Pulmonary aspergilloma

Pulmonary Aspergillosis is a fungal infection most often caused by Aspergillus fumigatus which grows on a pre-existing cavity is single or multiple. The aim of our study was to determine the epidemiology, diagnostic and therapeutic means. We conducted a retrospective study between January 2000 and December 2013 on 64 cases. Males accounted 38cas (59%) with a mean age of 43 years. Aspergilloma had occurred on pulmonary tuberculosis sequelae in 58 cases (91%) after a mean period of 14 years. It occurred on pulmonary hydatid cyst in two cases and two patients were already operated aspergilloma. Hemoptysis was the primary clinical manifestation in 56 cases (87.5%), followed by bronchial suppuration in 46 cases (72%). Chest radiography objectified typical bell image in 19 (30%). Chest CT objectifying a picture bell Trendelenburg and mobile in 38 cases (59%). Aspergilloma was multiple and bilateral in 13 cases (20%), sitting in the upper lobes 54 cases (85%). The aspergillus serology was positive in 42 cases (66%). Culture on Sabouraud medium of bronchial aspirate was used to isolate the fungus in 7 cases (11%). Surgical treatment was initiated in 43 cases (67%), 4 (6%) cases of multiple aspergilloma. Medical treatment was prescribed in other cases, but the results were not satisfactory. The single or multiple pulmonary aspergilloma poses a problem making therapeutic management.
