Lysosomal Acid Lipase Deficiency Leading to Liver Cirrhosis: a Case Report of a Rare Variant Mutation.
We describe the case of a 53-year-old Brazilian man who was referred to our center due to the diagnosis of liver cirrhosis of unknown etiology. He was asymptomatic and had normal body mass index. He had dyslipidemia, and family history of myocardial infarction and stroke. Abdominal imaging tests showed liver cirrhosis features and the presence of intrahepatic calcifications. Initial investigation of the etiology of the liver disease was not elucidated, but liver biopsy showed microgoticular steatosis and cholesterol esters deposits in Kuppfer cells. The dosage of serum lysosomal acid lipase was undetectable and we found th...

Efficacy and Safety of Anticoagulation for Atrial Fibrillation in Patients with Cirrhosis: A Systematic Review and Meta-analysis
The atrial fibrillation-related stroke is clearly prevented by anticoagulation treatment, however, management of anticoagulation for AF in patients with cirrhosis represents a challenge due to bleeding concerns. To address this issue, a systematic review and meta-analysis of the literature was performed.

Predictors of inadequate bowel preparation for colonoscopy: a systematic review and meta-analysis
The aim of this study was to investigate factors contributing to poor bowel preparation in patients undergoing colonoscopy procedures. We used a reproducible search strategy to identify studies, searching 10 medical databases, including PubMed, Ovid, Medline, and Cochrane Library Database for reports published between 2000 and 2016. Fully published studies, evaluating risk factors for inadequate bowel preparation, were included. Two reviewers independently scored the identified studies for methodology and abstracted pertinent data. Pooling was conducted with both fixed-effects and random-effects models; results were presen...

β-Blocker therapy for cirrhotic cardiomyopathy: a randomized-controlled trial
Background Cirrhotic cardiomyopathy is characterized by an attenuated contractile response to stress. Long-term exposure of β-adrenergic receptors to persistently high levels of catecholamines has been implicated in its pathogenesis. We hypothesized that β-blockade with metoprolol could reverse the changes in heart function and morphology in cirrhotic cardiomyopathy. Patients and methods In this prospective randomized trial, we included 78 patients aged between 18 and 60 years with abnormal cardiac output response under dobutamine stress echocardiography, without primary cardiac disease or a history of alcohol intak...

Ischemic stroke in liver cirrhosis: epidemiology, risk factors, and in-hospital outcomes
Conclusion Ischemic stroke was often observed in patients with cirrhosis, and it significantly increased the in-hospital mortality. The association of inflammation, coagulation disorders, and viral hepatitis with development of ischemic stroke in liver cirrhosis should be further evaluated in prospective cohort studies.

New predictive model for acute gastrointestinal bleeding in patients taking oral anticoagulants: A cohort study
ConclusionsGastrointestinal bleeding increased the risk of subsequent mortality during follow‐up of anticoagulated patients, highlighting the importance of prevention. The study developed a new scoring model for acute GI bleeding risk based on five factors (no‐proton pump inhibitor use, chronic kidney disease, chronic obstructive pulmonary disease, history of peptic ulcer disease, and liver cirrhosis), which was superior to the HAS‐BLED score.

Is cirrhosis associated with lower odds of ischemic stroke: A nationwide analysis?
CONCLUSION: Our study suggests that in a large, nationally representative sample of the United States population, cirrhosis is associated with a lower likelihood of stroke. PMID: 28050237 [PubMed]

Impact of Renal Impairment on Cardiovascular Disease Mortality After Liver Transplantation for Nonalcoholic Steatohepatitis Cirrhosis
ConclusionNASH recipients have an increased risk of CVD mortality after liver transplantation explained by a high prevalence of co‐morbid cardiometabolic risk factors that in aggregate identify those at highest risk of post‐transplant CVD mortality. This article is protected by copyright. Clinical features in liver cirrhosis patients who have experienced ischemic stroke.
CONCLUSION: Our results show that a comparatively high frequency of ischemic stroke occurs in patients with alcohol-induced liver cirrhosis and in patients with a less compromised liver function. In addition, it was found that most patients possessed at least one risk factor of ischemic stroke. PMID: 25599784 [PubMed - in process]

Risk score to predict gastrointestinal bleeding after acute ischemic stroke
Conclusion: The AIS-GIB score is a valid clinical grading scale to predict in-hospital GIB after AIS. Further studies on the effect of the AIS-GIB score on reducing GIB and improving outcome after AIS are warranted.

Reduced thoracic fluid content in early-stage primary biliary cirrhosis that associates with impaired cardiac inotropy
This study has confirmed that TFC is reduced in those with PBC, that this is specific to PBC, and that it associates independently with markers of cardiac inotropy.
