Non-surgical therapies for Peyronie's disease
CONCLUSIONS: There is little evidence supporting the effectiveness of most non-surgical treatments for Peyronie's disease. Existing trials are mostly of poor methodological quality and/or fail to address patient-centered outcomes. Injectional collagenase appears to have some effectiveness; however, many individuals may not experience the improvement as clinically relevant, and this comes with the risk of increased adverse events. I have read with interest the manuscript written by Cooper et al. [1] entitled “What is the angle of a banana? The difficulty in reliable assessment of hypospadias chordee”. In which they reported “hypospadias chordee is often not a discrete angulation, but rather it is often an arc-like curvature similar to that of a banana. In this manner, penile curvature associated w ith hypospadias may differ from that seen in Peyronie's disease, where a fibrotic plaque may cause a fixed angulation. Study protocol for a prospective, multi-centered randomized controlled trial comparing pelvic fascia-sparing radical prostatectomy with conventional robotic-assisted prostatectomy: The PARTIAL trial
CONCLUSION: If PFS-RARP is non-inferior to RARP in terms of cancer control and has better functional outcomes, it should be the surgical standard of care for men with localized prostate cancer. Using the innovative two-stage consent process, completion of the trial will not only provide much needed evidence on one of the most common cancer surgeries but also insight on improving surgical RCT methodology.
