How to measure the largest basal dimension of choroidal melanoma: a mathematical study
Purpose: To identify the most accurate ultrasonographic technique to measure the largest basal dimension (LBD) of choroidal melanoma. Methods: B-scan ultrasound images were retrospectively reviewed in 99 eyes of 99 choroidal melanoma patients. The LBD was measured using one, two, and three straight lines along the inner and outer sclera. Theoretical arc length, calculated using trigonometry formulas based on the spherical model with the axial length as the sphere diameter, was used for comparisons with the actual measurements using straight lines. Results: For straight-line measurements in the inner sclera, the lowest error was found when using two straight-line measurements (P = 0.118). Uveal melanoma (UM), the most common primary intraocular tumour, is often complicated by exudative retinal detachment (RD). Sometimes, this exudative RD is mistaken for a rhegmatogenous detachment and is subsequently treated with vitrectomy with silicone oil (SiOil) tamponade. Conclusion: Swept-source optical coherence tomography angiography represents a valid imaging technique to evaluate patients affected by choroidal melanomas. In our series, SS-OCT-A disclosed the intrinsic microvasculature of the tumor in all cases despite their size, location, and history of previous treatments.
CONCLUSION: Optical coherence tomography angiography can provide useful information for assessing and differentiating between choroidal nevi and small melanomas. Significant differences between these conditions were found for the pattern of reflectivity, and presence/absence of avascular zones and vascular anomalies, which could be helpful for supporting the diagnosis. CONCLUSIONS: Whole-body PET/CT and abdominal ultrasound complement each other in the staging of uveal melanoma. Benign hepatic abnormalities found using ultrasound is common. Of importance, a second asymptomatic primary malignancy associated with uveal melanoma was detected almost one in 10 patients.
Conclusions: New multiple enhancing solid liver lesions should raise suspicion of uveal melanoma liver metastases. CONCLUSIONS: Slit lamp biomicroscopy is sensitive for detecting anterior ESE. Most posterior ESE is microscopic, but macroscopic posterior ESE may also be missed by B-scan ocular ultrasound. Orbital surgeons should be suspicious of clinically undetected posterior ESE, and consider adjuvant orbital radiotherapy in cases with macroscopic ESE. Conclusion: Current literature suggests the majority of choroidal melanoma recurrences occur within 5 years following treatment. However, this case of recurrence 10 years after brachytherapy emphasizes the importance of life-long ophthalmic care for these patients.
