Paradoxical low-flow aortic stenosis is defined by increased ventricular hydraulic load and reduced longitudinal strain

Medtronic plc (NYSE:MDT) today announced FDA approval and U.S. launch of the CoreValve ™ Evolut™ PRO valve for the treatment of severe aortic stenosis for symptomatic patients who are at high or extreme risk for open heart surgery. The approval comes on the heels of new 30-day clinical data that was unveiled at the American College of Cardiology (ACC) 66th Annual Scientific Sessio n, which showed high survival, low rates of stroke, minimal paravalvular leak (PVL) and excellent hemodynamics for the self-expanding valve.

Comparing characteristics and clinical and echocardiographic outcomes in low ‐flow vs normal‐flow severe aortic stenosis with preserved ejection fraction in an Asian population
ConclusionsLow‐flow AS despite normal LVEF appears similar to normal‐flow in terms of LVEF deterioration and clinical outcomes in our Asian population. AVR rate was lower even though low‐flow may not reflect less severe disease.

Conclusion: The presence of LVH by Sokolow-Lyon and Cornell ECG voltage criteria poorly correlates with the presence of LVH and critical aortic stenosis in TAVR patients. Women are more likely to have voltage criteria for LVH. Therefore, ECG may not be a suitable method of screening patients with severe aortic stenosis for LVH, especially in men.
