Recurrent embolic strokes due to antiphospholipid syndrome and non-bacterial thrombotic endocarditis in a patient with basal cell carcinoma
CONCLUSION: Outside of SLE and antiphospholipid syndrome, NBTE is a rare and underdiagnosed disease associated with thromboembolic events. Adequate anticoagulation is a cornerstone of its treatment. Anticoagulation management during perioperative care and valve surgery deserves specific attention and helps to protect the patient from embolic complications. In the case of stroke and thromboembolic events of unclear cause or suspected NBTE, echocardiography and thrombophilia assessments including an immunological workup are recommended.PMID:37170311 | DOI:10.1186/s13019-023-02266-6

Middle cerebral artery infarction in a cancer patient: a fatal case of Trousseau's syndrome.
This report describes a young, previously healthy man diagnosed to have an acute middle cerebral arterial ischaemic stroke and lower-limb deep vein thrombosis, who subsequently succumbed to pulmonary arterial embolism. During the course of his illness, he was diagnosed to have a malignant pleural effusion secondary to an occult adenocarcinoma. The impact of preoperative neurological events in patients suffering from native infective valve endocarditis
CONCLUSIONS NVE complicated by neurological events remains a challenging disease with high mortality and morbidity. Cardiac surgery seemed to be safe in the observed time interval, particularly for patients suffering from ischaemic stroke. A multidisciplinary approach is advocated for very individualized risk estimation.

The findings of the oesophageal echocardiography in patients with acute cerebral ischaemia
Conclusion Transoesophageal echocardiography is an independent method in examination of potential sources of cardioembolism. In our sample, a pathology in more than half of the patients was diagnosed that could refer to a suffered ischaemic stroke and the diagnosis led to a change in a treatment strategy in 17 cases of the total number of 95 patients (i.e. at 18%).

Surgical management of infective endocarditis: an analysis of early and late outcomes ADULT CARDIAC
CONCLUSIONS Surgical management of endocarditis continues to be challenging and is associated with significant morbidity and mortality. This report of 191 patients who underwent valve surgery for IE shows that in-hospital mortality is influenced by preoperative renal function and stroke at the time of presentation. The optimal timing for surgery in patients with stroke remains controversial. Long-term survival was negatively influenced by increasing age, moderate to severely impaired LVEF, prosthetic valve IE and S. aureus infection.

Timing of surgery in infective endocarditis with cerebral complications: Time to think outside the nonexistent box
In the April 2017 issue of the Journal, Murai and coworkers1 report on the effect of the interaction between stroke severity and timing of surgery on clinical outcomes in patients with active infective endocarditis (IE) and cerebral complications with moderate neurologic deficits. Their retrospective study cohort consisted of 170 consecutive patients from a single institution during a 24-year period starting in 1990. The National Institutes of Health Stroke Scale was used to evaluate the severity of stroke.

Intracranial haemorrhage in infective endocarditis.
CONCLUSIONS: ICH is a common complication of left-sided IE. The impact on prognosis is dependent on mechanism (haemorrhage of undetermined aetiology). We observed a higher mortality rate in patients who had conservative treatment when cardiac surgery was indicated compared with in those who underwent cardiac surgery. Intracranial complications of acute bacterial endocarditis.
Conclusion: Here, we reviewed the literature for intracranial pathology accompanying ABE versus SBE. ABE typically results in acute ischemia, septic emboli, stroke/hemorrhagic infarcts, or ICH. SBE more classically produces septic microemboli and mycotic aneurysms that may leak, but rarely producing ICH. We also presented a patient with ABE attributed to S. aureus whose septic emboli/stroke was accompanied by a mycotic aneurysm; the ruptured resulting in a large right occipital ICH.
