Ventral striatal and septal area hypermetabolism on FDG-PET in herpes simplex viral encephalitis

AbstractA 71-year-old man presented with sudden onset, generalized tonic-clonic seizures and altered mental status. Initial brain magnetic resonance imaging was normal but a brain FDG-PET scan showed hypermetabolism in the left ventral striatum and septal area. Initial cerebrospinal fluid (CSF) examination showed mildly elevated protein but herpes simplex virus (HSV) polymerase chain reaction (PCR) was negative. A repeat CSF examination performed 9  days later showed a positive HSV PCR. Histopathological and immunohistochemical examination of autopsy specimen confirmed the presence of CD45+ lymphocytes and HSV antigen, suggesting the presence of both inflammation and viral infection corresponding to PET abnormality.

Her polymorphic psychotic symptoms first occurred after she had received a Tdap-IPV booster vaccination. AbstractThis case report presents a 1-year-old boy from China, with sudden onset of fever, convulsion, and sleepiness, screened for viral DNA in blood and cerebrospinal fluid (CSF) sample using next-generation sequencing (NGS) to diagnose herpes simplex virus type 1 (HSV-1) encephalitis, further validated by PCR. Central nervous system (CNS) infections are one of the leading causes of acquired epilepsy [1]. Patients with encephalitis have a risk of developing seizures at the acute stage, as well as later developing unprovoked seizures [2]. It is well recognized that the risk of developing unprovoked seizures in these patients is 7 to 16 times higher compared to the general population [3,4], especially in those with Herpes simplex virus and Japanese encephalitis (JE), and the odds ratio of subsequent epilepsy was reported to be 8.06 for JE [5].

We describe a patient with acute herpes simplex encephalitis with left-hemispheric hippocampal, parahippocampal and insular lesions. Although prototypic language areas were unaffected, the patient suffered from an inability to name objects or animals displayed on pictures. This deficit was transient and gradually disappeared 8 weeks after the initial diagnosis.
