E-152 Cone-beam CT angiogram acquisition in the angiography suite for LVO detection in acute ischemic stroke
ConclusionDTAS triage workflow with cone-beam head CT and CTA image acquisition in the angiography suite may be a next step to further optimize LVO stroke pathways by bypassing conventional CT/MR. This new workflow has the potential to further decrease door-to-needle times and thereby may improve patient outcome.Disclosures A. Kuhn: None. J. Singh: None. M. Garcia: None. S. Sarid: None. M. Current advances in ischemic stroke research and therapies
Publication date: Available online 15 September 2018Source: Biochimica et Biophysica Acta (BBA) - Molecular Basis of DiseaseAuthor(s): Derek Barthels, Hiranmoy DasAbstractWith more than 795,000 cases occurring every year, stroke has become a major problem in the United States across all demographics. Stroke is the leading cause of long-term disability and is the fifth leading cause of death in the US. Ischemic stroke represents 87% of total strokes in the US, and is currently the main focus of stroke research. This literature review examines the risk factors associated with ischemic stroke, changes in cell morphology and s...

Case-crossover analysis of short-term particulate matter exposures and stroke in the health professionals follow-up study.
CONCLUSIONS: Our study provides evidence that ambient PM10 may be associated with higher risk of ischemic stroke and highlights that ischemic and hemorrhagic strokes are heterogeneous outcomes that should be treated as such in analyses related to air pollution. Incidence and Current Treatment Gaps in Pediatric Stroke and TIA: An Ontario-Wide Population-Based Study
Conclusion: Our population-based study yielded a higher incidence rate than prior North-American studies. Important care gaps exist including delayed diagnosis, lack of expert care, and departure from published treatment guidelines. Variability in ICD PPV and yield underlines the need for prospective data collection and for improving the pediatric stroke and TIA coding processes.Neuroepidemiology 2019;52:119 –127

Acute ischemic stroke: improving access to intravenous tissue plasminogen activator.
This article gives an overview of the recent approaches pursued to improve access to alteplase for acute ischemic stroke patients. Utilization of stroke systems of care, quality metrics, and quality improvement initiatives to improve alteplase treatment rates are discussed. The implementation of Telestroke networks to improve access and timely evaluation by a stroke specialist are also reviewed. Lastly, this review discusses the use of neuroimaging techniques to identify alteplase candidates in stroke of unknown symptom onset or beyond the 4.5-hour treatment window.Expert commentary: Expanding access to alteplase therapy f...

Risk of Ischemic Stroke after an Inpatient Hospitalization for Transient Ischemic Attack in the United States
Conclusions: Patients hospitalized for TIA have a lower risk of ischemic stroke compared to that reported in the studies based on the emergency room and/or outpatient clinic evaluation. Among these patients, those with cardiovascular comorbidities remain at a higher risk of readmission due to ischemic stroke despite undergoing an inpatient evaluation and should therefore be the target for future preventive strategies.Neuroepidemiology

Types of Stroke Among People Living With HIV in the United States
Conclusion: Ischemic stroke, particularly small vessel and cardioembolic subtypes, were the most common strokes among PLWH. Traditional and HIV-related risk factors differed by stroke type/subtype. Precipitating factors including infections and drug use were common. These results suggest that there may be different biological phenomena occurring among PLWH and that understanding HIV-related and traditional risk factors and in particular precipitating factors for each type/subtype may be key to understanding, and therefore preventing, strokes among PLWH.

HIV Viremia and Risk of Stroke Among People Living with HIV Who Are Using Antiretroviral Therapy
Conclusions: Our findings are consistent with the hypothesis that elevated HIV VL may increase stroke risk, regardless of previous VL levels.

E-102 Transport distances for bypass and non-bypass routing models of stroke patients in the United States
ConclusionsBypass incurs a modest distance penalty for non-LVO patients and offers a modest distance benefit for LVO patients. These differences in distance are likely insignificant to intra-hospital delays that contribute greatly to delays in EVT treatment in non-bypass transport. Based on current guidelines, bypass of the nearest stroke center for an EVT-capable center may be warranted for a majority of Americans.Disclosures C. Yu: None. P. Panagos: None. A. Kansagra: None.

An Inflection Point for Stroke Care
In the past six months, stroke care in the United States has taken an epoch-changing turn. Techniques, technologies, and treatment pathways hinted at in earlier trials have become fully vetted and are a new gold standard giving doctors more time to treat stroke patients and help them regain pre-stroke capabilities. Two recent trials, especially, DAWN and DEFUSE 3, have precipitated industry-wide shifts in the way hospitals treat stroke, especially acute ischemic stroke. "I guess the best thing I can say is we have altered our practice here, essentially immediately, based on the results of those studies. I guess that shows ...

Roles of integrins and extracellular matrix in stroke.
Abstract Ischemic stroke is a leading cause of death and disability in the United States, but recent advances in treatments (i.e. endovascular thrombectomy and tissue plasminogen activator (t-PA)) that target the stroke-causing blood clot, while improving overall stroke mortality rates, have had much less of an impact on overall stroke morbidity. This may in part be attributed to the lack of therapeutics targeting reperfusion induced injury after the blood clot has been removed, which, if left unchecked, can expand injury from its core into the surrounding at risk tissue (penumbra). This occurs in two phases of in...

Nerve stimulation could provide new treatment option for most common type of stroke
Research led by a UCLA scientist found that a new nerve stimulation therapy to increase blood flow could help patients with the most common type of stroke up to 24 hours after onset.A study of 1,000 patients found evidence that the technique, called active nerve cell cluster stimulation, reduced the patients ’ degree of disability three months after they suffered an acute cortical ischemic stroke, which affects the surface of the brain.Dr. Jeffrey Saver, director of theUCLA Comprehensive Stroke Center, was the co-principal investigator of the study, which was conducted at 73 medical centers in 18 countries.“We believe ...

Time to Target Stroke: Examining the Circadian System in Stroke.
Authors: Stubblefield JJ, Lechleiter JD Abstract Stroke is the 5th leading cause of death in the United States and a leading cause of long-term disability. Ischemic strokes account for 87 percent of total stroke cases, yet the only FDA-approved treatments involve disruption of the blood clot to restore blood flow. New treatments aimed at saving or protecting neural tissue have largely failed in clinical trials and so new methodology or targets must be found. The occurrence of strokes significantly increases between 6 AM and 12 PM, implicating the circadian system in the onset of this debilitating brain injury. But ...

Use of insertable cardiac monitors for the detection of atrial fibrillation in patients with cryptogenic stroke in the United States is cost-effective.
Authors: Maervoet J, Bossers N, Borge RP, Thompson Hilpert S, van Engen A, Smala A Abstract Atrial fibrillation (AF) is the most common arrhythmia and a major marker of stroke risk. Early detection is crucial and, once diagnosed, anticoagulation therapy can be initiated to reduce stroke risk. The aim of this study was to assess the cost-effectiveness of employing an insertable cardiac monitor (ICM), BIOMONITOR, for the detection of AF compared to standard of care (SoC) ECG and Holter monitoring in patients with cryptogenic stroke, that is, stroke of unknown origin and where paroxysmal, silent AF is suspected. A Mar...
