Assessment of Gender Diverse Children
This article provides an overview of the World Professional Association of Transgender Health (WPATH) Standard of Care 8th edition (SOC8) chapter on transgender and gender diverse (TGD) prepubescent children (see Scott Leibowitz ’s article, “Assessment of Transgender and Gender Diverse Adolescents: Incorporating the Standard of Care 8th Edition,” in this issue). This is the first WPATH SOC chapter in history, specifically devoted to children, acknowledging that the developmental needs and experiences of these youth ca n be distinguished from those of TGD adolescents. Common Intersection of Autism and Gender Diversity in Youth
Autism and gender diversity often intersect. Many transgender youth seeking gender-related medical interventions are autistic. Clinicians serving these youth lack an autism-specific evidence base to guide gender care decisions. At present, care decisions are based on extrapolation of care models from transgender youth samples, generally. At this point, there is no evidence to suggest that autistic youth are likely to experience shifts in gender or gender-related medical requests, although this has been insufficiently studied. In this article, cowritten by expert clinicians and autistic gender-diverse collaborators, an over...

Assessment of Transgender and Gender-Diverse Adolescents
This article provides an overview of the World Professional Association of Transgender Health (WPATH) Standard of Care 8th edition (SOC8) recommendations for adolescents seeking gender-affirming care. SOC8 was the first iteration of the guidelines to employ a Delphi consensus process that required 75% agreement of 120 multidisciplinary international transgender health experts for approval of its recommendations. While the evidence base for adolescent care is evolving, gender-affirming treatment is medically necessary and promotes long-term psychological wellbeing. The guidelines emphasize the importance of an assessment to...

Gender Literacy Across Childhood and Adolescence
Gender--once an afterthought despite its significant yet unspoken role in the average American ’s daily life (public restrooms, clothes shopping, grooming, sports teams)--has become a fraught sociopolitical issue. The concept of gender as a construct, once relegated to the realm of Women’s and Gender Studies courses, went mainstream while concurrently, gender reveal parties have experienc ed a surge in popularity. Meanwhile, youth (and adults) have become increasingly comfortable exploring their gender identities and expression, which has led to an increase in inquiries regarding gender-affirming care--along with an ac...

Transgender and Gender Diverse Youth in Inpatient and Other Residential Care
Inpatient and other residential care environments require special considerations for safety and unique opportunities to provide affirming care to TGD youth. Gender-positive policies, staff training, communication, placement, programming, and discharge planning are imperative; however, data and literature are limited in regard to affirming the care of TGD youth in such environments. This chapter draws from published research and best practice to support the wellness of TGD individuals in inpatient and similar settings. It offers clinical guidance for an organization ’s clinicians, administrators, educators, and advocates ...

Supporting Transgender Youth Across Psychosocial Systems
Transgender children and adolescents are at an elevated risk for negative mental health outcomes due to exposure to stigma and discrimination regarding their identity. While various environments may perpetuate this stigma, many supports also exist that can bolster safety, affirmation, and resilience in this population. Opportunities for support exist within schools, broader communities, religious organizations, and with medical professionals who practice gender-affirming care. Clinicians who are familiar with resources in their communities can effectively guide transgender youth and their families to these affirming spaces...

Individual Affirming Care
Clinicians working with youth encounter transgender and gender-diverse youth and play a role in supporting the psychological and social well-being of youth and their families. Mental health clinicians start with a thorough assessment, development of a biopsychosocial formulation of the youth, and a collaborative treatment plan. Determining if the youth meets diagnostic criteria for Gender Dysphoria and identifying comorbid mental health diagnoses inform recommendations for psychiatric medication management, individual and/or family therapy interventions, and/or referrals to medical or surgical colleagues. Supporting develo...

Beyond Race, Sex, and Gender
This article explores how race, sex, and gender are better thought of as a continuum rather than binary categories. Starting with a discussion of intersectionality, we examine the importance of ethnic-racial identity and explore unique cultural considerations for working with Black, Latinx, and AAPI transgender and nonbinary youth. We then examine intersex youth and variations of sex development, as well as specific challenges they face. Finally, we explore nonbinary gender identities and the importance of individually tailoring affirming interventions. For all sections, we highlight the strengths and resilience of the you...
