Risk Factors Influencing Outcomes of Atrial Fibrillation in ALLHAT.
CONCLUSIONS: AF/AFL is a significant risk factor for stroke, HF, and mortality. Additional risk factors for these outcomes were generally similar for participants with and without baseline or incident AF/AFL. PMID: 30126559 [PubMed - in process]

Oral Anticoagulants in Japanese Patients with Atrial Fibrillation and Active Cancer.
Conclusion The rates of stroke or systemic embolism and major bleeding events were not negligible among Japanese cancer patients with AF receiving OACs. Further investigations on the optimal management of Japanese patients with AF and cancer are needed. PMID: 30799369 [PubMed - as supplied by publisher]

Detection of Atrial Fibrillation and Atrial Flutter by Pacemaker Device Interrogation After Transcatheter Aortic Valve Replacement (TAVR): Implications for Management.
CONCLUSION: Subclinical NOAF is common after TAVR, usually occurs months after hospital discharge, and is associated with lack of anticoagulation therapy and increased risk of stroke. Prolonged surveillance of subclinical NOAF may be warranted after TAVR. PMID: 31257211 [PubMed - in process]

A Comprehensive Evaluation of Rhythm Monitoring Strategies in Screening for Atrial Fibrillation: Insights from Patients at Risk Long-Term Monitored with Implantable Loop Recorder.
Conclusions: In screening for AF among participants with stroke risk factors, the diagnostic yield increased with duration, dispersion and number of screenings, although all strategies had low yield compared to ILR. The sensitivity was higher among participants who were older, males, or had higher NT-proBNP. Clinical Trial Registration: URL: https://clinicaltrials.gov Unique Identifier: NCT02036450. Factors that influence awareness and treatment of atrial fibrillation in older adults
Conclusions: With projected doubling of numbers of persons over 80 in the next 30 years in the British Isles, detection and management of AF is pressing. Two-thirds of adults at high risk of stroke were inadequately treated. More regular screening for AF, application of criteria for stroke and bleeding risk and awareness of factors influencing diagnosis and treatment is recommended.

The Atrial Fibrillation in Turkey: Epidemiologic Registry AFTER.
This study aims to evaluate stroke risk in non-valvular atrial fibrillation (AF) and anticoagulant drug utilization within conformity to AF guidelines. AFTER (Atrial Fibrillation in Turkey: Epidemiologic Registry) is a prospective, multicentre study designed with the aim of describing the prevalence and epidemiology of AF practice in Turkey. Methods; Patients were recruited in 17 referral hospitals reflecting all the population of seven geographical regions of Turkey. 2242 consecutive patients who had been admitted with AF on their electrocardiogram were included in the study. 1745 of these patients, who had non-valvular AF...

Imaging the aortic arch to improve diagnostic and prognostic accuracy in transient ischemic attack patients
This study raises a final relevant question: since the prognosis of TIA patients worsens in proportion to the severity of AAA, what therapeutic options are available for these patients? The clinical guidelines recommend risk factor modification and therapy with statins (Class IIa, level of evidence C), an approach consistent with treatment of any patient affected by atherosclerosis. Treatment with coumadin or anti-platelet agents can be considered in patients with AAA >4 mm, i.e. severe AAA, but it remains very controversial because of a potential risk of thrombo-embolism (Class IIb, level of evidence C) . Finally, surgi...

Painkiller use linked to irregular heartbeat
ConclusionThis prospective cohort study claims an association between NSAIDs usage and developing atrial fibrillation. However, there are many limitations to this research.Despite this being a large prospective cohort study that followed people over a period of time, the assessments within it were predominantly cross-sectional. That means it assessed the person’s current or recent prescription of NSAIDs at the time they were diagnosed, but this cannot prove that using NSAIDs caused atrial fibrillation. A better method may have been to assess the use of NSAIDs in people without atrial fibrillation at the start of the stud...

Atrial fibrillation: Cryptogenic stroke—can we abandon this apologetic diagnosis?
Nature Reviews Cardiology 11, 504 (2014). doi:10.1038/nrcardio.2014.111 Author: A. John Camm Cryptogenic stroke is an apology for ignorance about the cause of ischaemic stroke. Now, in two new studies involving long-term electrocardiogram monitoring, investigators from the EMBRACE and CRYSTAL-AF trials suggest that many instances of cryptogenic stroke might be caused by undetected atrial fibrillation.

P-wave dispersion and left atrial indices as predictors of paroxysmal atrial fibrillation in patients with non hemorrhagic cerebrovascular strokes and transient ischemic attacks
Conclusion PAF is a possible etiology of patients with ischemic cerebrovascular accidents patients even in those who had normal sinus rhythm on admission. Pmax ⩾125mm, Pdis ⩾40mm, and LAVI⩾24 are highly significant predictors of PAF with PPV of 99%, 96% and 78%, respectively, sensitivity of 98%, 94% and 89%, respectively, specificity 96%, 93% and 75%, respectively and AUC of 0.99, 0.98 and 0.87, respectively.

Feasibility of Extended Ambulatory Electrocardiogram Monitoring to Identify Silent Atrial Fibrillation in High‐risk Patients: The Screening Study for Undiagnosed Atrial Fibrillation (STUDY‐AF)
ConclusionsOutpatient extended ECG screening for asymptomatic AF is feasible, with AF identified in 1 in 20 subjects and sustained AT/AF identified in 1 in 9 subjects, respectively. We also found a high prevalence of asymptomatic AT and frequent supraventricular ectopic complexes, which may be relevant to development of AF or stroke. If confirmed in a larger study, primary screening for AF could have a significant impact on public health.

Biotronik wins FDA nod for BioMonitor 2
Biotronik said today it won FDA premarket approval for its BioMonitor 2 magnetic resonance imaging-compatible insertable cardiac remote monitor. The BioMonitor 2 device is designed for remote monitoring of patients with atrial fibrillation, syncope, bradycardia and tachychardia, Biotronik said. The device uses Biotronik’s ProMRI technology and is approved for full-body MRI scans at 1.5 and 3 tesla strength. “Accurate diagnosis is the first step towards saving and improving patient lives. Too many patients suffer from undiagnosed heart conditions that lead to traumatic events including stroke and heart attack. BioMo...

Prevalence of Atrial Fibrillation and Antithrombotic Therapy in Hemodialysis Patients: Cross-Sectional Results of the Vienna InVestigation of AtriaL Fibrillation and Thromboembolism in Patients on HemoDIalysis (VIVALDI)
ConclusionsThe prevalence of AF is high amongst HD patients and is associated with age, sex, and distinct comorbidities. Practice patterns of antithrombotic treatment indicate a lack of consensus for stroke prevention in HD patients with AF.

Impact of atrial fibrillation detected by extended monitoring —A population‐based cohort study
ConclusionsTele‐ECG identifies significantly more AF cases in a population‐based setting compared to conventional ECG. The impact of AF diagnosed only by extended monitoring differs from conventionally diagnosed AF. Additional studies are warranted, since this might have an impact on clinical management.
