Shisa7 is a GABAA receptor auxiliary subunit controlling benzodiazepine actions
The function and pharmacology of -aminobutyric acid type A receptors (GABAARs) are of great physiological and clinical importance and have long been thought to be determined by the channel pore–forming subunits. We discovered that Shisa7, a single-passing transmembrane protein, localizes at GABAergic inhibitory synapses and interacts with GABAARs. Shisa7 controls receptor abundance at synapses and speeds up the channel deactivation kinetics. Shisa7 also potently enhances the action of diazepam, a classic benzodiazepine, on GABAARs. Genetic deletion of Shisa7 selectively impairs GABAergic transmission and diminishes t...

Dishing out more drugs won ’t stop the pain. Doctors need new tools | Ann Robinson
As a GP, I know opioids have their place. But we shouldn ’t be complacent about a US-style crisis happening in BritainGrowing concern about the prescription of drugs that cause dependence has been backed up by a recent report from Public Health England (PHE). The agencydiscovered that in 2017-18, 13% of adults in England received at least one prescription for opioids such as morphine or oxycodone (although this represented a slight downturn in prescribing after a long upward trend). Other drug classes were implicated too, with 25% of adults prescribed one or more of the following drugs: benzodiazepines (such as Valium), ...

What Makes Consumers Satisfied?
Dear Dr. Jeff: Our facility administrator has asked all the “providers” to participate in a chain-wide program to boost “consumer satisfaction.” Our medical team would be happy to participate in quality improvement projects. But most of us object to the paradigm where our patients become consumers of medical care and we become short order cooks in a medical fast food chain, selling drugs on demand while wearing smile buttons. Good care often requires saying “no” to antipsychotics used as sleeping pills, excessive opioids, siderails, antibiotics for foul-smelling urine, Valium prescriptions for 90-year-olds, and...

How Some Generic Drugs Could Do More Harm Than Good
For the 16 years that Dr. Brian Westerberg, a Canadian surgeon, worked volunteer missions at the Mulago National Referral Hospital in Kampala, Uganda, scarcity was the norm. The patients usually exceeded the 1,500 allotted beds. Running water was once cut off when the debt-ridden hospital was unable to pay its bills. On some of his early trips, Westerberg even brought over drugs from Canada in order to treat patients. But as low-cost generics made in India and China became widely available through Uganda’s government and international aid agencies in the early 2000s, it seemed at first like the supply issue had been ...
