Disagreement about perceptions of patient disability between the stroke patient and caregiver: a cross sectional study exploring the association to patient and caregiver quality of life
CONCLUSION: Findings suggest the association of disagreement with QOL differs between patients and caregivers. Further research is needed to carefully appraise the relationship and interaction between patient and caregiver.PMID:36036267 | DOI:10.1080/13607863.2022.2116400

NIH launches grant program aimed at closing the funding rate gap between Black and white investigators
After having one idea batted down last year, some National Institutes of Health (NIH) institutes are taking a new tack to bolster the success rate of Black scientists and researchers from other underrepresented groups seeking research grants. A program aiming to diversify the NIH workforce could award up to $20 million a year to neuroscience, drug abuse, and mental health investigators from minority groups. The program will create a new class of NIH’s standard R01 research grant designed to “encourage a more diverse pool of PIs [principal investigators],” said Walter Koroshetz, director of the National Institute...

Cardiovascular risk prediction in healthy older people
AbstractIdentification of individuals with increased risk of major adverse cardiovascular events (MACE) is important. However, algorithms specific to the elderly are lacking. Data were analysed from a randomised trial involving 18,548 participants  ≥ 70 years old (mean age 75.4 years), without prior cardiovascular disease events, dementia or physical disability. MACE included coronary heart disease death, fatal or nonfatal ischaemic stroke or myocardial infarction. Potential predictors tested were based on prior evidence and using a m achine-learning approach. Cox regression analyses were used to calculate 5-year p...

Acute stroke treatment and outcome in the oldest old (90  years and older) at a tertiary care medical centre in Germany-a retrospective study showing safety and efficacy in this particular patient population
Stroke is among the most common causes of death and disability worldwide. Despite the relevance of stroke-related disease burden, which is constantly increasing due to the demographic change in industrialized ...

Combined association of multiple chronic diseases and social isolation with the functional disability after stroke in elderly patients: a multicenter cross-sectional study in China
Multiple chronic diseases (MCDs) and social isolation are independent risk factors related to stroke and disability, but it is unknown whether the combination of these two conditions resulted from aging-relate...

Emergent and Non-Emergent Agitation in the Older Adult: Evaluation and Management
AbstractPurpose of ReviewThis paper aims to describe the heterogenous presentation of agitation in older adults. It will delve into semiology, diagnosis, classification, as well as treatment options available to address agitation.Recent FindingsAgitation is a common and distressing symptom that poses risk to patients and caregivers. In older adults, it is often a manifestation of medical and neuropsychiatric conditions such as dementia, stroke, delirium, psychiatric disorders, catatonia, substance intoxication or withdrawal. Agitation in dementia is defined as excessive motor behavior and verbal or physical aggressive beha...

Survival and outcomes for stroke survivors living in care homes: a prospective cohort study
ConclusionsRates of secondary stroke prevention prescribing increased over 20 years but remained lower in care home residents. The lower levels of rehabilitation received by stroke survivors in care homes, despite their higher levels of disability, suggest a gap in care and urgent need for restorative and/or preventative rehabilitation.

The role of medications in successful aging
Climacteric. 2021 May 12:1-8. doi: 10.1080/13697137.2021.1911991. Online ahead of print.ABSTRACTSuccessful aging includes good health and low levels of disability. To that end, primary prevention is far better than managing subsequent organ damage. When medication is needed to prevent or manage disease, the preferred choice should be associated with the greatest benefits and fewest adverse effects. Cardiovascular diseases are the leading cause of morbidity and mortality in postmenopausal women worldwide. Considering disease-adjusted life years, other leading causes are chronic obstructive pulmonary disease, diabetes mellit...

Different Strokes for Impaired Folks: A Medical-Legal Case Report on Psychosis and Grave Disability following Cerebrovascular Accident
At least 30% of cerebrovascular accident (CVA) survivors suffer from neuropsychiatric symptoms as a direct sequela of their stroke, with poor long-term outcomes due to underdiagnosis and undertreatment. A 2018 systematic review of 76 studies looking at psychotic symptoms following stroke found an increased prevalence in delusional disorders, schizophrenia-like diagnoses, and mood disorders with congruent psychotic features. Right hemispheric lesions accounted for approximately 80% of patients demonstrating subsequent psychotic symptoms in studies with identification of lesion location.

Impact of endovascular reperfusion therapy in nonagenarians with anterior circulation large-vessel ischaemic stroke
ConclusionsSuccessful reperfusion therapy improves the functional outcome of nonagenarians who should not be excluded from EVT. The presence and severity of comorbidities should be considered in the procedural management of this vulnerable population.

IMAGINE study protocol of a clinical trial: a multi-center, investigator-blinded, randomized, 36-month, parallel-group to compare the effectiveness of motivational interview in rehabilitation of older stroke survivors
Rehabilitation pathways are crucial to reduce stroke-related disability. Motivational Interviewing (MI), as a person-centered complex intervention, aimed to empower and motivate, and could be a resource to imp...

Stress, Burden, and Well-Being in Dementia and Non-Dementia Caregivers: Insights from the Caregiving Transitions Study.
DISCUSSION AND IMPLICATIONS: Dementia and non-dementia caregivers showed relatively few differences in indicators of overall well-being in this population-based sample, perhaps because both groups of caregivers in this study were providing substantial care. Dementia caregivers may require special assistance with dementia-specific problems such as behavioral problems. Clinical interventions and policy changes targeting highly burdened caregivers are needed to support them in allowing their care recipients to age in place at home.
