Stroke Mortality in Kenya ’s Public Tertiary Hospitals: A Prospective Facility-Based Study
Conclusions: Mortality due to stroke is high, with poor survival observed in the first year after stroke. The risk of death increases with increasing age and duration of hospital stay. There is need for attention to quality of care and long-term needs of stroke patients to mitigate the high mortality rates observed. Public health initiatives aimed at early screening and diagnosis should be enhanced. Further research is recommended to establish the true burden of stroke at the community level to inform appropriate mitigation measures.Cerebrovasc Dis Extra 2018;8:70 –79 (Source: Cerebrovascular Diseases Extra)

Predictors of Cardioembolic Stroke in Japanese Patients with Atrial Fibrillation in the Fushimi AF Registry
Conclusions: In this population-based cohort of Japanese patients with AF, in addition to previous stroke/TIA and older age, sustained AF and low body weight emerged as risk factors for CES, as opposed to any stroke, which may have a different risk profile. Patients with CKD or previous stroke/TIA who developed cardioembolic infarction exhibited more advanced severity. There is a need for direct oral anticoagulants that can be used safely in patients with comorbid AF and CKD.Cerebrovasc Dis Extra 2018;8:50 –59 (Source: Cerebrovascular Diseases Extra)

Knowledge about Stroke in Belo Horizonte, Brazil: A Community-Based Study Using an Innovative Video Approach
Conclusion: Female sex and a family history of stroke increased the odds of recognizing the signs of stroke. Knowledge about the clinical manifestations, risk factors, and management of stroke was low in the studied population.Cerebrovasc Dis Extra 2018;8:60 –69 (Source: Cerebrovascular Diseases Extra)

Spotty Carotid Plaques Are Associated with Inflammation and the Occurrence of Cerebrovascular Symptoms
Conclusions: The present study supports the concept that spottiness is a feature of the carotid plaques rich in inflammation and can be associated with the typical phenotype of high-risk plaques.Cerebrovasc Dis Extra 2018;8:16 –25 (Source: Cerebrovascular Diseases Extra)

Intracranial Arterial Calcification in Black Africans with Acute Ischaemic Stroke
Conclusions: The burden of IAC is very high among native African patients with AIS and preferentially involves proximal inflow arteries. Therefore, the role of large vessel atherosclerosis in ischaemic stroke in native Africans should be explored in future multinational, multimodality studies.Cerebrovasc Dis Extra 2018;8:26 –38 (Source: Cerebrovascular Diseases Extra)

Erythrocyte-Rich Thrombus Is Associated with Reduced Number of Maneuvers and Procedure Time in Patients with Acute Ischemic Stroke Undergoing Mechanical Thrombectomy
Conclusions: In this study, erythrocyte-rich thrombus was associated with noncardioembolic etiology, higher thrombus density, and reduced procedure time.Cerebrovasc Dis Extra 2018;8:39 –49 (Source: Cerebrovascular Diseases Extra)

Oral Health and Brain Injury: Causal or Casual Relation?
Conclusions: Overall, oral health and stroke were related. Periodontitis and tooth loss were independently associated with stroke. However, prevention and timely intervention may reduce the risk of stroke. Stroke was the main cerebral lesion studied in the literature, with almost no publications on other brain lesions.Cerebrovasc Dis Extra 2018;8:1 –15 (Source: Cerebrovascular Diseases Extra)

Frequency, Aetiology, and Outcome of Small Cerebellar Infarction
Conclusions: Small cerebellar infarction accounted for one-third of the ischaemic strokes in this location, most often involved the posterior lobe, causing fewer clinical features, and had a better clinical outcome than large cerebellar infarction. Patients with small cerebellar infarction require appropriate vascular management including investigation for a cardioembolic source.Cerebrovasc Dis Extra 2017;7:173 –180 (Source: Cerebrovascular Diseases Extra)

Blunt Traumatic Extracranial Cerebrovascular Injury and Ischemic Stroke
Background: Ischemic stroke occurs in a significant subset of patients with blunt traumatic cerebrovascular injury (TCVI). The patients are victims of motor vehicle crashes, assaults or other high-energy collisions, and suffer ischemic stroke due to injury to the extracranial carotid or vertebral arteries.Summary: An increasing number of patients with TCVI are being identified, largely because of the expanding use of computed tomography angiography for screening patients with blunt trauma. Patients with TCVI are particularly challenging to manage because they often suffer polytrauma, that is, numerous additional injuries i...

Factor XII as a Risk Marker for Hemorrhagic Stroke: A Prospective Cohort Study
Conclusion: We found an independent association between FXII levels and the risk of hemorrhagic stroke, but not between FXII levels and ischemic stroke or myocardial infarction.Cerebrovasc Dis Extra 2017;7:84 –94 (Source: Cerebrovascular Diseases Extra)

Outcomes of Endovascular Thrombectomy with and without Thrombolysis for Acute Large Artery Ischaemic Stroke at a Tertiary Stroke Centre
Conclusion: Our study highlights the relatively large proportion of patients suitable for EVT who have a contraindication to alteplase and raises the hypothesis that adding alteplase to successful EVT may not be necessary to optimize functional outcome. The results are consistent with observational data from other endovascular centres and support a randomised controlled trial of EVT versus EVT with bridging alteplase.Cerebrovasc Dis Extra 2017;7:95 –102 (Source: Cerebrovascular Diseases Extra)

Dosage Calculation for Intravenous Thrombolysis of Ischemic Stroke: To Weigh or to Estimate
Conclusion: This first prospective study of weight assessment in acute stroke shows that the use of an easily accessible scale makes it feasible to weigh patients with acute stroke without the treatment delay associated with additional patient transfers. Physicians ’ estimates of patients’ weights demonstrated substantial aberrations from measured weights. Avoiding these deviations would improve the accuracy of tPA dosage.Cerebrovasc Dis Extra 2017;7:103 –110 (Source: Cerebrovascular Diseases Extra)

In Potential Stroke Patients on Warfarin, the International Normalized Ratio Predicts Ischemia
Conclusions: Sub-therapeutic INR and atrial fibrillation are strongly associated with ischemia in patients on warfarin presenting with acute neurologic symptoms. Ischemia is far less likely in patients with an INR of ≥2 and rare in those with an INR ≥3.6. This study shows that the INR value of a patient on warfarin can help stratify patients’ risk for acute ischemic stroke and guide further neurologic imaging and workup.Cerebrovasc Dis Extra 2017;7:111 –119 (Source: Cerebrovascular Diseases Extra)
