Pattern of paracetamol poisoning: influence on outcome and complications - Castanares-Zapatero D, Dinant V, Ruggiano I, Willem H, Laterre PF, Hantson P.
Acute paracetamol poisoning due to a single overdose may be effectively treated by the early administration of N-acetylcysteine (NAC) as an antidote. The prognosis may be different in the case of intoxication due to multiple ingestions or when the antidote... (Source: SafetyLit)

Can we improve prognostic indications in acetaminophen-induced hepatic failure?
The objective was to compare the markers of hypoglycemia (glucose < 50 mg/dL,) coagulopathy, and lactic acidosis (lactate above hospital’s “normal” range) in predicting a composite endpoint of death or liver transplantation. The authors identified 334 unique cases. 19 of these were listed for transplant, and 13 actually received liver transplant. (2 transplanted patients subsequently died.) Although 96 patients (29%) met the King’s College Criteria, it is not clear how many of these who were not transplanted died. There were 40 deaths in all. After some statistical manipulation  which was not at...

Is there a better way to administer IV N-acetylcysteine?
3 out of 5 stars Fewer adverse effects with a modified two-bag acetylcysteine protocol in paracetamol overdose. McNulty R et al. Clin Toxicol 2017 Dec 8 [Epub ahead of print] Abstract Current recommendations for dosing intravenous N-acetylcysteine (Acetadote) for treating acute acetaminophen (paracetamol) poisoning involves a somewhat complex three-bag protocol, using 3 direct concentrations of the antidote given over a 21-hour period. Unfortunately, this protocol results in a high incidence of (usually mild) adverse events, including cutaneous effects, nausea and vomiting, and anaphylactoid reactions. Severe reactions s...

Multi-mechanism approach to treating neonatal hypoxic ischemia
(Medical University of South Carolina) Male sex is a risk factor for worse outcome following neonatal insult, including hypoxic ischemia. While N-acetylcysteine and hypothermia promote functional improvement in female rodents, similar improvement in males requires the addition of vitamin D, report investigators at the Medical University of South Carolina in the September 1, 2017 issue of Neuropharmacology. Multi-mechanism approaches are needed to treat neonatal hypoxic ischemia, and those approaches appear to be different depending on sex. (Source: EurekAlert! - Medicine and Health)
