The Many Faces (and Names) of Mood Dysregulation
Explosive outbursts in children and adolescents have been long identified by clinicians and have been described using many different conceptualizations and terms. The topography of explosive outbursts is complex, heterogeneous, and includes the interactions of different emotional and behavioral constructs. Included here are pre-existing central nervous system vulnerabilities including psychiatric and neurologic diagnoses, various contributing emotions that generally carry a negative valence, and aggressive behaviors that are usually overt and reactive. Emotional impulsivity and deficient emotional self-regulatory mechanism...

The Phenomenology of Outbursts
Outbursts (severe temper loss) in children are a common reason for treatment referral. However, the diagnostic system has not classified them in a way that expands knowledge. Outbursts are nested in the concept of irritability, which consists of a feeling and a behavioral dimension. Both need to be identified but kept separate. This review summarizes the phenomenology of outbursts normatively and clinically. Severe temper loss needs a consistent label, an operationalized way of classification and measurement, and an assessment approach independent of diagnosis until other data are gathered to more accurately determine what...

Measurement of Dysregulation in Children and Adolescents
This article highlights the breadth of measures available for measuring emotion dysregulation, or facets thereof, in children and adolescents, and reviews in detail a subset of these measures. We describe broadband measures and measures that are specific to emotion dysregulation, including observational tools, clinical interviews, and rating scales. Furthermore, we discuss the strengths, weaknesses, and psychometric properties of each approach and specific contexts or populations in which certain methods may be particularly useful. Finally, recommendations for thorough assessment of emotion dysregulation in future studies ...

Attention-Deficit Hyperactivity Disorder and the Dysregulation of Emotion Generation and Emotional Expression
Individuals with attention-deficit/hyperactivity disorder (ADHD) frequently experience strong reactions to emotionally evocative situations. Difficulties modulating anger and other upsets have clinically significant behavioral consequences. Those with ADHD may have anomalies in emotion generation, emotion expression, or both that predispose to these problems. The association between ADHD and emotion dysregulation raises Important clinical and research issues, including possible heterogeneity in the mechanisms by which they are related. Although first-line treatments for ADHD often help to resolve emotional dysregulation sy...

Ticked Off
Tourette disorder is a complex neuropsychiatric syndrome of childhood onset characterized by multiple motor and phonic tics and is associated with high rates of psychiatric comorbidity. Symptoms of impulsive aggression (explosive outbursts or “rage”) are commonly encountered in the clinical setting, cause significant morbidity, and pose diagnostic and treatment challenges. These symptoms may be multifactorial in etiology and result from a complex interplay of illness severity and psychosocial factors. Treatment strategies require car eful differential diagnostic evaluation and include both behavioral and pharmacologic ...

Emotional and Behavioral Dysregulation in Severe Mental Illness
Emotional and behavioral dysregulation are common in severe mental illnesses, including schizophrenia, bipolar disorder, and borderline personality disorder. Emotional instability and behavioral outbursts can be driven by internal processes and/or environmental triggers and interpersonal interactions. Understanding the underlying diagnosis is important in determining the best course of treatment. Disorder-specific treatments are important in addressing underlying drivers of emotional dysregulation, irritability, and aggression. Coping skills training and behavioral modification strategies have broad applicability and are u...
