Surgical left atrial appendage amputation/ligation (LAA-A/L) during off-pump coronary artery bypass grafting (OPCAB) is routinely performed in our institution. Clinical outcomes and management associated with major bleeding in patients with atrial fibrillation treated with apixaban or warfarin: insights from the ARISTOTLE trial
Conclusion Major bleeding was associated with substantially increased risk of death, ischaemic stroke, or MI, especially following ICH, and this risk was similarly elevated regardless of treatment with apixaban or warfarin. Abstract 198: Racial Disparity In Resuming Anticoagulation For Atrial Fibrillation After An Episode Of Major Gastrointestinal Bleeding Poster Session II
Conclusion: In conclusion, the racial disparity in resuming warfarin after an episode of major GIB in anticoagulated patients for atrial fibrillation was more for Caucasians than African American. This may be explained by uncertainty of outcomes that frequently leads to overtreatment of minority patients. Cultural beliefs for not resuming warfarin might have also played a role.

Nonvalvular atrial fibrillation (AF) confers an increased risk of thromboembolism, with a 5-fold higher risk of ischemic stroke. Simultaneous hybrid carotid stenting and coronary bypass surgery versus concomitant open carotid and coronary bypass surgery: a pilot, feasibility study ADULT CARDIAC
CONCLUSIONS Although limited by a small sample size, our results show that the hybrid procedure of carotid stenting and coronary surgery might be a good therapeutic option but further extended studies are needed to assess its real value.

Frequency, Trend, Predictors, and Impact of Gastrointestinal Bleeding in Atrial Fibrillation Hospitalizations
Atrial fibrillation (AF) is associated with a five-fold increased risk of stroke 1. Anticoagulation alone or in combination with other treatment strategies are implemented to reduce the risk of stroke in patients with AF. Gastrointestinal bleeding (GIB) is a common complication of oral anticoagulation (OAC) with a prevalence of 1%-3% in patients on long term OAC2. GIB in AF patients leads to discontinuation of anticoagulation, which can increase the risk of ischemic stroke, subject patients to endoscopic procedures, blood transfusions, and related side effects.

Conclusions: OPCAB compared with CCAB may improve outcomes in the short-term (stroke, renal dysfunction, blood transfusion, respiratory failure, atrial fibrillation, wound infection, ventilation time, and length of stay). Variation in Warfarin Use at Hospital Discharge After Isolated Bioprosthetic Mitral Valve Replacement - An Analysis of the Society of Thoracic Surgeons Adult Cardiac Surgery Database.
CONCLUSIONS: Although patient characteristics and post-operative events may be associated with the prescription of warfarin following BMVR, substantial surgeon and hospital variability remains.
