Radiation Myelopathy Caused by Palliative Radiotherapy and Intrathecal Methotrexate
Radiation myelopathy is a rare, late-stage adverse event that develops following irradiation at or above 50 Gy. Here, we report a case of irreversible paraplegia caused by palliative radiation (20 Gy in 5 fractions) to the spinal cord combined with intrathecal methotrexate (IT-MTX). A 69-year-old man presented with back pain, prompting a diagnosis of acute myeloid leukemia. At the first visit, he complained of muscle weakness and hypoesthesia in both legs; spinal magnetic resonance imaging (MRI) revealed an epidural mass compressing the spinal cord at the fifth to seventh level of the thoracic vertebrae. This was considere...

Metastatic Extrapulmonary Small Cell Carcinoma Presenting as Obstructive Jaundice
Extrapulmonary small cell carcinoma (EPSCC) is a rare cancer with a poor prognosis. It can arise from almost any site and is usually associated with extensive metastasis at the time of diagnosis. Due to the rarity of this cancer, very limited data is available in the literature and most of the recommendations for its evaluation and treatment are based on retrospective studies and expert opinion. This case report shares one such presentation of EPSCC. A 78-year-old male was admitted to the hospital with presenting symptoms of abdominal pain and discoloration of the eyes and urine for 2 months. Initial laboratory investigati...

Neoadjuvant FOLFIRINOX Followed by Pancreatoduodenectomy for Pancreatic Cancer in Patients with Previous Transhiatal Esophagectomy for Esophageal Cancer
During pancreaticoduodenectomy after transhiatal esophagectomy, the preservation of the blood supply to the gastric conduit is technically difficult due to adhesion. Here, we present a case of successful pancreaticoduodenectomy after neoadjuvant chemotherapy in a patient with pancreatic head cancer who previously underwent subtotal esophagectomy with gastric reconstruction for esophageal cancer. A 69-year-old man who had undergone cholecystectomy 20 years prior and transhiatal esophagectomy 6 years prior for esophageal cancer presented to our hospital for indigestion. Computed tomography and magnetic resonance imaging reve...

Late Onset of Cardiac Metastasis of a Melanoma following Nivolumab Immunotherapy
In this report, we present a case of cardiac metastasis of a malignant melanoma originating from the nasal cavity and presenting with cardiac tamponade detected during immunotherapy. The patient was a 66-year-old man diagnosed with malignant melanoma of the right nasal cavity 4 years ago. Two years ago, the size of the melanoma increased making it unresectable; therefore, he was treated thrice with a combination therapy of nivolumab and ipilimumab. Subsequently, the treatment was changed to single-agent nivolumab therapy, which was continuously administered for one and a half years. Imaging evaluation conducted every 3 mon...

Stage 4 Cytokine Release Syndrome Caused by the First Dose of Nivolumab and Ipilimumab Combination Therapy in a Patient with Metastatic Melanoma Successfully Treated with Methylprednisolone, Tocilizumab, and Etanercept
The authors report the first case of stage 4 cytokine release syndrome (CRS) (graded by the National Cancer Institute Common Terminology Criteria for Adverse Effects scale) involving a patient with advanced metastatic melanoma who was treated with the combination of two monoclonal antibodies, nivolumab (anti-programmed cell death receptor 1 inhibitor [PD-1]) and ipilimumab (a cytotoxic T lymphocyte-associated antigen 4 inhibitor [CTLA-4]) after her first dose of both. The patient was treated initially with methylprednisolone and tocilizumab but was refractory to treatment. A trial of etanercept was initiated due to her ele...

Adrenal Gland Metastasis of Breast Invasive Mucinous Carcinoma: A Rare Case Report and Review of Literature
This report described a rare case of right adrenal gland metastasis in a 48-year-old female, who was diagnosed with breast invasive mucinous carcinoma and underwent right modified radical mastectomy with axillary lymph node dissection 5 years previously. A mass located on the right adrenal gland was detected during a routine examination 2 months ago. The patient was asymptomatic and adrenal gland MRI revealed a mass in the right adrenal gland. Definitive preoperative diagnosis failed to be established. Right adrenal gland laparoscopic adrenalectomy was performed and the diagnosis of adrenal gland metastasis of breast carci...

Complete Spontaneous Regression of Squamous-Cell Lung Cancer: A Case Report
We report a case of complete spontaneous regression of squamous-cell lung cancer (SCLC) following a core needle biopsy in a 67-year-old female patient with two previous lung cancers and concomitant follicular lymphoma. The patient was diagnosed with SCLC after 4 core needle biopsies from a nodule in the left upper lobe and at the same time suspected of having follicular lymphoma. Treatment for the lung cancer was delayed by approximately 8 weeks because the diagnosis of lymphoma was both challenging and time-consuming. A computed tomography scan was performed in relation to the scheduled treatment for SCLC, showing that th...

Disseminated Intravascular Coagulopathy in Cranial Surgery: Case Report and Review of the Literature
We present a rare case with multifocal osseous metastases of prostate cancer and disseminated intravascular coagulopathy (DIC). The patient received systemic tumor therapy, including antiandrogen agents as well as cerebral radiation. Due to rapid clinical deterioration, indication for surgery of the space-occupying calvarial metastasis was made as a salvage therapy procedure. Depleted clotting factors were substituted, and patient underwent tumor embolization prior to surgery. Although his neurological status improved postoperatively, the patient died suffering from multiple supra- and infratentorial infarctions. Our case ...

Paraganglioma of the Fallopian Tube Presenting as Isolated Fallopian Tube Torsion
A 32-year-old woman with no previous disease history was presented with worsening right-lower abdominal pain, which lasted for 4 days. On magnetic resonance imaging, a solid mass measuring 48 mm in longitudinal diameter connected with a tortuous structure that appeared to be a fallopian tube was identified in the right-lower abdomen. Because the right ovary was identified at a slightly distant location, isolated fallopian tube torsion with heterogeneous mass was suspected. The isolated fallopian tube torsion without ovarian involvement was laparoscopically confirmed. After detorsion, solid necrotized mass in the distal por...

Unusual Presentation of Bilateral Chylothorax and Chylous Ascites with Pancreatic Adenocarcinoma: A Case Report
We present a case of a 60-year-old man who presented with dyspnea for 1 week and generalized abdominal distention. He was diagnosed with advanced-stage pancreatic adenocarcinoma and received palliative chemotherapy. On follow-up examination, he showed decreased breathing sounds in both lungs and marked ascites. Chest radiography showed bilateral pleural effusion, which was more prominent in the left hemithorax than the right hemithorax. Thoracocentesis and abdominal paracentesis were performed, and the finding of pleural and ascites fluid was compatible with bilateral chylothorax and chylous ascites. Although the patient w...

Combination of Mirogabalin and Duloxetine Attenuates Peripheral Neuropathy by Eribulin: A Novel Case Report
Chemotherapy-induced peripheral neuropathy (CIPN) is one of the most severe complications associated with chemotherapy for breast cancer. We encountered a case in which mirogabalin initially ameliorated, and additional duloxetine further attenuated eribulin-induced CIPN. Herein, we report its management. A 53-year-old woman received eribulin treatment as third-line chemotherapy for recurrent breast cancer. She experienced grade 2 CIPN with adjuvant docetaxel and cyclophosphamide treatment (worst numeric rating scale (NRS) 6/10 for numbness and 6/10 for pain) and had baseline grade 1 symptoms only in the hands (NRS 1/10 for...
