Telepsych, Why Has No One asked this yet?
In the forums surrounding telepsychiatry on here I have seen many of the older posts mention that "you must be licensed in the state the pt is located in, IF you are to work abroad and do telepsych sans medicare. Since generally most ppl in telepsych are working for a telepsych corp and not pp then lets say that i want to graduate and only do telepsych, which state should i get licensed in? Which telepsych company has the most power and what state is it located in? (Source: Student Doctor Network)

Medicare /medicaid pts and cash practice
Is it true that one must specifically opt out of Medicare / medicaid prior to charging a patient out of pocket for a cash private practice visit? Am I only allowed to charge the patient whatever their Medicare/medicaid fee schedule would allow? (Source: Student Doctor Network)

Surely everybody already knew this
Hospitals charge (and private insurers pay) more than Medicare rates. I thought everybody knew that. Many Hospitals Charge More Than Twice What Medicare Pays for the Same Care The gap between rates set for private insurers and employers vs. those by the federal government stirs the debate over a government-run health plan. www.nytimes.com I remember a little over a year ago the Rand Corporation published a study (by at least one of the same authors) announcing that Medicare rates...

RFC Private Practice
This is gonna be the Mack Daddy of TFP posts but since I'm a new associate graduating from a program that was very strict with how/when RFC was returned, so I figured I'd ask. In general, how frequent do you return your RFCs? I know Medicare's global period is 61 days for nail care but if I returned all my new nail care consults at 61 days then that's pretty much all I will be doing in 61 days. I try to spread it out between 3-6 months depending on physical exam and risk factors and stuff... RFC Private Practice (Source: Student Doctor Network)

Where can I find IM RVUs?
Wanted to do some calculations on locums rates... I have some examples from radiology, but wanted some respective numbers from IM Let's say you are going to get $2500 for the "day". You need to do enough work to justify that. Currently, Medicare pays about $36 for each professional RVU. This covers the labor cost. $2500/$36 = 69 RVU 69 RVU is a doable day of work. What does this translate to? -CXRs are worth 0.22 RVU for the professional component. So this is about 313 CXRs. If you are... Where can I find IM RVUs? (Source: Student Doctor Network)

Patient filed medical board complaint after 1st visit
Patient was aggressive/rude/ demanding multiple controlled substances during initial evaluation. When she was told that she was not going to be accepted as a patient, she filed a complaint immediately alleging patient abandonment. Patient had medicare. If someone was not accepted into the practice after initial visit, what is the usual course of action. Do we still need to give a 30 day letter even when they were told they would not be accepted into the private practice? (Source: Student Doctor Network)

wRVUs and compensation
I received some education about RVUs and am relatively knowledgeable with CPT codes and billing and such, but I'm wondering what types of RVUs are typically included into a standard RVU-based physician compensation package. For example, we bill for nurse visits and Medicare's Chronic Care Management here, but those are not included on my monthly reports and I feel they should be. I have a meeting scheduled with my employer to review these questions further, but I'm hoping someone can point... wRVUs and compensation (Source: Student Doctor Network)
