Effects of Gabapentin on Emergence Delirium in Pediatric Tonsillectomy/Adenoidectomy Patients: A Post-Hoc Analysis
AANA J. 2023 Oct;91(5):380-384.ABSTRACTEmergence delirium (ED) is a significant source of both short- and long-term negative effects in the postoperative pediatric population, most notably following otolaryngology surgeries with an occurrence rate of 17.9%. Gabapentin, a gamma aminobutyric acid agonist, has been used for enhanced recovery in adult patients and for the purpose of decreasing ED in some pediatric patients undergoing strabismus surgery. This secondary analysis examined the effects of preoperative administration of gabapentin on the reduction of postoperative ED in pediatric patients between the ages of 3-18 un...

Adenotonsillar pathology in mucopolysaccharidoses – lysosomal storage predominates in paracortical CD63+ cells
AbstractDespite the adenoids are regularly removed in patients with mucopolysaccharidoses (MPS), the underlying tissue and cellular pathologies  remain understudied. We characterized an (immuno)histopathologic and ultrastructural phenotype dominated by lysosomal storage changes in a specific subset of adenotonsillar paracortical cells in 8 MPS patients (3 MPS I, 3 MPS II, and 2 MPS IIIA). These abnormal cells were effectively detected by a n antibody targeting the lysosomal membrane tetraspanin CD63. Important, CD63+ storage vacuoles in these cells lacked the monocytes/macrophages lysosomal marker CD68. Such a distinct ...

2.8 Age-Dependent Relationship Between Adenotonsillectomy and Subsequent ADHD Diagnosis
ADHD presents symptoms similar to obstructive sleep apnea (OSA), yet OSA is not included in the common practice parameters. This research sought to identify the potential influence of tonsillectomy and adenoidectomy (T&A), a common treatment for OSA, on the development of ADHD. Comparison of Two Anesthetic Regimens on Extubation Time and Postoperative Recovery in Children Undergoing Ambulatory Adenoidectomy: A Retrospective Study
CONCLUSIONS: Remifentanil-pronounced anesthesia was superior to propofol-pronounced anesthesia in children undergoing ambulatory adenoidectomy, given that the former was associated with a faster recovery time from anesthesia without jeopardizing patient safety.PMID:37768264 | DOI:10.1016/j.jopan.2023.06.004 (Source: Journal of Perianesthesia Nursing)

Torus Tubarius Hypertrophy After Adenoidectomy
Conclusions and Significance: TTH is a rare complication after adenoidectomy, which is common in male children (75.0%) and in patients who took adenoidectomy before the age of 5 years (94.4%). TTH can occur as early as 3 months after adenoidectomy. AR and the operation way might have relationships with TTH.PMID:37700607 | DOI:10.1177/01455613231199676 (Source: Ear, Nose and Throat Journal)

Contemporary Management of Pediatric Obstructive Sleep Apnea
Pediatric sleep is a young area in medicine; pediatric obstructive sleep apnea was only identified about 50 years ago. However, understanding pediatric obstructive sleep apnea (OSA) has developed exponentially recently. For a long time, we had only one tool in our armamentarium for this disorder; we had little to offer beyond tonsillectomy and adenoidectomy (TA). Over the last 2 decades, various compelling innovations proved efficacious and were subsequently developed into surgical techniques. As diagnostic and treatment protocols are beginning to take shape, a comprehensive resource for surgical management of pediatric OS...

Posterior Midline Glossectomy
Surgical treatment of obstructive sleep apnea in children who fail tonsillectomy and adenoidectomy, or in those children with small non-obstructing tonsils is performed infrequently. Further evaluation including drug induced sleep endoscopy may uncover residual upper airway obstruction at the tongue base. When this finding is identified, targeted treatment of tongue base obstruction can be performed. A straight forward easy to perform tongue reduction procedure with minimal complications and good clinical outcomes when part of a multi-level approach is described. The Prevalence and Association of Biofilms With Otitis Media With Effusion: A Systematic Review and Meta-Analysis
CONCLUSION: The data presented in this study coincide with the significant role of bacterial biofilms in the pathogenesis of chronic otitis media with effusion. The involvement of bacterial biofilm as a component of the OME pathogenic process can help us to explain why antimicrobial therapy is not always effective in the eradication of the disease process and, also explain the recurrence of middle ear effusion after treatment with tympanostomy tubes either with or without adenoidectomy.PMID:37553806 | DOI:10.1177/00034894231188855 (Source: The Annals of Otology, Rhinology, and Laryngology)
