Highlights of the New Neonatal Resuscitation Program Guidelines
In 2015, the neonatal guidelines for resuscitation were published with several new treatment guidelines. Many of these are highlighted in this review. They included changes in the algorithm, timing of cord clamping in the preterm infant, optimizing detection of heart rate after birth, maintaining the premature infant temperature in the delivery room, initiating oxygen use during resuscitation, and using sustained inflation to establish functional residual capacity. In the term infant, changes included management of the nonbreathing infant delivered in the presence of meconium-stained amniotic fluid and consideration for wh...

International Perspectives: Reducing Birth Asphyxia in China by Implementing the Neonatal Resuscitation Program and Helping Babies Breathe Initiative
Although China has advanced as a leading economic world power since World War II, its perinatal health outcomes, including neonatal mortality, have lagged behind those of other rapidly developing countries. Some factors behind the relatively high neonatal mortality rate include geographic challenges to providing health care in rural regions and disparities in care across different levels of hospitals. To tackle the problem of neonatal mortality, specifically to reduce birth asphyxia, key stakeholders have worked to implement the Neonatal Resuscitation Program and Helping Babies Breathe initiatives across China in the past ...

Transient Hypothyroxinemia of Prematurity
Thyroid hormones play an important role in central nervous system development, starting from the exclusive maternal contribution in the first trimester and transitioning to a fully independent servo-control system at term birth. Extremely preterm delivery before 28 weeks’ gestational age disrupts this normal transition and reveals the challenge of whether replacement hormone will benefit cognitive and motor development. Although the evidence regarding deficiency is compelling, the hormone replacement question remains unanswered for preterm neonates. (Source: NeoReviews recent issues)

Early Nutrition in Preterm Infants: Effects on Neurodevelopment and Cardiometabolic Health
Nutrition provided during a NICU hospitalization and after hospital discharge is a major determinant of infant growth. Nutritional practices for preterm infants have evolved in recent decades, with an emphasis on increasing growth, primarily through greater macronutrient provision. Specific interventions, such as providing specialized preterm formula or fortified breast milk, are effective in increasing weight gain and other measures of growth, both in the NICU and after discharge. Further, substantial evidence supports the long-term benefits of several nutritional interventions on neurodevelopmental outcomes. More recentl...

Probiotics for the Prevention of Necrotizing Enterocolitis in Preterm Neonates
Necrotizing enterocolitis is a life-threatening condition in preterm neonates that is associated with severe morbidity and mortality. Several studies have suggested that probiotics can potentially decrease the risk of developing necrotizing enterocolitis via several proposed mechanisms of action, including increasing diversity of the intestinal flora. However, due to a lack of standardized study designs, including variability in product selection, dose, time of initiation, and duration, as well as a concern for safety in this vulnerable population, the use of probiotics in this population remains controversial. Regulations...

Importance of Amniotic Fluid in Gastrointestinal Development
Amniotic fluid (AF) is a bioactive medium containing various trophic factors and other nutrients that are necessary for fetal growth and organogenesis. Many trophic factors present in AF are responsible for the development of the fetal gastrointestinal tract. Development and maturation of the gastrointestinal tract is a complex cascade that begins before birth and continues during infancy and childhood by breastfeeding. Many factors, such as genetic preprogramming, local and systemic endocrine secretions, and many trophic factors from swallowed AF, modulate the development and growth of the gastrointestinal tract. Studies ...

Pharmacology Review: Maternal Selective Serotonin Reuptake Inhibitor Use and Neurologic Effects on the Neonate
The management of depression during pregnancy is complicated by the risks to the mother and the fetus by both the disease state itself as well as the medications used to treat it. Serotonin reuptake inhibitors, which include the selective serotonin reuptake inhibitors (SSRIs), are commonly used medications for the treatment of depression and other mood disorders. SSRIs readily cross the placenta and can affect fetal 5-hydroxytryptamine levels. Research into the effects of SSRIs on neonates has shown conflicting results, likely confounded by the fact that maternal depression itself may also lead to adverse outcomes for the ...
