Rapid determination of nine barbiturates in human whole blood by liquid chromatography‐tandem mass spectrometry
A rapid, simple and sensitive liquid chromatography‐tandem mass spectrometry (LC‐MS/MS) method was developed for the qualitative and quantitative analysis of nine barbiturates (barbital, phenobarbital, pentobarbital, amobarbital, secobarbital, thiopental, butalbital, butabarbital, and hexobarbital) in human whole blood. Barbiturates were extracted from 100 μL of human whole blood samples using a simple liquid‐liquid extraction (LLE) procedure, and detected by LC‐MS/MS. An UPLC C18 (2.1 mm × 100 mm, 1.7 µm) column was used at 40 °C for the separation and acetonitrile/water system was used as the mo...

Medication Overuse Headache Due to Butalbital, Acetaminophen, and Caffeine Tablets.
Authors: Feeney R Abstract Questions from patients about pain conditions and analgesic pharmacotherapy and responses from authors are presented to help educate patients and make them more effective self-advocates. Medication overuse headache is a relatively common cause for chronic daily headache in the migraine patient. In reply to a question, medication overuse headache is reviewed, acute and preventive medications are discussed, and quality of life is considered. Managing Migraine During Pregnancy and Lactation
Abstract While over half of women with migraine report improvement during pregnancy, having a history of migraine may increase the chance of negative health outcomes. The state of pregnancy increases the risk of several dangerous secondary headache disorders, especially those associated with hypertensive disorders of pregnancy, and providers need to know the red flags to diagnose and treat emergently. Non-pharmacological migraine treatments can be instituted in advance of pregnancy as many are considered the safest options during pregnancy, but understanding the safety of medications and dietary supplem...

Therapeutic Drug Monitoring of Pentobarbital: Experience at an Academic Medical Center
Conclusions: Pentobarbital TDM can be performed by immunoassay on an automated clinical chemistry platform, providing an alternative to chromatography-based methods. Confusion in barbiturate names is common, especially pentobarbital and phenobarbital. (Source: Therapeutic Drug Monitoring)

Application of data screening to drug exposure in large risk factor studies of birth defects
ConclusionWhile screening programs such as the one described can be a valuable tool for exploring potential associations in large data bases, they must be applied with caution. The issue of multiple testing and chance findings is a major concern. While statistics are a necessary component, human judgment must be an integral part of the process. Migraine Diagnosis and Treatment: A Knowledge and Needs Assessment among Primary Care Providers (S23.006)
Conclusions: PCPs may not be aware of the specific recommendations for managing migraine patients. Future work should focus on innovative ways to provide decision support and education to PCPs who are caring for patients with migraine.Disclosure: Dr. Minen has nothing to disclose. The Acute Treatment of Migraine in Adults: The American Headache Society Evidence Assessment of Migraine Pharmacotherapies
The study aims to provide an updated assessment of the evidence for individual pharmacological therapies for acute migraine treatment. Pharmacological therapy is frequently required for acutely treating migraine attacks. The American Academy of Neurology Guidelines published in 2000 summarized the available evidence relating to the efficacy of acute migraine medications. This review, conducted by the members of the Guidelines Section of the American Headache Society, is an updated assessment of evidence for the migraine acute medications. A standardized literature search was performed to identify articles related to acute ...

Butalbital and Pediatric Headache: Stay off the Downward Path.
Abstract Despite limited evidence from the literature surrounding safety or efficacy, butalbital-containing medicines (BCMs) have maintained their rank as "go-to" prescribed migraine and headache relief drugs in the United States, despite bans on these barbiturates in Germany and other European countries. Providers at the Pediatric Headache Program at Boston Children's Hospital recommend that clinicians prescribe triptan-based medications instead of BCMs, given the known negative side effects of BCMs on the general population, and the uncertain longitudinal trajectory of BCMs on developing brains. ...

Evaluating the safety and tolerability profile of acute treatments for migraine
Publication date: March 2005 Source:The American Journal of Medicine Supplements, Volume 118, Supplement 1 Author(s): Vincent T. Martin , James A. Goldstein Among the medications that have been used as acute treatments for migraine are nonspecific agents, including nonsteroidal anti-inflammatory drugs (NSAIDs), analgesics (either single or combination), and narcotics, as well as migraine-specific medications, including ergot alkaloids and triptans (5-hydroxytryptamine 1B/1D agonists). All of these drugs have side effects that vary in type and severity. Side effects of nonspecific medications, including gastrointestin...
