression and to minimize the risk of lower airway disease. Endoscopic surgery has undergone a significant evolution and refinement but the most common surgical complication remains persistent infla...

Technical notes on the endoscopic endonasal approach to the craniovertebral junction for odontoidectomy.
CONCLUSION: Endoscopic endonasal odontoidectomy offers excellent exposure and less morbidity than traditional transoral approaches. This technique should be considered in appropriately selected patients. PMID: 29644899 [PubMed - in process]

Age-related prevalence of chronic rhinosinusitis and nasal polyps and their relationships with asthma onset.
CONCLUSION: This study found distinct age-related patterns of CRSwNP and asthma and demonstrated their significant associations in a general population. However, low prevalence of asthma in CRSwNP is in sharp contrast to the findings in Western populations, which warrants further investigation for ethnic or regional difference in CRSwNP-asthma relationships. Corticosteroid nasal irrigations are more effective than simple sprays in a randomized double ‐blinded placebo‐controlled trial for chronic rhinosinusitis after sinus surgery
ConclusionIn the setting of diffuse or patchy CRS disease, the use of corticosteroid delivered by nasal irrigation is superior to simple nasal spray in postsurgical patients.

Comparison between endoscopic and external dacryocystorhinostomy by using the Lacrimal Symptom Questionnaire: A pilot study.
CONCLUSION: Our study indicated that both endonasal and external DCR can lead to improvement in quality of life by using a validated questionnaire. Although there are differences in age and anatomic success rates between the two groups, subgroup analyses indicated that the differences in the Lac-Q scores persisted when age and anatomic patency were removed as potential confounding factors. Further larger, randomized studies would be helpful. PMID: 29336290 [PubMed - in process]

How often is sinus surgery performed for chronic rhinosinusitis with versus without nasal polyps?
CONCLUSION: Almost 30% of ESS were performed for CRSwNP, and these cases were, on average, more extensive, used more OR time, and more often used image guidance than surgeries for CRSsNP. The rate of surgery performed for CRSwNP varied based on geography, payer, and surgical volume, which indicted that patient selection impacted surgical management. Relationships between upper airway diseases and lower airway obstruction in the adult Korean general adult population
Conclusions: Smoking may be a confounder of the relationships between nasal polyps and lower airway obstruction in a general population samples. However, considering the heterogeneity of nasal polyps, phenotype investigation is warranted.

Asthma onset pattern and patient outcomes in a chronic rhinosinusitis population
ConclusionWhile CRS‐specific and asthma‐specific patient‐reported outcome measures (PROMs) were not significantly different among groups, CRS patients with late‐onset asthma had poorer physical function, more frequent nasal polyposis, and required increased treatment for CRS. Late‐onset asthma may predict more severe disease in CRS.

Vitamin D ₃ deficiency and its association with nasal polyposis in patients with cystic fibrosis and patients with chronic rhinosinusitis.
CONCLUSION: VD3 deficiency seemed to be associated with the presence of nasal polyps in the patients with CRS and in the patients with CF in a similar manner. The lower the level of serum VD3, the more severe the mucosal disease was found in the imaging studies and the more frequent microbial colonization of the patients with CF and the patients with CRS. Prevalence of eosinophilic esophagitis in patients with gastroesophageal reflux symptoms: A cross-sectional study from a tertiary care hospital in North India
ConclusionIn this hospital-based study from northern part of India, prevalence of EoE in patients with GERD was 3.2%. EoE should be considered as a diagnostic possibility, especially in those with history of allergy, no-response to PPI, and absolute eosinophil count of ≥250/cumm.
