Cytology as a screening tool for anal squamous intraepithelial lesion for HIV positive men: 10-year experience in an inner city hospital
Conclusion Screening in high-risk patients detected almost all high- and low-grade squamous intraepithelial lesions, however, anal cytology alone could not predict the degree of dysplasia. It may be prudent to perform anal bx in all atypical anal cytology. Clear guidelines are needed for screening of a high risk population. (Source: Journal of the American Society of Cytopathology)

Routine Endoscopic Surveillance for Local Recurrence of Rectal Cancer is Futile
Conclusions Rectal surveillance at this center was not beneficial. This study supports the recent (2015) change in NCCN guidelines, which no longer recommend routine rectal surveillance, and challenges other society guidelines. Teaser A retrospective review that examines the utility of routine endoscopic surveillance following surgically resected rectal cancer. No recurrences were identified via endoscopy, which supports the most recent change in NCCN guidelines. (Source: The American Journal of Surgery)

High-Resolution Anoscopy
To the Editor I read with concern Dr Fleshman’s commentary in a recent issue of JAMA Surgery. There is little reason for colorectal surgeons to fear high-resolution anoscopy (HRA). On the contrary, there are good reasons to embrace this helpful tool. (Source: JAMA Surgery)

HPV load and anal cytological abnormalities
ConclusionsThe estimated high‐risk HPV viral load is significantly higher in samples with cytological abnormalities than in negative anal samples and may be useful as an adjunct to anal cytology for triage of patients to high‐resolution anoscopy and biopsy. (Source: HIV Medicine)

Effects of Referral Bias on Estimates of Anal Intraepithelial Neoplasia Progression and Regression Rates in a 3-State Markov Model
Abstract: The study aim is to compare anal intraepithelial neoplasia (AIN) progression and regression rates in a cytology inception cohort to estimates based on the subcohort referred for ≥1 high-resolution anoscopies (HRAs). A cytology-based retrospective cohort was assembled including the anal cytology histories and invasive anal cancer (IAC) outcomes of all HIV-infected adults under care between 2001 and 2012. Laser capture microdissection as a tool to evaluate human papillomavirus genotyping and methylation as biomarkers of persistence and progression of anal lesions
Introduction Anal squamous cell carcinoma is preceded by persistent infection with high-risk human papillomavirus (HPV) and the cancer precursor, high-grade squamous intraepithelial lesion (HSIL). Detection of specific HPV genotypes and HPV-related biomarkers may be an option for primary anal screening. However, more data on the natural history of HPV-related anal lesions are required. The outcomes from this study will enhance our understanding of the clinical and biological behaviour of HPV-related anal lesions and inform the development of future HPV genotype and/or biomarker screening tests. Methods and analysis HIV-ne...

Human papillomavirus status, anal cytology and histopathological outcome in HIV‐positive patients
ConclusionAnal cytology as the solitary screening tool for anal cancer fails to detect anal dysplasia in a considerable number of patients. Additionally, HPV typing and possibly further biomarkers might be applied to identify those patients with a higher risk of developing anal carcinoma, in order to monitor them more closely or directly transfer them to HRA. The Addition of High-Risk HPV Testing to Anal Cytology Increases the Identification of Anal Dysplasia in HIV-Infected Patients.
Conclusions: Universal screening for AD in an HIV+ population yielded a high percentage of abnormal findings. Addition of HRH to cytology screening increased positive screens by 24%. Larger studies are needed to determine the ideal screening method. PMID: 26411174 [PubMed - in process] (Source: Connecticut Medicine)

Rectal Optical Markers for In-vivo Risk Stratification of Premalignant Colorectal Lesions.
CONCLUSIONS: We demonstrate the viability of in-vivo LEBS measurement of histologically normal rectal mucosa to predict the presence of clinically relevant adenomas throughout the colon. The current work represents the next step in the development of rectal LEBS as a tool for colorectal cancer risk stratification. Human papillomavirus mRNA testing for the detection of anal high‐grade squamous intraepithelial lesions in men who have sex with men infected with HIV
Currently, screening for anal high‐grade squamous intraepithelial lesions (anal HSIL) relies on anal cytology and high‐resolution anoscopy. Since this approach has limited sensitivity and specificity for detecting anal HSIL, there is increasing interest in the role of biomarkers for predicting anal HSIL. The aim of this study is to evaluate the diagnostic accuracy of HPV E6/E7‐mRNA expression for the detection of anal HSIL in MSM infected with HIV, in comparison to DNA‐HR‐HPV and anal cytology. This cross‐sectional screening study included 101 MSM followed at the HIV‐unit of La Paz University Hospital. Intra...

High Prevalence of Anal Human Papillomavirus-Associated Cancer Precursors in a Contemporary Cohort of Asymptomatic HIV-Infected Women
Conclusions. HIV-infected women with a history of HPV-associated cervical disease are at increased risk for HG-AIN+ and should be offered anal cancer screening. Anal cytology and HPV-16 genotyping had the best screening performance. Anal cytology is easy to perform routinely; it may be the best candidate for screening for HG-AIN among HIV-infected women.
