Short-term outcomes of a multicentre randomized clinical trial comparing laparoscopic pylorus-preserving gastrectomy with laparoscopic distal gastrectomy for gastric cancer (the KLASS-04 trial)
CONCLUSION: Postoperative complications and mortality was comparable in patients undergoing LPPG and LDG. Registration number: NCT02595086 (http://www.clinicaltrials.gov).PMID:34487147 | DOI:10.1093/bjs/znab295 (Source: The British Journal of Surgery)

Further Expansion of the Mutational Spectrum of 3MC Syndrome: A Novel < b > < i > MASP1 < /i > < /b > Pathogenic Variant in a Male Patient
The 3MC syndrome is a rare autosomal recessive syndrome characterized by facial dysmorphism, multiple congenital abnormalities, and postnatal growth deficiency. Hypertelorism, blepharophimosis, blepharoptosis, high-arched eyebrows, and cleft lip/palate compose the facial gestalt, which is the key component for diagnosing the syndrome. Biallelic pathogenic variants inMASP1, COLEC11, andCOLEC10 are responsible for 3MC syndrome in which both genotypic and phenotypic heterogeneity is described. To date, 16 homozygous/compound heterozygous pathogenic variations in 27 patients from 22 families have been reported in theMASP1 gene...

Effects of Preoperative Pyloric Stenosis on Outcomes and Nutritional Status in 73 Patients Following Curative Gastrectomy for Gastric Cancer: A Retrospective Study from a Single Center
CONCLUSIONS Distal GC patients with PS have poor clinicopathological and nutritional status and poor prognosis. However, PS does not increase surgery-related morbidity and mortality.PMID:34315845 | DOI:10.12659/MSM.930974 (Source: Medical Science Monitor)

A new era of gastroenterostomy bypass using a lumen-apposing metal stent: A paradigm shift from laparoscopy to EUS?
Gastric outlet obstruction (GOO) is common in advanced malignant diseases, causing vomiting and loss of appetite resulting from pyloric stenosis and duodenal stenosis, and significantly reduces the patients ’ quality of life. Gastric cancer is the most common malignant disease, followed by pancreatic cancer.1 GOO may be seen as an adverse event of peptic ulcers, including postendoscopic submucosal dissection and acute pancreatitis, even in benign diseases. In advanced GOO, several interventions are r equired for the passage of water and food to the anal side and decompression of the dilatated stenotic gastric/duodenum. ...

Brief Overview and Updates on Infantile Hypertrophic Pyloric Stenosis: Focus on Perioperative Management
This article provides a brief overview and updates on hypertrophic pyloric stenosis in infants, with a focus on the surgical approach as well as perioperative management of this pathology. The most controversial aspect of the management of infants with pyloric stenosis is post-pyloromyotomy feeding, as there is no clear consensus in the literature on the best regimen. More randomized controlled trials are needed to establish the optimal resuscitation protocol in the preoperative phase and the ideal feeding regimen in the postoperative phase for infants with hypertrophic pyloric stenosis. A History of the Surgical Correction of Pyloric Stenosis
Infantile hypertrophic pyloric stenosis (IHPS) is a complex disorder with an incidence from 4 to 5 per 1000 live births with a strongly male predilection. The natural history of this initially fatal condition with nearly 100% mortality has evolved to a disease with a good prognosis. Pyloromyotomy by Ramstedt was first described 110 years ago and still remains the standard of surgical treatment for patients with IHPS. The laparoscopic approach is becoming more common and is the preferred method of treatment now in many centres. (Source: Journal of Pediatric Surgery)

Analysis of Pediatric Surgery Using the National Healthcare Insurance Service Database in Korea: How Many Pediatric Surgeons Do We Need in Korea?
CONCLUSION: The proportion of the pediatric surgery performed by PS rather than GS is increasing in Korea, but it is still widely performed by GS. PSs have better operative outcomes for acute abdominal surgery in the NICU than GSs. We believe that at least the index operation or the NICU operation should be performed by PS for better outcome, and that a minimum of 63 PSs are needed in Korea to do so. In addition, approximately 200 PSs will be required in Korea in order to manage main pediatric diseases and to achieve more advanced pediatric surgery in the future.PMID:33975393 | DOI:10.3346/jkms.2021.36.e116 (Source: J Korean Med Sci)
