Perinatal Transient Myeloproliferative Disorder in Trisomy 21
Transient myeloproliferative disorder (TMD), also known as transient acute myelopoiesis, is a myeloproliferative condition that occurs in the perinatal period in up to 10% of patients who have Down syndrome. Because of the perinatal presentation of this disorder, neonatal clinicians should be familiar with its clinical presentation, management, and outcomes. Affected patients develop severe myeloproliferation of megakaryocytic precursor cells, leading initially to hepatosplenomegaly and liver dysfunction. If the disorder develops prenatally, affected fetuses may also develop hydrops. The disorder requires the presence of 2...

Historical Perspective: A "Quick" Narrative of a Blood Coagulation Trailblazer
Physicians know Dr Armand Quick as the "Father of Blood Coagulation." His discovery of prothrombin time enabled clinicians to understand the use of vitamin K to prevent "hemorrhagic disease of the newborn" and to identify hereditary and acquired bleeding disorders in pediatric and adult patients. Quick suggested 50 years ago that aspirin could prevent myocardial infarction and stroke, a concept that perseveres today. His lifetime of research on bleeding and anticoagulants created this discipline in modern hematology. (Source: NeoReviews recent issues)

Evaluation of the Cyanotic Newborn: Part 2--A Cardiologists Perspective
Congenital heart disease is a major cause of cyanosis in the newborn, which should be considered and recognized promptly. Careful history and meticulous physical examination along with appropriate investigations are essential for the diagnosis of cyanotic congenital heart diseases. Although cardiac echocardiography is the diagnostic choice for congenital cardiac lesions, many centers may not have it readily available. In such cases, use of pulse oximetry, the hyperoxia test, chest radiography, and electrocardiograpy will usually differentiate heart disease from other causes of cyanosis. If a ductal-dependent lesion is susp...

Evaluation of The Cyanotic Newborn: Part I--A Neonatologists Perspective
Cyanosis is a commonly observed symptom in the newborn; however, its recognition and prompt management in a timely manner might be challenging in some instances. The presence of fetal hemoglobin and the concentration of hemoglobin in the blood have implications in the assessment of severity of cyanosis. Methodical evaluation and testing are essential in the diagnosis and treatment of the underlying condition. Pulse oximetry screening for critical congenital heart disease is important for detecting unsuspected life-threatening cardiac conditions. This 2-part article discusses the evaluation of cyanosis in the newborn period...

Current Practice in Neonatal Interventional Cardiology
Interventional cardiology is a growing specialty, with new technologies developing. In addition to development of new interventions that can reduce the invasiveness of congenital heart procedures, there is growing interest in applying existing procedures to our smallest patients. From the first use of catheterization for neonatal intervention with the balloon atrial septostomy, there are many new and evolving therapies that can treat neonates born with congenital heart disease. Complex Fetal Care: Importance of Fetal Arrhythmias to the Neonatologist and Pediatrician
Sudden unexplained death during the perinatal period remains a major longstanding challenge. Recent advances in diagnostic techniques and genetic testing have provided evidence that a significant fraction of these deaths may result from lethal cardiac arrhythmias. In this article, we review current methods of diagnosing arrhythmia in the fetus and strategies for managing life-threatening arrhythmia throughout the perinatal period, including transitional care at the time of delivery.
