Hypersensitivity reactions to folinic acid: mechanisms involved based on two case reports and a literature review
CONCLUSIONS: These two cases of anaphylaxis to FA document the clinical manifestations associated with two different mechanisms of HSR. This paper provided the opportunity to review the limited literature on HSR to FA. Through these cases, we hope to draw the practitioner's attention to FA as a potential agent of severe hypersensitivity, especially if symptoms remain after withdrawing the most suspected chemotherapeutic agents. We want also to stress the importance of allergy testing.PMID:36550565 | PMC:PMC9783773 | DOI:10.1186/s13223-022-00752-5

Primary sinonasal lymphoma: A multi ‐institutional experience of clinical presentation, treatment, and outcomes
ConclusionsThis is the largest multi-institutional analysis of SL characteristics and outcomes. Our work suggests that while disease control in the first 5 years is reasonable, 10-year outcomes remains challenging. Future studies investigating new treatment paradigms and risk stratification are needed. This article is protected by copyright. All rights reserved

IL-6 and Cytokine Release Syndrome: a new understanding in drug hypersensitivity reactions
Ann Allergy Asthma Immunol. 2022 Nov 4:S1081-1206(22)01905-6. doi: 10.1016/j.anai.2022.10.025. Online ahead of print.ABSTRACTImmediate drug hypersensitivity reactions (DHRs) are historically thought to be due to IgE cross-linking, causing mast cell degranulation and release of mediators like tryptase and histamine. With the increasing use of monoclonal antibodies, it has become apparent that some patients present atypical features during immediate DHRs, including occurrence in initial exposure, a lack of urticaria and angioedema, and presence of fever, chills rigors and musculoskeletal pain as the predominant symptoms. Thi...

A case of acute focal bacterial nephritis caused by methicillin-resistant < em > Staphylococcus saprophyticus < /em > in a 13-year-old adolescent girl treated with daptomycin
IDCases. 2022 Aug 6;29:e01594. doi: 10.1016/j.idcr.2022.e01594. eCollection 2022.ABSTRACTStaphylococcus saprophyticus is a gram-positive, coagulase-negative member of the Staphylococcus genus and is second only to Escherichia coli as a cause of urinary tract infections in the young female population. S. saprophyticus usually has good susceptibility to drugs commonly used to treat urinary tract infections, but it is often methicillin-resistant. Here we report a case of acute focal bacterial nephritis in a 13-year-old female patient caused by methicillin-resistant S. saprophyticus and treated with daptomycin (DAP). The patie...

Why Acupuncture Is Going Mainstream in Medicine
When the opioid addiction crisis began to surge in the U.S. about a decade ago, Dr. Medhat Mikhael spent a lot of time talking to his patients about other ways to heal pain besides opioids, from other types of medications to alternative treatments. As a pain management specialist at MemorialCare Orange Coast Medical Center in Fountain Valley, Calif., he didn’t anticipate leaving behind the short-term use of opioids altogether, since they work so well for post-surgical pain. But he wanted to recommend a remedy that was safer and still effective. That turned out to be acupuncture. “Like any treatment, acupuncture...

Impact of palliative treatment on survival in sinonasal malignancies
ConclusionIn this study we identify the impact of palliative treatment modalities on survival in sinonasal cancer. Palliative surgery was shown to have the highest 1Y ‐OS and M‐OS compared with other palliative treatment modalities.

Desensitization protocol for hypersensitivity to carboplatin in-patient with metastatic cancer, a safe and effective method
Use of chemotherapy compounds in clinical practice is increasing leading to a rise in the incidence of hypersensitivity reactions. Reactions to carboplatin usually occur between the fourth and eighth course of chemotherapy. The reaction manifests with rash, dyspnea, tightness in the chest, hypotension/hypertension, edema, back pain.

Analysis of tolerance and security of chemo hyperthermia with Mitomycin C for the treatment of non-muscle invasive bladder cancer.
CONCLUSIONS: QHT has proven to be a safe alternative for the treatment of intermediate and high risk NMIBC, with AE mainly grade 1-2. The AEs reported have little variation with respect to the dose of MMC used, presenting different "profiles" related to the device used for its administration. The treatments with QHTMMC are well tolerated, without adding significantly more AE than the instillations of MMC alone and presenting a better toxicity profile than those reflected in the literature with respect to the treatment with BCG. What Causes Flushing?
Discussion Flushing is usually thought of as a benign condition such as blushing, but can be quite irritating if it is frequent or persistent (i.e. menopausal hot flashes) or even pathologic (i.e. malignancy). “Flushing is a subjective and sensation of warmth that is accompanied by reddening of the skin anywhere on the body but favors the face, neck, and upper torso…. Flushing can be broadly divided into episodic or persistent. Episodic flashing is mediated by the release of endogenous vasoactive mediators or medications, while persistent flushing result in a fixed facial erythema with telangiectasias and cyano...

Lymphoma of the nasal cavity and paranasal sinuses: A case series.
CONCLUSION: Sinonasal lymphomas are characterized by meager survival rates, which differ by histopathologic subtype. The diagnosis of sinonasal lymphoma is challenging because symptoms frequently parallel those of chronic rhinosinusitis. Increased awareness of these rare malignancies may improve detection and more timely treatment. Clinical trial registration NCT01332136. PMID: 27657899 [PubMed - in process]

Stiff person syndrome presenting as 'stiff eyes'
A 47-year previously fit and well lady presented with 6 weeks history of generalised lethargy, weight loss and neck pain. Her examination revealed left supra-clavicular lymph nodes. Subsequent investigations revealed left grade 2 invasive ductal carcinoma of her left breast. This was treated with local excision. She presented to hospital four weeks following her surgery with progressive double vision and unsteadiness. Her cranial nerve examination showed severly restricted pursuit and saccadic horizontal eye movements with intact vertical eye movements. She had normal strength with intact deep tendon reflexes with plantar ...
