Acute coronary artery dissection after multiple bee stings
We report a case of a 33-year-old man who presented with anterior wall acute myocardial infarction immediately after honeybee stings without the presence of cutaneous manifestations other than the bee sting lesions. Emergent coronary arteriography revealed dissection of the proximal left anterior descending artery, which was treated with balloon dilation with an excellent outcome.PMID:34970046 | PMC:PMC8682857 | DOI:10.1080/08998280.2021.1958579 (Source: Baylor University Medical Center Proceedings)

Allergic Acute Coronary Syndrome-Kounis Syndrome
Emerg Med Clin North Am. 2022 Feb;40(1):69-78. doi: 10.1016/j.emc.2021.08.010. Epub 2021 Oct 29.ABSTRACTAcute coronary syndrome (ACS) in the setting of an allergic/immunologic reaction is known as Kounis syndrome. It is an underdiagnosed and underrecognized disease entity. One must keep a high index of suspicions when managing a patient presenting with cardiac as well as allergic symptoms. There are 3 main variants to the syndrome. Treating the allergic reaction may alleviate the pain; however, ACS guidelines should be followed if cardiac ischemia is present.PMID:34782092 | DOI:10.1016/j.emc.2021.08.010 (Source: The Medica...

Kounis Syndrome: Kill Two Birds With One Stone
J Invasive Cardiol. 2021 Nov;33(11):E920-E921.ABSTRACTA 59-year-old man with a 95% stenosis in his diagonal branch and no stenosis in the right coronary artery (RCA) or left circumflex artery was scheduled for PCI. After exhibiting symptoms consistent with type I Kounis syndrome (KS), nitroglycerin was administered. While giving him intravenous epinephrine to maintain his hemodynamics, the ST level recovered, and he became fully conscious. When he complained of chest pain, a new ST elevation was noticed in the lateral leads, with ST levels in the inferior leads turning into depression. Repeat angiography demonstrated neith...

Acute Myocardial Infarction Within 24  Hours After COVID-19 Vaccination: Is Kounis Syndrome the Culprit?
In an important report published in the American Journal of Cardiology,1 2 patients (1 female and 1 male) who received the same COVID-19 vaccine messenger RNA-1273 (Moderna) developed acute myocardial infarction 1  day after the first dose. The authors speculated that the Moderna vaccine can induce an antibody response with a lipid-nanoparticle-encapsulated messenger RNA, whereas the ChAdOx1 nCoV-19 vaccine (AstraZeneca) can trigger an immune response by the nCoV-19 spike protein. Angina due to coronary artery spasm (variant angina): diagnosis and intervention strategies
Expert Rev Cardiovasc Ther. 2021 Oct 11. doi: 10.1080/14779072.2021.1991314. Online ahead of print.ABSTRACTINTRODUCTION: Since Prinzmetal first described a "variant" form of angina pectoris, with predominantly resting episodes of pain and cyclic severity variations, it has gradually become apparent that this clinical presentation is caused by episodes of coronary artery spasm (CAS) involving focal or diffuse changes in large and/or small coronary arteries in the presence or absence of "fixed" coronary artery stenoses. However, most clinicians have only limited understanding of this group of disorders.AREAS COVERED: We exam...
