Medical Options for the Adjuvant Treatment and Management of Pediatric Melanoma
AbstractAlthough melanoma is a rare diagnosis in the pediatric population, advances in the management of adults with melanoma offer the prospect of promising therapeutic options for children. At this time, medical management is not considered curative but may reduce the risk of recurrence or prolong survival. Surgical management remains the mainstay of treatment. Medical therapy of pediatric melanoma is not thought to have a role for in situ, early-stage, or localized disease, but adjuvant therapy may have a role in improving the prognosis of patients with positive sentinel lymph node biopsy (SLNB), spread beyond the regio...

Correction to: Medical Options for the Adjuvant Treatment and Management of Pediatric Melanoma
Study NCT01519323 (A study of vemurafenib in pediatric patients with stage IIIC or stage IV melanoma harbouring BRAFV600 mutations) was included in a table listing ongoing clinical trials of adjuvant therapies for pediatric melanoma (Table  1) in error. The study was in fact closed early due to low enrollment as correctly noted in section 4 of the article.

Clinical counseling on sun protection and indoor tanning avoidance: A survey of current practices among U.S. health care providers
Publication date: Available online 17 July 2019Source: Preventive MedicineAuthor(s): Dawn M. Holman, Jin Qin, Elizabeth A. Gottschlich, Sophie J. BalkAbstractClinicians can play a role in skin cancer prevention by counseling their patients on use of sun protection and indoor tanning avoidance. We used data from the 2016 DocStyles, a web-based survey of U.S. primary care providers, to examine skin cancer prevention counseling practices among 1506 providers. In 2018, we conducted logistic regression analyses to examine factors associated with regularly providing counseling. Almost half (48.5%) of all providers reported regul...

“Personalized Excision” of Malignant Melanoma—Need for a Paradigm Shift in the Beginning Era of Personalized Medicine
Abstract: The premises on which guidelines for the excision of primary cutaneous melanoma are based are illogical and fail to take into account peculiarities of the individual lesion. The horizontal margins of excision continue to be adjusted to the vertical thickness of the neoplasm, and recommended clinical margins do not reflect the histopathologic borders of melanoma. Micrographically controlled surgery has become accepted for acral melanomas and melanomas of the face and neck but not for melanomas on the trunk, arms, and legs, although the latter tend to be more sharply confined. Extending margins of excision for th...

Skin care shouldn ’t be colorblind
In medicine, we are trained to be colorblind and treat all patients equally, to the best of our ability. The irony is that in dermatology we have to take skin color into account, because the color of the skin affects the presentation of skin conditions. This is important to consider in order to make an accurate diagnosis and weigh the best treatment options. Appearance of skin conditions may vary based on skin color Skin conditions may look different on darker skin than on lighter skin. For example, many rashes, including allergic reactions to medication, appear pink or red on lighter skin. On a person with darker skin, an...

Palliative care in the context of immune and targeted therapies: A qualitative study of bereaved carers' experiences in metastatic melanoma.
CONCLUSION: Immune and targeted therapies have resulted in increased prognostic challenges. There is a need to engage, educate and support patients and carers to prepare and plan amid these challenges. Educational initiatives must focus on improving communication between patients, carers and clinicians; the differences between palliative and end-of-life care; and increased competency of clinicians in having goals-of-care discussions. Clinicians must recognise and communicate the benefit of collaborative palliative care to meet patient and family needs holistically and comprehensively. Underserved Pacific Islanders With Locally Advanced Cervical Cancer Receive Higher Rates of Standard-of-Care Radiation Treatment Through the Pacific Island Health Care Project and Military Health System Compared to the Average U.S. Population
CONCLUSIONS: Women with cervical cancer from USAPI in the PIHCP program treated at TAMC received significantly higher rates of SOC radiation treatment than the U.S. population on average. This highlights the ability of PIHCP, through the MHS, to deliver SOC treatment for cervical cancer to an otherwise underserved patient population.PMID:34453178 | DOI:10.1093/milmed/usab325

A survey on management of pediatric vulvoperineal and perianal lichen sclerosus in individuals assigned female sex at birth.
Background: Lichen sclerosus (LS) is a chronic condition which can result in anogenital scarring and vulvar squamous cell carcinoma. In pediatric patients, the risk of developing long-term complications is unknown. Although LS often symptomatically improves at puberty, active disease can persist through adulthood. We performed a survey to identify variations in methods of diagnosis, treatment and management of pediatric vulvoperineal and perianal lichen sclerosus (PVPLS) in individuals assigned female sex at birth.

Factors for not performing total body skin examinations in primary care in association with teledermoscopy
CONCLUSION: Since TDS reduces the opportunities to have a TBSE by dermatologists, the standard management of patients with suspicious skin lesions in primary care must be revised and evidence-based. TBSE is strongly recommended for patients with increased risk of skin cancer, for example old persons with fair skin and a history of skin cancer, when managing them with TDS.PMID:36944927 | PMC:PMC10031956 | DOI:10.1186/s12875-023-02034-4
