Can I realistically match into pulm/crit?
I am a US MD who graduated from a mid-tier university-affiliated community program in 2023. Currently doing a PGY-4 Chief residency in a different program. Step 1 240 Step 2 229 Step 3 225 ABIM - passed I applied for cardiology and did not match. Given my situation, I think cardiology is unrealistic. However, I can easily see myself happy in pulm/crit or critical care. I know pulm/crit also is very competitive. All my research/work (manuscript x1, abstract x1, poster x1) are all in... Lipid boards
Anyone take lipid boards? Between the test, self-assessment modules, and practice materials it’s north of $3200. Painful, but seems like it’s be a nice feather to add to your cap. Thoughts? (Source: Student Doctor Network)

Plan B to reapply to Gen Cards: external chief resident position or HF fellowship
Everyone told me they expected me to be a good candidate for at least 10 interviews or so. I only ended up with 3 (1 internal, 1 through a connection). and I am weighing my options for Plan B. I go to a university program in the South, USDO. Steps 243/237, Level 3 pass. No obvious red flags (at least not obvious enough where my faculty, PD, or mentors have warned me). As far as scholarship activity, our PD primarily supports case reports saying "It's a numbers game", but I have a few... Read more (Source: Student Doctor Network)

Breast CA antihormones, concurrent vs sequential
I have a patient with very poor health who cannot medically undergo surgery - heart failure, O2 dependent - but we are treating a bulky breast mass about 7 cm in size and grossly involved regional nodes. ER is 90 plus percent positive, so she will be getting anti estrogens of some kind; the question is whether to give the radiation and anti hormones together or sequentially. Is there any data either way? I believe there was a breast chemo study that showed better response rate if the... Read more (Source: Student Doctor Network)
