Welcome to McMedicine, may I please take your order?
A.J. Smith, a pseudonym of course, walked into my office today, unhappily.  Most of her topical medications for acne caused too much irritation.  The ones that didn’t, weren’t working.  The doxycycline caused photosensitivity in the past.  But her friend’s dermatologist gave her isotretinoin, better known as Accutane, and she completely cleared.  As such, that’s what my patient demanded.  There was only one problem.  The degree of her acne didn’t warrant such an aggressive approach.  I explained the risks at length with her, and I explained politely why she wasn’t a candidate.  This fell on deaf ears....

People are going broke because they can’t afford care. It needs to end.
“I need help with my colitis.” “Really? I thought we had things pretty well controlled.” I hadn’t seen her in the better part of a year. I remembered how hard it had been to get her ulcerative colitis into remission. How sick she had been, how miserable her life was. There was a bit of trial and error in the office, followed by a hospitalization for intravenous steroids, then a tapering dose of prednisone and a transition to budesonide, a steroid who’s metabolism greatly limits many of the nasty side effects of prednisone.  And she had lots of prednisone side effects: swollen ankles, blood pressure creeping up...

Are boar bristle brushes better for your hair? Episode 138
Are boar bristle brushes better for your hair? Georgina asks…Are boar bristle brushes better for your hair? I’m looking at a Mason Pearson brush that’s about $100 and I want to know if it’s worth it. It’s tough to give a definitive answer because as you might imagine there aren’t many double blind, peer reviewed scientific studies comparing different hair brushes. But we DID find a couple of studies that may be helpful. The first study, “A Statistical Analysis of Hair Breakage,” pointed out the something that seems obvious: different combs and brushes will affect your hair differently depending on their str...

MKSAP: 32-year-old man with a rash on his face and midchest
Test your medicine knowledge with the MKSAP challenge, in partnership with the American College of Physicians. A 32-year-old man is evaluated for a 2-week history of a rash on his face and midchest. He describes the rash as consisting of small, reddish “lumps” that are intensely itchy; they develop and begin to resolve with development of new lesions. He otherwise feels well. Medical history is significant for a recent diagnosis of HIV infection. Medications are tenofovir, emtricitabine, efavirenz, and trimethoprim-sulfamethoxazole. On physical examination, vital signs are normal. The patient has 1- to 3-mm p...

Can I mix my own sunscreen? Episode 132
How do I find a sunscreen that doesn’t cause acne? Allison asks…What’s the difference between a sunscreen and a sunblock? Also, what ingredients should I look for in a product that will not contribute to acne? And, when’s the best time to apply sunscreen in my morning skin care routine? Sunscreens use UV absorbers to protect your skin while sunblocks use minerals like zinc and titanium compounds to scatter the sunlight and prevent it from reaching your skin. (Actually, the regulations in the US have changed recently and companies are not allowed to call their products sunblocks anymore.) Both types of sun p...

More Legal Trouble Ahead for Horizon Pharma
Horizon Pharma has been in the spotlight for questionable ties to specialty pharmacies and its patient assistance programs. Wheat Belly: Self-Directed Health?
Director chair, film slate and load horn. Here’s a proposal for you: If, by following the Wheat Belly lifestyle, a long list of conditions are reduced or reversed at no risk, almost no cost, reversing even chronic and potentially fatal conditions . . . does that mean that the notion of self-directed health might be on the horizon, i.e., putting control over health back in our own hands? I think it does. No, we will never implant our own defibrillators or take out our own gallbladders. But so many chronic health conditions afflicting modern humans recede that I believe that it is entirely reasonable to start talking a...

Allison says… I have a question about bath soaks. Thanks for the question Allison first of all let’s get one thing clear: taking a bath in anything is not going to detox your body. That’s because your body doesn’t cleanse itself of to...

Sexism in Medical Education
The medical school professor stands affront  a group of first year students in a mid-size auditorium. “I need a go-to guy,” he says, “someone to direct my questions towards.” He scans the room. “I’ve never actually had a go-to girl, before,” he admits. Later in the lecture, he makes a joke at a male student’s expense. “I joke!” he laughs. “Usually I don’t pick on the girls of the class – they can be too emotional – its true! My wife tells me it’s true.” During an exercise aimed at discussing issues of public health, the facilit...
