MR Three-Territory Sign in Cancer-Associated Hypercoagulation Stroke (Trousseau Syndrome): An Overlooked Diagnostic Feature: MR Three-Territory Sign
Conclusion: TTS is a frequently missed diagnostic magnetic resonance feature in cancer-associated hypercoagulation stroke and often heralds occult malignancy.

Cancer-related Stroke due to Mural Thrombus in the Extracranial Carotid Artery.
Authors: Ando D, Kobayashi J, Kuroda H, Aoki M Abstract A 41-year-old man was admitted to our hospital because of a cancer-related stroke (CRS) caused by a thrombus of the extracranial carotid artery. He had undergone neoadjuvant radiochemotherapy for metastatic colorectal adenocarcinoma. The serum D-dimer values were within the normal range. We treated him with intravenous unfractionated heparin followed by warfarin. There were no recurrent stroke events over six months. The leading cause of a CRS is an embolism caused by hypercoagulopathy, mainly represented by non-bacterial thrombotic endocarditis. However, it w...

267 When investigating stroke is a waste of time
Young stroke should alert clinicians to investigate for unusual causes. A previously well 43-year-old Caucasian female presented with 2 days of punctuated left sided weakness and sensory inattention. MRI demonstrated multiple territory infarcts, predominantly on the right side. With no vascular risk factors, high dose steroids were commenced assuming primary angiitis of the CNS (PACNS). Blood tests including ESR, HIV, an autoimmune and thrombotic screen and blood cultures were negative. A 24-hour ECG and TOE failed to identify an embolic source, though interval MRIs plus CT angi- ography revealed evolving cerebral and sple...

A Case Series Of Non-Bacterial Thrombotic Endocarditis Associated With Gynecological Malignancies (P6.248)
Conclusions:NBTE is an important ischemic stroke mechanism. We have reported three cases where the underlying malignancy was gynecological (endometrial, ovarian, and cervical). In the first two cases, the malignancy was discovered during the investigations for the stroke mechanism, while the third had a known underlying malignancy. This series highlights the need to consider gynecological malignancies as an underlying cause of stroke in young women; and that the ischemic event can occur prior to the malignancy diagnosis.Disclosure: Dr. Schabas has nothing to disclose. Cerebral ischemic events in patients with pancreatic cancer: A retrospective cohort study of 17 patients and a literature review
Abstract: Stroke is a dramatic complication of pancreatic cancer with mechanisms related to oncological disease. A better description of the characteristics of cerebrovascular events would help better understand the pathogeny and protect vulnerable patients. We thus conducted a descriptive analysis of clinical, biological, and radiological features of patients from our centers and literature. We reviewed consecutive cases of patients who presented cerebrovascular events and pancreatic cancer in 4 stroke units in Lorrain (France) between January 1, 2009 and March 31, 2015, and all reported cases of literature. We identifie...

Middle cerebral artery infarction in a cancer patient: a fatal case of Trousseau's syndrome.
This report describes a young, previously healthy man diagnosed to have an acute middle cerebral arterial ischaemic stroke and lower-limb deep vein thrombosis, who subsequently succumbed to pulmonary arterial embolism. During the course of his illness, he was diagnosed to have a malignant pleural effusion secondary to an occult adenocarcinoma. This report highlights the need for a high degree of suspicion for occult malignancy and non-bacterial thrombotic endocarditis in young (<60 years old) ischaemic stroke patients with no identifiable conventional cardiovascular risks. In selected patients, transoesophageal echocard...

Nonbacterial thrombotic endocarditis presenting as intracerebral hemorrhage.
We report a case of nonbacterial thrombotic endocarditis complicated by intracerebral hemorrhage as the first manifestation of adenocarcinoma of the lung. The endocarditis led to severe aortic regurgitation. In view of the advanced stage of lung cancer, the patient refused further therapy. He passed away 3 weeks after first diagnosis of the adenocarcinoma. PMID: 27325214 [PubMed - as supplied by publisher]

Nonbacterial Thrombotic Endocarditis Due to Primary Gallbladder Malignancy with Recurrent Stroke Despite Anticoagulation: Case Report and Literature Review
AbstractNonbacterial thrombotic endocarditis (NBTE) associated with malignancy is rare; its infrequency and similarity to other diagnoses make it a significant diagnostic challenge. A 63-year-old woman on rivaroxaban for prior deep vein thrombosis presented with left upper extremity weakness and left facial droop with imaging demonstrating multiple strokes. Echocardiograms revealed mitral and aortic valve vegetations. The patient was switched to apixaban and started on vancomycin and ceftriaxone for presumed culture-negative endocarditis. Despite continuing apixaban, her hospital course was complicated by new acute embolic...

Nonbacterial Thrombotic Endocarditis Related to Adenocarcinoma of the Uterine Cervix
We report a 66-year-old female patient who presented with acute onset of visual loss with relative afferent pupillary defect, hemineglect, hemihypesthesia, and apraxia. Magnetic resonance imaging of the brain demonstrated different stages of ischemic stroke in different vascular territories, suggesting cardiogenic embolism. Past history was significant for advanced-stage adenocarcinoma of the uterine cervix under chemoradiation treatment. On echocardiogram, vegetation at the aortic valve was observed. With the absence of evidence of infectious endocarditis, diagnosis of nonbacterial thrombotic endocarditis was made, and th...

E-164 Recurrent strokes as the initial presentation of colorectal adenocarcinoma
ConclusionCryptogenic strokes in patients with cancer are cardioembolic manifestations of cancer-mediated hypercoagulability such as NBTE, which consists of sterile, platelet-fibrin vegetations on cardiac valves. The mechanism of thromboembolism in patients with cancer is based on the interaction between tumor cells and leukocytes, which creates a thrombogenic vascular lining and induces the release of substances that activate the coagulation cascade. The diagnosis of NBTE should depend on the distribution of occluded vessels, identification of thrombus, and exclusion of other possible causes such as infective endocarditis...
