Embolization of Endocardial Vegetation With Stroke Presentation
This case report details the unique cause of death of a 37-year-old Caucasian woman with a history significant for intravenous drug abuse. Before her death, she complained of extremity weakness and pain. Although her death was discovered to be the result of endocarditis, her symptoms were similar to that of a stroke. Autopsy revealed a large endocardial vegetation infecting both the tricuspid and mitral valves and a patent foramen ovale. The subsequent embolization of this vegetation caused blockages in the lungs, liver, and brain. An acute embolization of these vegetations to the bilateral middle cerebral arteries is the ...

Catastrophic Multiple Microbleeds Caused by Infective Endocarditis Following Intravenous Thrombolysis and Endovascular Thrombectomy
Ischemic stroke is one of the most common complications of infective endocarditis (IE). IE must be considered as one of the causes of acute ischemic stroke (AIS) with emergent large vessel occlusion (ELVO), but early diagnosis of IE is difficult. AIS with ELVO must be treated using endovascular thrombectomy (EVT), with or without intravenous thrombolysis (IVT). IVT for AIS due to IE is not well established and remains controversial because of the risk of intracranial hemorrhage. A 42-year-old man suffered from right hemiparesis and disorientation, and AIS with ELVO was diagnosed.

Outcome of surgery for acute infective endocarditis: does preoperative stroke have an impact on mortality?
ConclusionSurgical treatment for IE still remains a challenge with high morbidity and mortality. Patients with preoperative neurologic dysfunction due to septic embolism have good early postoperative results without increased mortality.

Case Report: Α Case of Endocarditis and Embolic Stroke in a Child, Suggestive of Acute Q Fever Infection.
Abstract Acute Q fever is usually asymptomatic or is associated with a mild self-limited course and a favorable outcome. The occurrence of endocarditis during acute infection by Coxiella burnetii is an emerging clinical entity observed in adults that has been attributed to an autoimmune complication of early infection. Herein, we report the first case of a previously healthy 2-year-old child with endocarditis complicated by septic embolic stroke, in which the identified microbiological evidence was suggestive of acute rather than chronic C. burnetii infection. The development of endocarditis in this case occurred ...

Abiotrophia defectiva, an infrequent cause of cardioembolic stroke.
We present the case of a 51-year-old patient who suffered an ischemic stroke as a complication of A. defectiva endocarditis, and the cases of CNS infections caused by this microorganism are reviewed. The relevance of the case lies in its singular character, due to the absence of previous valve disease in the patient without a recent history of invasive procedures or surgical interventions. White thrombus as a possible feature of atypical stroke etiology: < em > Coxiella burnetii < /em > as the primary cause of acute ischemic stroke
We describe a singular case report of a 62- year-old male with AIS related to the occlusion of the left middle cerebral artery. Thrombus-aspiration allowed retrieving a 6 millimeters white thrombus. The real-time polymerase chain reaction performed on the thrombus detected Coxiella Burnetii allowed the diagnosis of infective endocarditis (IE) and the identification of the specific pathogen. Coxiella Burnetii is an endemic, small, intracellular, gram-negative coccobacillus and it is a rare cause of IE. The management of AIS caused by IE remains controversial, although in the cases of major occlusion mechanical thrombectomy ...

267 When investigating stroke is a waste of time
Young stroke should alert clinicians to investigate for unusual causes. A previously well 43-year-old Caucasian female presented with 2 days of punctuated left sided weakness and sensory inattention. MRI demonstrated multiple territory infarcts, predominantly on the right side. With no vascular risk factors, high dose steroids were commenced assuming primary angiitis of the CNS (PACNS). Blood tests including ESR, HIV, an autoimmune and thrombotic screen and blood cultures were negative. A 24-hour ECG and TOE failed to identify an embolic source, though interval MRIs plus CT angi- ography revealed evolving cerebral and sple...

Paradoxical Brain Embolism Associated with Kimura Disease Mimics Watershed Infarction
Kimura disease (KD) is an uncommon chronic inflammatory disease presenting as subcutaneous lymphadenopathy with eosinophilia. To date, only a single case of brain embolism caused by fibroblastic endocarditis associated with KD has been reported. Watershed infarction was seen in patients with episodes of severe hypotension or cardiac surgery. We here report a young case of KD who developed ischemic stroke and showed multiple small infarcts in the border zones between the territories of major cerebral arteries, mimicking watershed infarction.

Stroke from Meningitis Induced Vasculopathy: Vasospasm vs. Vasculitis (P4.378)
Conclusions:This case illustrates that vasculopathy from bacterial meningitis is likely often from vasospasm rather than true vasculitis.Disclosure: Dr. Moussawi has nothing to disclose. Dr. Vaitkevicius has nothing to disclose. Dr. Feske has nothing to disclose.

Meta-Analysis of Early or Late Surgery for Infective Endocarditis and Associated Stroke
Background: Surgery is recommended for infective endocarditis patients who develop ischaemic stroke, however the optimal timing of surgery remains controversial. In this meta-analysis, we compared the outcomes of early and delayed cardiac surgery for infective endocarditis in this setting.
