Aortic Root Replacement in Octogenarians Offers Acceptable Perioperative and Late Outcomes
Conclusions Octogenarians can safely undergo aortic root replacement with moderately worse but acceptable perioperative mortality and late survival. Further studies are necessary to determine which subset of octogenarians are at the highest operative risk and may benefit from a conservative approach.

Increasing Evidence for an Association Between Periodontitis and Cardiovascular Disease.
Abstract Periodontitis is a chronic inflammatory disease caused by bacterial colonisation, which results in destruction of the tissues between the tooth surface and gingiva, loss of connective tissue attachment, erosion of alveolar bone and tooth loss.(1) Periodontitis is common and increases with age. In a US survey about half of adults aged over 30 years have some periodontitis and almost 10% have severe disease.(2) Evidence for an association between periodontitis and atherosclerotic vascular disease, including stroke, myocardial infarction, peripheral vascular disease, abdominal aortic aneurysm, coronary heart...

Results of Open Surgical Repair in Patients With Marfan Syndrome and Distal Aortic Dissection
Conclusions In these patients with MFS with aortic dissection, open TAAA repair incurred reasonable operative risk, but improvements are needed to reduce rates of renal failure. Extent II TAAA repair does not appear to increase operative risk in patients with MFS.

The Risk Factors and Outcomes of Acute Kidney Injury after Thoracic Endovascular Aortic Repair.
CONCLUSION: Preoperative stroke and postoperative paraparesis and paraplegia were identified as predictors for AKI. Patients with AKI experienced longer average ICU stays and greater 30-day mortality than those without AKI. Perioperative identification of high-risk patients, as well as nephroprotective strategies to reduce the incidence of AKI, should be considered as important aspects of a successful TEVAR procedure. Thoracic Endovascular Aortic Repair of Aortic Arch Pathologies with the Conformable Thoracic Aortic Graft: Early and 2 year Results from a European Multicentre Registry.
CONCLUSION: The CTAG stent graft shows high deployment accuracy, good conformability, and clinical effectiveness in the treatment of aortic arch pathologies. However, thoracic endovascular aortic repair in the arch is associated with a relatively high stroke rate. Further studies with more patients and longer follow up are needed to evaluate the long-term results. PMID: 27107487 [PubMed - as supplied by publisher]

“Phacing” a New Cause of Carotid Artery Dissection
Conclusions: Noteworthy, among vascular abnormalities of PHACE, CADs have never been reported before. Our 2 cases suggest that CAD is an underecognized cerebrovascular manifestation of PHACE and it should be searched for in these patients. Ultrasound, being noninvasive and portable, is a useful tool for the assessment and follow-up of these patients.

Coexistence of obstructive sleep apnea worsens the overall outcome of intracranial aneurysm: a pioneer study.
CONCLUSIONS Complications of IA such as rupture and vasospasm are often the consequence of uncontrolled OSA. Overall outcome (mRS) of IAs is also affected by the co-occurrence of OSA. Therefore, the coexistence of OSA with IA affects the outcome of IAs. Obstructive sleep apnea is a risk factor for a poor outcome in IA patients. PMID: 28338434 [PubMed - as supplied by publisher]

The use of intraoperative epiaortic ultrasonography in monitoring patients over 75 years old treated with aortic valve replacement.
CONCLUSIONS: Epiaortic ultrasound scanning imaging is superior to TTE and manual palpation in the detection and localization of ascending atherosclerosis. This technique should be introduced as a standard perioperative examination in older patients at risk of neurological complications. PMID: 28515742 [PubMed - in process]

Effectiveness of Screening Postmenopausal Women for Cardiovascular Diseases: A Population Based, Prospective Parallel Cohort Study.
CONCLUSIONS: This multifaceted screening offer to a general population sample of postmenopausal women had no effects on all cause mortality or hospital admission for MI, IHD, PAD, and stroke within a short-term follow up period. PMID: 29625727 [PubMed - as supplied by publisher]

Urgent Endovascular Treatment for Non-traumatic Descending Thoracic Aortic Rupture
ConclusionEndoluminal repair of non-traumatic rupture of the descending thoracic aorta can be performed percutaneously. It showed a 30-day mortality of 18.1%, and no spinal cord injury or renal complications were observed in surviving patients at follow-up.Level of EvidenceLevel 4, case series.

Homocysteine and homocysteine-related compounds: an overview of the roles in the pathology of the cardiovascular and nervous systems.
HOMOCYSTEINE AND HOMOCYSTEINE-RELATED COMPOUNDS: AN OVERVIEW OF THE ROLES IN THE PATHOLOGY OF THE CARDIOVASCULAR AND NERVOUS SYSTEMS. Can J Physiol Pharmacol. 2018 Aug 21;: Authors: Djuric D, Jakovljevic V, Zivkovic VI, Srejovic IM Abstract Homocysteine, sulfhydryl group containing amino acid, is intermediate product during metabolism of the amino acids methionine and cysteine. Hyperhomocysteinema (HHcy) is used as a predictive risk factor for cardiovascular disorders, the stroke progression, screening for inborn errors of Met metabolism, and as a supplementary test for vitamin B12 deficiency. Two org...

Delphi Study to Reach International Consensus Among Vascular Surgeons on Major Arterial Vascular Surgical Complications
ConclusionVascular surgeons reached international consensus on major and minor complications following AAA, CAD and PAD treatment. This consensus may be helpful in harmonizing the information patients receive and improving standardization of the informed consent procedure. Since major complications differed between diseases, consensus on disease-specific complications to be discussed with patients is necessary.
