Disease-Related Worries in Persons With Crohn Disease: An Interview Study
This study explored disease-related worries in persons with Crohn disease to gain deeper insight into how this worry is experienced and handled. Eight women and 4 men with Crohn disease who reported their disease-related worries as high or very high in a questionnaire were interviewed. The interviews were evaluated by qualitative content analysis, generating 3 categories: worries about the disease itself; feelings related to the worries; and management of the worries. The unpredictable course of the disease, impaired function due to fatigue, and lack of bowel control were the most prominent causes of worry. The worries cre...

Nonalcoholic Fatty Liver Disease: A Silent Epidemic
Nonalcoholic fatty liver disease is a growing condition among adolescent and adult populations, present in around 20%–30% of people in the United Kingdom. Nonalcoholic fatty liver disease is known as a silent disease and over many years may go on to cause nonalcoholic steatohepatitis. In the future it may become a leading contributor to cirrhosis, liver transplantation, and mortality. In recent years, programs have been set up to raise awareness of this condition with the first International NASH (nonalcoholic steatohepatitis) Day taking place in 2018; nevertheless, nonalcoholic fatty liver disease and nonalcoholic steat...

The Evidence on Feeding Initiation After Percutaneous Endoscopic Gastrostomy Tube Placement
Feeding initiation after percutaneous endoscopic gastrostomy tube insertion might occur anywhere from 4 to 24 hours after insertion in the practice setting. A review and appraisal of the literature reveals that feeding initiation is both safe and prudent within 4 hours of placement. Early feeding initiation allows for medications and nutrition to be given by enteral means earlier, resulting in a cost benefit for facilities and allowing patients to be discharged home or transferred to long-term care sooner. Nursing theory supports early feeding after percutaneous endoscopic gastrostomy tube placement. Early feedings provide...

Acute Pancreatitis and Fluid-Filled Collections: Etiology and Endoscopic Management
Acute pancreatitis is an inflammatory process of the pancreas, which can range from a localized inflammatory process to a systemic response, resulting in sepsis and multisystem failure. Pancreatic fluid collections are a complication of pancreatitis. Treatment of these fluid collections is dependent on correct classification. The 2012 Atlanta Criteria divides fluid collections into four categories: acute peripancreatic fluid collections, pancreatic pseudocysts, acute necrotic collections, and walled-off necrosis. Endoscopic ultrasound-guided management of chronic fluid collections is currently the preferred treatment modal...

Comparison of the Preference of Nutritional Support for Patients With Severe Acute Pancreatitis
This study aimed to compare the preference of different methods of nutritional support for patients with severe acute pancreatitis (SAP). Patients with SAP were divided into the enteral nutrition group (EN group, 16 cases), total the parenteral nutrition group (TPN group, 14 cases), and the enteral plus total parenteral nutrition group (EN+TPN group, 15 cases). At 7 days after admisson, TPN and EN+TPN groups showed significantly increased Ranson scores compared with the EN group (p (Source: Gastroenterology Nursing)

Patient Understanding of “Flare” and “Remission” of Inflammatory Bowel Disease
This study demonstrates that an understanding of “flare” and “remission” by patients with inflammatory bowel disease is largely symptom-based. The role of inflammation, medication failure, and targets of inflammatory bowel disease treatment beyond symptom control are not currently well known to patients with inflammatory bowel disease. To create a shared understanding of symptoms and treatment goals between the patient and the nurse, patient education on emerging expectations of inflammatory bowel disease care should be prioritized. Utilizing Information Technology to Improve Influenza Vaccination in Pediatric Patients With Inflammatory Bowel Disease
The objective of this quality improvement project was to evaluate the implementation of information technology prompts within the electronic medical record to improve influenza vaccination during specialty clinic visits. The proportion of patients with yearly influenza vaccination was evaluated at baseline, Year 1, and Year 2 following implementation. At baseline, only 10% of a random sample had documented influenza vaccination. Vaccination documentation improved to 39% (96/246) by Year 1 and to 61% (175/287) by Year 2 (p (Source: Gastroenterology Nursing)

Coping in African Americans With Inflammatory Bowel Disease: An Integrative Review of the Literature
Given the chronic nature of inflammatory bowel disease, understanding the coping behaviors of individuals affected with the disease is important to influence health outcomes. Although minorities comprise a significant portion of individuals with the disease, little is known about the potential influence of one's culture, specifically among African Americans, on coping with inflammatory bowel disease. This integrative literature review examined the past decade of research related to the coping behaviors of African Americans living with inflammatory bowel disease to identify opportunities for further research. Five studies w...

A Literature Review of Percutaneous Endoscopic Gastrostomy: Dealing With Complications
Sufficient energy and essential nutrients are vital for normal growth and development in childhood. Ideally, nutrition should be provided orally. However, if the gastrointestinal system is functional, enteral tube feeding can be used when nutritional requirements cannot be provided orally, thus providing nutritional benefits and enabling positive disease management in pediatric patients. Postoperative care in children allows monitoring of the position and functionality of the tube; performing nutrition intolerance, growth, hydration, and nutritional assessments; and performing metabolic and complication follow-ups. Tube fe...
