Chamber enlargements on ECG
Atrial enlargement  The normal P wave has a maximum amplitude of 2.5 mm (0.25 mV) and a maximum width of 2.5 mm (100 ms). In right atrial enlargement, the amplitude increases while in left atrial enlargement, it is the width (duration) which increases. Both increases in biatrial enlargement. P wave abnormalities are best assessed in leads II and V1. Normal P wave is upright in lead II. In V1, a tiny initial spike is followed by a shallow negative wave. P mitrale: P mitrale is a notched and broad P wave with taller second peak indicating left atrial enlargement. It may be noted that initial part of P wave is contributed b...

General examination – eyes and facial dysmorphism
General examination – eyes and facial dysmorphism Clinically examination is guided by the symptoms. Unless the history is not correlated with findings, important diagnostic possibilities may be missed. Clinical examination starts off with a focused general examination followed by a detailed examination of the cardiovascular system. Relevant points in other systems like basal crepitations, hepatosplenomegaly and neurological deficits should be looked for. Examination strategy should be fitting to the clinical situation. When a patient presents to the emergency room, it should be a short but focused examination to perm...

Pulmonary atresia with VSD : Does routine uni-focalisation strategy justify all the trouble ?
Pulmonary atresia with VSD is one of the complex CHD subsets that requires a meticulous understanding of anatomy, physiology of pulmonary circulation. It can be termed as TAPAC -Total anomalous pulmonary arterial connection in extreme cases. Should we attempt to reverse this total chaotic pulmonary blood supply is the question? It demands a highly focused cath study(hands & brain) and CT Imaging which might actually throw more light. Post-study Interaction with surgeon and team of cardiologist are vital. The decision to take up the challenge of surgery or abandoning poses equal intellectual stress. Continuous and criti...

Osia, a New Type of Implantable Hearing Solution by Cochlear, Secures FDA Clearance: Interview
Earlier this month, Cochlear received FDA clearance for the Osia 2 System, the first active osseointegrated steady-state implant (OSI). The Osia System represents a new type of bone conducting hearing solution that uses digital piezoelectric stimulation to transmit sound vibration directly to the inner ear. While present in other auditory equipment and some medical devices, piezoelectricity has not previously been used in this type of hearing implant. The Piezo Power transducer allows the Osia System to transmit without any moving parts and avoids the use of natural hearing systems which may be damaged or functioning impro...

Segmental bronchial atresia-Case Report
Discussion –-       Bronchial atresia usually is benign and asymptomatic and is incidental finding& clinical manifestations may range from recurrent pulmonary infections to mild wheezing and dyspnea. Bronchial atresia is a congenital abnormality resulting from focal interruption of a lobar, segmental, or subsegmental bronchus with associated peripheral mucus impaction (bronchocele, mucocele) and associated hyperinflation of the obstructed lung segment. The apicoposterior segmental bronchus of the left upper lobe is most common site to be involved, followed by segmental bronchi ...

Will IVF affect my future reproductive cycles?
One of the biggest concerns patientshave with IVF is that when IVF doctors make them grow so many eggs during the IVF cycle, that this will affect the remaining eggs in their ovary .Theyare worried that if they're left with fewer eggs, because the doctor has taken somany out, they will reach menopause more quickly , becausetheir ovarian reserve has been reduced. This is not true.Remember that youstart to produce about 40 follicles in every menstrual cycle. Of these , 39 will die naturally in the bedroom every month, whether or not you do anything. This is a natural process , called follicle atresia, ...

Tricuspid Atresia: DAMS Unplugged
Tricuspid atresia is a form of congenital heart disease whereby there is a complete absence of the tricuspid valve. Therefore, there is an absence of right atrioventricular connection. This leads to a hypoplastic or absent right ventricle.Famous Radiology Blog http://www.sumerdoc.blogspot.com TeleRad Providers at www.teleradproviders.com Mail us at sales@teleradproviders.com (Source: Sumer's Radiology Site)

A life moment you dare not dream of
As a third-generation physician, I grew up thinking and dreaming of a being a physician, and 33 years later, I am still living the dream.  I dreamt of saving people’s lives.  I dreamt of a day like today when I received a LinkedIn request from a young lady on whom, 21 years ago, I performed a living donor liver transplant from her mother. Today she is graduating from college and hopes to work in healthcare and would “love to catch up and thank you for everything.”   I dreamt of using my skills to perform liver transplants so children could grow up, parents could enjoy raising their children, and grandparents coul...

Robot Lengthens Disconnected Bowels to Restore a Contiguous Gut
A number of congenital conditions result in missing parts of the gastrointestinal tract, such as esophageal atresia, while a few others can necessitate surgical removal of a part of the bowel that then has to be reconnected. Fragile and precious newborns are perhaps the most common patients for procedures requiring bowel regeneration and reconnection. In terms of esophageal atresia, the current standard of care, known as a Foker process, puts the child into an induced coma so that body movement doesn’t hamper the disconnected ends of a malformed esophagus from being pulled together by sutures attached to the back. N...

The Price of Progress
By ANISH KOKA, MD No one knows who Bennie Solis is anymore. He had the misfortune of being born in the early 1960s marked for death. He had a rare peculiar condition called biliary atresia – a disease defined by the absence of a conduit for bile to travel from his liver to his intestinal tract. Bile acid produced in the liver normally travels to the intestines much like water from a spring travels via ever larger channels to eventually empty into the ocean. Bile produced in the liver with no where to go dams up in the liver and starts to destroy it. That the liver is a hardy organ was a fact known to the ancient Gree...

Midgut Volvulus CT
Discussion by Dr MGK Murthy, Dr Pritam, Dr GA PrasadIntestinal malrotation is described as abnormal positioning of the bowel loops within the peritoneal cavity in the intrauterine life. It is caused by defective rotation of primitive intestinal loop around the axis of SMA during embryogenesis.Midgut volvulus is a complication of malrotation in which clockwise twisting of the bowel around the superior mesenteric artery axis occurs because of the narrowed mesenteric attachment. Degree of twisting is variable& determines symptoms. Severe volvulus = 3& ½ turns, can lead to bowel necrosis.Age at presentation – usuall...
