Impact of baseline left ventricular ejection fraction on outcome after transfemoral transcatheter aortic valve implantation in patients with and without low ‐gradient aortic stenosis
ObjectivesTo evaluate the impact of baseline left ventricular ejection fraction (LVEF) and its interaction with low ‐gradient aortic stenosis (LGAS) on all‐cause mortality after transfemoral aortic valve implantation (TF‐TAVI).MethodsWe reviewed mortality data of 624 consecutive single center TF ‐TAVI patients and categorized LVEF according to current ASE/EACVI recommendations (normal, mildly‐, moderately‐, and severely abnormal).ResultsBaseline LVEF was normal in 336 (53.8%), mildly abnormal in 160 (25.6%), moderately abnormal in 91 (14.6%), and severely abnormal in 37 (5.9%) patients, and 1 ‐year mortality ...

Trends in use of echocardiography in hospitalized patients with syncope
PurposeWe sought to assess the trends in use, predictors of echocardiography, and its impact on in ‐hospital mortality in patients admitted with syncope using a large national database.MethodsUtilizing the Nationwide Inpatient Sample (NIS) database from 2001 to 2014, we identified adult patients (>18  years) with a primary discharge diagnosis of syncope and use of echocardiogram was ascertained.ResultsA total of 3  174 619 patients with a primary discharge diagnosis of syncope were identified, of which 184 167 (5.8%) underwent an echocardiogram. The rate of syncope hospitalization remained constant...

Teamwork using strain imaging in the echocardiographic assessment of right ventricular systolic function: A cardiac magnetic resonance imaging correlation study
AimThe aim of this study was to investigate whether conventional echocardiographic assessment of right ventricular (RV) systolic function can be improved by the addition of RV strain imaging. Additionally, we also aimed to investigate whether dedicated reading sessions and education can improve echocardiographic interpretation of RV systolic function.MethodsReaders of varying expertise (staff echocardiologists, advanced cardiovascular imaging fellows, sonographers) assessed RV systolic function. In session 1, 20 readers graded RV function of 19 cases, using conventional measures. After dedicated education, in session 2, th...

Double ‐chambered left ventricle: A rare case in a child
Double ‐chambered left ventricle (DCLV) is an extremely rare congenital heart disease. In this condition, the left ventricle is divided into two chambers by a septum or muscle fiber with abnormal proliferation. A symptomatic boy was diagnosed with DCLV at our hospital. The patient was admitted with the m ajor complaint of 8 years of cardiac murmur, which was discovered through physical examination, and 5 years of palpitations and shortness of breath. He has been followed up without treatment. (Source: Echocardiography)

Impact of appropriate use criteria for transesophageal echocardiograms on clinically meaningful care
IntroductionThe aim of this study was to evaluate appropriateness of transesophageal echocardiography (TEE) studies based on 2011 Appropriate Use Criteria (AUC) for Echocardiography and its impact on patient management. Methods and ResultsIn a retrospective analysis of 100 inpatient TEEs, 85% of the TEEs ordered were determined to be appropriate, 9% were inappropriate, and 6% were uncertain. Inter ‐observer differences in the determination of AUC were seen in 24% of the studies, requiring a senior‐level cardiologist to make the final determination of AUC score. The variance in interpretation by experts is concerning for...

The presbycardia phenotype: Cardiac remodeling and valvular degeneration in nonagenarians
BackgroundNonagenarians (NON) are a growing segment of the population and have a high prevalence of cardiac disease. Many findings encountered on their echocardiograms are also found in younger individuals with valvular or myocardial disease. Therefore, the purpose of this study was to describe this distinct echocardiographic phenotype.MethodsWe identified our study population by querying our echo database to identify unique septuagenarians (SEPT) and nonagenarians (NON) who underwent a transthoracic echocardiogram (TTE) from January 1, 2010 to December 31, 2014. Exclusion criteria were LVEF

Influence of mildly and moderately elevated pulmonary artery systolic pressure on post ‐renal transplantation survival and graft function
BackgroundSevere pulmonary hypertension (PH) has been associated with decreased post –kidney transplant survival and increased rate of long‐term cardiovascular complications. Despite a high prevalence of PH in patients with end‐stage renal disease, data on post‐transplant renal allograft survival in recipients with pre‐existing mild‐to‐moderate PH are limited.MethodsThe single ‐center retrospective study cohort consisted of 192 consecutive (2008–2015) renal transplant recipients with documented pretransplantation transthoracic echocardiogram (TTE) pulmonary artery systolic pressure (PASP). New classification of geometric patterns considering left ventricular volume in patients with chronic aortic valve regurgitation: Prevalence and association with adverse cardiovascular outcomes
BackgroundLeft ventricular (LV) remodeling due to aortic regurgitation (AR) often leads to maladaptive responses. We assessed the prevalence and clinical implications of LV remodeling considering LV volume, mass, and relative wall thickness at the time of AR diagnosis. Methods and ResultsBetween 2008 and 2017, 370 consecutive patients (mean age 67.3  ± 16.1 years, 56.5% males), with moderate or severe AR, were retrospectively analyzed. LV geometric patterns and clinical outcomes (cardiovascular death, hospitalization for heart failure, or aortic valve replacement) were evaluated. LV dilatation (LV end...

Physiological left ventricular segmental myocardial mechanics: Multiparametric polar mapping to determine intraventricular gradients of myocardial dynamics
ConclusionThis study generated a database of seven STE ‐derived parameters of physiological segmental and global myocardial LV mechanics. The resulting sets of three‐dimensional intraventricular mappings of the entire LV provide physiological parameter gradients in baso‐apical and circumferential direction by applying the 17‐segment polar model. This will facilitate comparison of systolic myocardial activity of the healthy LV with diseased or otherwise altered (eg, sports) hearts. (Source: Echocardiography)
