Human parainfluenza virus type 3 infections in hematopoietic stem cell transplantation patients: The mode of nosocomial infections and the prognosis. Jpn J Infect Dis. 2017 Dec 26;: Authors: Kakiuchi S, Tsuji M, Nishimura H, Wang L, Takayama-Ito M, Kinoshita H, Lim CK, Taniguchi S, Oka A, Mizuguchi M, Saijo M Abstract   There have been few prospective and comprehensive surveillance studies on the respiratory viral infections (RVI) among patients undergoing hematopoietic stem cell transplantation (HSCT). A 2-year prospective cohort surveillance study of symptomatic and asymptomatic RVIs in hospitalized HSCT patients was performed. Oropharyngeal (OP) swab samples were serially collected each week from 1 week before and up to 100 days after HSCT and were tested for virus isolation with cell culture-based viral isolation (CC-based VI) and a multiplexed PCR (MPCR). 2,747 OP swab samples were collected from 250 HSCT patients (268 HSCT procedures) in total. Overall, 79 RVIs were detected (CC-based VI, n=63; MPCR, n=17). Parainfluenza virus type 3 (PIV3) accounted for 71% (57/80) of the RVIs. Some PIV3 infections were asymptomatic and involved a longer virus-shedding period. PIV3 was often cultured from samples taken before the onset of respiratory disease. The PIV3 infections were likely attributable to nosocomial transmission. PIV3 infections before engraftment were more likely to result in lower respiratory tract infections and a worse ...
