Army misses recruiting goal by 15,000 soldiers Officials said the Army brought in about 45,000 soldiers during the fiscal year that ended Friday. The article implies that this number is only going to increase in subsequent years and that all services will be affected beginning next year. Difficult neonate case - what would you do?

Tricare Rant
I few months ago I posted a thread about the sudden influx of Tricare patients into my practice. I solicited ideas as to why this happened and if anyone else was experiencing this phenomenon. This post is a follow-up to my previous post and a plea to all those active duty docs seeing Tricare. DON'T DUMP PATIENTS! Has anyone else seen an enormous influx of Tricare in their practices? Over the last several weeks, our practice has noticed that the number of referrals increased almost 10-fold. We are within an hour of a major military post so there is a large local military population. It seems to me that (at least ENT) has stopped seeing all dependents and retirees. Question: Role of ductus arteriosus in transposition of great vessels
In utero, the ductus ateriosus shunts oxygenated blood from the pulmonary trunk to the aorta to maintain systemic circulation. In transposition of great vessels, the aorta is exiting the right ventricle with oxygenated blood. What role does the ductus arteriosus perform in this situation? The left heart+pulmonary circulation is at a higher pressure. Will this lead to atresia of the ductus arteriosus or do the circulating prostaglandins maintain it? (Source: Student Doctor Network)

Case of treatment-resistant ADHD
I've been seeing an 8 year old boy for about the last year for treatment for ADHD, combined type. His history is pretty classic for hyperactivity as well as inattention, with Vanderbilt's completed by both mother and teacher, which are consistent and mostly 2's and 3's. Army Shenanigans
So my buddy is a fellowship-trained surgical subspecialist stationed at a major army MTF. He has two years left with his ADSO and has been stationed at the same MTF for the last 8 years. In the army's infinite wisdom, they are moving him to Bumblef#$%, AK unaccompanied (he is married with 2 children < 12 years of age) where he will not practice any of his skill set.
